[Role of surgical deobstruction in massive pulmonary emboli. Review of 12 operated cases].

A brief survey is made of a personal series of 12 embolectomies performed under varying clinical conditions for the management of massive pulmonary embolism. Four satisfactory results were achieved. This operation is carried out under CEC and is a sound procedure. Stress is laid on its fundamental importance in the treatment of cases of this kind. Its early choice should be made in the light of the clinical picture.

Discussion on the causes of failure of surgical treatment of partial epilepsies.

Despite continuous improvement in diagnosis and surgery, persistence of seizures following surgical treatment of partial epilepsies still occurs in a relevant number of cases (30--40%). The analysis of personal material and of data from the literature appears to indicate that relevant causes of surgical failure are difficulties in delimitation of the epileptogenic zone and therefore of complete surgical removal. These difficulties are illustrated and discussed.

Anatomo-clinical correlations in normotensive hydrocephalus. Reports on three cases.

The brains of 3 adult subjects suffering from normotensive hydrocephalus have been examined pathologically. The diagnosis of normotensive hydrocephalus was based on clinical symptoms, pneumoencephalography and isotope cisternography, in 1 case integrated with the results of the constant-infusion manometric test. Part of the neuropathological findings were common to the 3 patients: leptomeningeal non-obstructive fibrosis, ventricular ependymal disruption, subependymal glial reaction, periventricular demyelination and spongiosis. Other neuropathological abnormalities were peculiar to each patient: leptomeningeal signs of previous subarachnoid haemorrhage; arteriosclerosis and multiple brain cystic infarcts; Alzheimer's plaques in the gray matter. The possible pathogenetic significance of the neuropathological findings summarized above in relation to the development of normotensive hydrocephalus is discussed.

The study of cerebrospinal fluid dynamics in apparently 'arrested' hydrocephalus in children.

18 children having hydrocephalus, which could be regarded clinically as 'arrested', underwent cerebral angiography and a constant infusion manometric test. Intracranial pressure was directly recorded in 3 cases. The above examinations indicated that hydrocephalus was still 'active' in 8 children. This was confirmed by the good results of the surgical treatment.

Cerebral Ventriculography↗

"Chronic" electrodes in SEEG exploration of partial epilepsies.

The advantages of the stereoelectroencephalographic study of epileptic patients prior to surgical treatment through "chronically" implanted multielectrodes are illustrated and discussed. The reliability of the information provided by the cerebral electrical activity is strengthened by recording the patient on different days, in different physiological conditions and under different pharmacological treatment. The consistency of some of the electrical abnormalities recorded gives them a strong diagnostic value.

Surgical indications in normotensive hydrocephalus. A retrospective analysis of the relations of some diagnostic findings to the results of surgical treatment.

Twenty two of fifty five patients initially suspected of suffering from normotensive hydrocephalus were surgically treated (CSF ventriculoatrial shunt). The results of surgery were related to the findings of the different diagnostic examinations (pneumoencephalography, isotope cisternography, transfer fron CSF to blood of isotope labelled serum albumin, constant infusion manometric test, long lasting intraventricular pressure recording). 1.Pneumoencephalography, intraventricular pressure recording, and, above all, isotope cisternography provided the most reliable data for diagnosis and surgical prognosis. 2. The combined use of pneumoencephalography and isotope cisternography was sufficient for a correct diagnosis and surgical prognosis in about 50% of the patients examined. 3. When the diagnostic information given by the combined results of the two above examinations was not sufficient, intraventricular pressure recording proved to be the most useful examination to supplement it. 4. The relation of the results of the study of transfer of istope labelled serum albumin from CSF to blood and of the constant infusion manometric test to the surgical outcome was uncertain.

Interaction of epileptic activities of bilateral deep temporal origin. An experimental study.

The interaction of epileptic activities obtained with electrical or chemical (penicillin) stimulation of the amygdaloid nuclear complexes of the two sides was studied in acute experiments on the rabbit. Several findings indicate that reciprocal influences between the two epileptogenic amygdalae can occur. Inhibitory as well as facilitatory interactive phenomena were observed. The occurrence of interaction and its sign seem related to the level of epileptic activity.

Amygdala↗

Experimental bilateral deep temporal epilepsy. Effects of ablation of one focus and of different brain lesions.

In acute experiments in the rabbit, the amygdaloid nuclear complexes of the two sides were made epileptogenic through electrical stimulation or local injection of penicillin in gel. The effects on the epileptic pattern produced by surgical removal of one of the two epileptogenic amygdalae and the effects of sterotactic lesioning of the region of anterior commissure, head of caudate nucleus, and occipital cortex were analyzed. The occurrence of phenomena of both inhibitory and facilitatory interaction between the two epileptogenic amydalae was confirmed. In some experimental conditions, the restraining influence of an amygdaloid penicillin focus on the contralateral one was quite relevant, and its effect could persist even after surgical removal of the focus from which it originated. The mediation of the interamygdadoid epileptic interaction could not be ascribed to a single cerebral structure or anatomofunctionally homogenous group of structures. The phenomenon appears to involve several structures at different encephalic levels.

Cerebrospinal fluid pressure studies in normal pressure hydrocephalus and cerebral atrophy.

The surgical results obtained with a cerebrospinal fluid (CSF) shunt operation in 13 patients considered to be suffering from normal pressure hydrocephalus have been correlated with the findings of the constant manometric infusion test (IT) and of prolonged intracranial pressure recordings. A positive IT, high amplitude CSF pulse pressure and large transitory increases of CSF pressure during sleep seem to be useful criteria for the surgical prognosis. Ten more patients affected by primary cerebral atrophy have also been studied. The data obtained in both groups of patients have been utilized for a possible pathogenetic interpretation of the syndrome.

Considerations on the principles of surgical treatment of partial epilepsy.

Some considerations on the principles of surgical treatment of partial epilepsy are prompted by the still relatively high number of unsatisfactory surgical results. The fundamental principle of surgical treatment, based on the view that partial epilepsy is due to the hyperactivity of given neuronal aggregates consequent to a cerebral lesion (the lesional-functional epileptogenic complex), is still regarded as satisfactory. The present concept of the lesional-functional epileptogenic complex and the means to topographically locate and delimit it are illustrated and discussed. Surgical failures are ascribed mainly to the difficulty in reaching a complete and detailed spatial definition, and then a complete removal, of the epileptogenic complex.

Adaptation of the mitochondrial systems of Rhodotorula gracilis to low oxygen pressure.

The obligate aerobic yeast, Rhodotorula gracilis, was grown in a liquid minimal medium at 1 mm Hg partial pressure of oxygen, conditions under which growth (measured as the increase in total protein) is reduced to 30% of the maximum rate. A significant increase in the ratio between mitochondrial oxidative enzymes and total protein occurs rapidly under these conditions. A concurrent increase in the ratio area of mitochondria/area of cytoplasm was also observed. The relative increase in mitochondrial enzymes, oxidase activities and mitochondrial membranes is due to the inhibition affecting the increase in cytoplasmic structures more significantly than the increase in mitochondrial structures at low pO2. The difference between mitochondrial and cytoplasmic syntheses cannot be ascribed to changes in the availability of ATP but it might rest with some other oxygen-utilising process (pyrimidine redox coenzymes, synthesis of sterols). The experimental conditions studied appear to offer a valuable tool for the investigation of the relationships between mitochondrial and cytoplasmic structures.

[Research on the epileptogenic complex].

The great importance of a correct definition of the lesional-functional epileptogenic complex for the surgical treatment of the epileptic patient is stressed. The means utilizable to reach the knowledge of the topographic organizatif the latters are descri0ed and discussed in detail: the scalp EEG, the examinations based on the EEG effects of endocarotid injection of barbiturates and convulsants, the EEG during nocturnal sleep, the direct recording of electrocerebral activity from the cortical surface (electro-corticography) and from deep cerebral structures (stereoelectro-encephalography). The relative importance of the informations provided by the electrical activity of "lesional" type, by the interictal epileptic activity and by the ictal one is analyzed. The utilization of methodologies of automatic elaboration of the electrocerebral signals is reminded. Finally, the different modes of application of the means of analysis mentioned above in the different epileptic patients are discussed and exemplified.
