We report on the use of a modified Schiotz tonometer to evaluate the effects of therapies on burn scars. It is now possible to quantify the course of cicatrization with tonometry. Because tonometry affords a precise evaluation of burn scar diagnosis and prognosis, it is possible to avoid arbitrary clinical evaluation of the burn scarring process.
In a controlled double blind trial the effectiveness and tolerability of Kallikrein in patients suffering from astheno-oligozoospermia was examined in comparison to placebo. 30 patients with astheno-oligozoospermia of idiopathic origin entered the study; they were subdivided into two homogeneous groups of 15 patients each. Group A was treated with 600 K.U. daily for 3 months, group B received placebo tablets. In the Kallikrein group all parameters improved, as sperm motility, volume of semen, sperm concentration, total sperm value, and rate of normal spermatozoa. No changes of endocrinological findings were observed. Three pregnancies occurred in this group. No conceptions were observed in the wives of those patients treated with placebo (group B). Tolerability was very good in all patients.
CA 15-3, TPA and CEA were assayed before surgery in 60 patients with breast cancer. A significant association was found between preoperative CA 15-3 levels and some of the most important prognostic factors in breast cancer, such as lymph node status and tumor size. No similar association was discovered for CEA and TPA. Preoperative CA 15-3 levels were also significantly associated with early recurrences of the disease, thus adding useful information to prognosis especially in N+ patients.
In order to evaluate whether changes of adrenal steroid serum levels occurring during the prepubertal period influence the degree of osteopenia, dehydroepiandrosterone sulfate (DHEAS) was longitudinally monitored as marker of the onset of adrenarche. Fifteen thalassemic patients, 9 girls (Group 1) and 6 boys (Group 2), with chronological age (CA) from 6.1 to 10.3 years and bone age (BA) from 6 to 9.6 years, were studied. Two observations, 14 months apart, were made. All patients had no pubertal signs according to Tanner during the period of observation, and auxological data (height, BMI and height velocity) were evaluated in respect to bone age. There was a statistically significant difference between the two groups for serum DHEAS, BGP serum levels, height velocity and bone mineral density expressed as standard deviation score (BMDsds) in respect to both bone age and height age (the latter was calculated in order to eliminate the influence of height on BMD). Alterations of bone metabolism were excluded by determination of calcium, phosphorus, alkaline phosphatase activity, parathormone, 25-OH-D3, IGF-I serum levels, all these values being normal. In conclusion, our data show that a delayed adrenarche occurs in thalassemic boys which is correlated with a severe degree of osteopenia, even though the relationship between them is not yet established.
UNLABELLED: It is known that in thalassemic patients there is a disproportion between lower and upper segments whose causes have not yet been identified. We evaluated whether the administration of estrogens to induce puberty in hypogonadic thalassemic girls caused an inappropriate acceleration of bone maturation and whether this had a negative influence on final and sitting height. MATERIALS AND METHODS: Twelve thalassemic patients with spontaneous puberty (Group A) and seven patients with hypogonadism (Group B) were studied. The mutations of the beta gene were identified by DNA analysis. We took into account four observations, ranging from the onset of spontaneous puberty in group A or the start of substitutive therapy in group B, to 5 years later. At each observation we considered: chronological age (CA), bone age (BA), height (Ht) expressed in cm and as standard deviation score (HtSDS) calculated with respect to CA (HtSDSCA) and BA (HtSDSBA), growth spurt, sitting height, expressed as SDS (SH-SDS), and height gain (HG). The delta BA and delta CA were calculated between the first and the final observation values to evaluate the bone age acceleration (delta BA/delta CA). RESULTS: No acceleration of BA was noted. delta BA/delta CA was 0.98 +/- 0.1 in group A and 0.89 +/- 0.1 in group B (p > 0.05). All patients in group B had the most severe form (beta degree/beta degree) of thalassemia. During the final observation, SH-SDS was -1.43 +/- 1.2 and -2.9 +/- 0.6 in group A and B respectively (p < 0.002), while no difference between the two groups for HtSDSCA and HtSDSBA was observed. HG was greater in group A than in group B (17.7 +/- 5.4 cm vs 10.8 +/- 5.2 cm) (p < 0.002), such as the spurt 8.6 +/- 1.4 cm (group A) and 6.1 +/- 2.6 cm (group B) (p < 0.05). CONCLUSIONS: Girls with hypogonadism did not show an inappropriate acceleration of BA, as they reached near final height similar to girls with spontaneous puberty. The auxological parameters showed a more severe body disproportion with the prevalence of the lower segment in the hypogonadic girls. This could be explained by a higher degree of bone marrow hyperplasia related to the most severe form of thalassemia and a higher blood consumption. As a consequence, damage at the vertebral level might determine an inability of the bone tissue to respond to estrogens. We suggest beginning estrogen therapy earlier in order to obtain better truncal growth.

BACKGROUND: Carboplatin is a milestone drug against ovarian carcinoma; it is used both in front-line and second-line chemotherapy. Hypersensitivity reactions to carboplatin may occur during the treatment as salvage therapy. The purpose of this study was to describe the feasibility of the replacing of carboplatin with cisplatin in patients presenting with severe hypersensitivity reactions to carboplatin. PATIENTS AND METHODS: Ten consecutive patients with platinum-sensitive, recurrent ovarian carcinoma, presenting with moderate/severe hypersensitivity reactions to carboplatin were treated with cisplatin 60 mg/m2 from January 2000 to December 2002. Hypersensitivity reactions consisted of respiratory distress (chest tightness, wheezing, dyspnea), urticaria/erythema with tachycardia, facial swelling and hypotension. RESULTS: The total number of cisplatin cycles given was 44 (range 2-5). The treatment with cisplatin was generally well tolerated. No serious allergic reactions occurred. A mild allergic reaction was recorded (urticaria) in only one case, after one cycle of cisplatin, and the patient was not rechallenged because of progressive disease. No reductions of chemotherapy doses were needed. CONCLUSION: To date, platinum-based regimens remain the most effective treatment in recurrent platinum-sensitive ovarian cancer with a high rate of objective responses. Although our experience is limited, we suggest that, under anesthesiologic surveillance and providing immunologic blockade, the replacement of carboplatin salvage therapy with cisplatin can be considered a safe therapeutic strategy in patients who cannot continue carboplatin due to allergic reactions.

A biochemical-metabolic study has been conducted on TCM (4-carbomethoxythiazolidin hydrochloride) in rats, utilising a product traced with 14C. An in-vitro test has demonstrated that TCM is hydrolised by endogenous esterases to thiazolidincarboxylic acid, into the following decreasing order of activity: kidneys, lungs, plasma, liver, intestinal content and mucosa. Upon oral administration, TCM proved to have a good and rapid absorption and distribution level in the organs. In the beginning the liver and the kidneys have displayed higher concentrations with respect to the lungs; after 8 hours, however, the latter have displayed slightly lower concentrations than before, which were not very far from the liver's and kidneys'. TCM has been mainly eliminated through the urines (56.9% in 24 hours) and less through the feces (6.7% in the same period of time). By means of a thin-layer chromatography analysis and a mass-spectrometry it has been possible to study the metabolism of TCM. The results of both in-vitro and in-vivo tests have demonstrated that TCM is hydrolised to thiazolidin-carboxylic acid, which, in its turn, is transformed into cysteine. This last transformation does mainly occur in the liver, but significantly in the lungs as well. TCM is absorbed in part as such and in part as thiazolidincarboxylic acid. As no methyl-cysteinate has been traced, it has been assumed that the hydrolysis of the ester bond was a kinetically quicker event than the heterocycle opening.
Tumor Associated Glycoprotein 72 (TAG-72) is common to most epithelial tumors. In this study serum levels of TAG-72 were measured in 36 healthy female subjects, in 94 patients with breast cancer, and in 43 others with epithelial ovarian cancer. More particularly, 27 out of the 94 patients with breast cancer had early disease (Stage I-I; or T 0-2b; N 0-1b; M 0), while the remaining 67 had advanced disease (Stage III-IV; or T 0-4; N 0-3; M 0-1). All the 43 subjects with ovarian cancer were at stage III-IV (FIGO Classification), 12 of whom had minimal disease (lesions less than 2 cm) and the other 31 had bulky disease. 3.5 U/ml being the highest value found in the control group, we arbitrarily assumed 3.85 U/ml (mean +/- 3 SD) as the cut-off limit in this preliminary study. Among the patients with breast cancer, 17 out of the 67 subjects with advanced disease (25.3%) and 1 out of the 27 others with early disease (3.7%) exceeded the TAG-72 cut-off limit. Among the patients with ovarian cancer, 2 out of the 12 subjects with minimal disease (16.7%) and 22 out of the others 31 with bulky disease (70.9%) had TAG-72 levels above cut-off limit. High TAG-72 levels were found in all the histotypes of ovarian cancer including the mucinous type. Preliminary data seem to indicate that in ovarian cancer variations in serum levels of TAG-72 are in agreement with the trend of the disease.(ABSTRACT TRUNCATED AT 250 WORDS)
