[The ileal neobladder].

Currently we are witnessing an increase of interest in bladder substitution enterocystoplasty. The goal of this presentation is to describe operative technique and first clinical results of a ileal neobladder for total bladder replacement. Creation of a ileal neobladder for total bladder replacement is described in 11 patients. To achieve a low pressure system, disruption of directional bowel peristalsis with a longitudinal incision at the antimesenteric border of a 70 cm ileal segment is performed. A spherical pouch, the neobladder, is fashioned and anastomosed to the urethra. The ureters are implanted according to Le Duc and Camey. Videourodynamic studies during various postoperative phases demonstrate this neobladder to be a urinary reservoir with a capacity approximating that of a normal bladder, good compliance during filling by maintaining pressures lower than 30 cm water and no reflux. Eight of the 11 patients with the neobladder are completely dry day and night. Three are stress incontinent grade I. All 11 patients developed recognizable sensations of bladder distension closely simulating those of their earlier bladders. The use of this ileal neobladder in male patients undergoing radical cystectomy offers a stoma free alternative to urinary diversion resulting in a highly compliant low pressure bladder. A urodynamic comparison between the most recent developments (Kock-pouch, Camey, Mainz-pouch, "Le Bag") is made and this clearly shows the ileal neobladder to have several distinct advantages: the ileocolonic junction and the terminal ileum are saved; the operative technique is safe, simple and reliable (no Kock valve, no 180 degree rotation of the reservoir, simple antireflux technique); development of recognizable sensations of bladder filling in all cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[Nonbulky nonseminomas: N-staging value of image-producing study procedures with reference to possible surveillance management].

In a retrospective study 72 patients with non-bulky nonseminomas were investigated in order to determine the value of computed tomography, sonography and lymphography in the detection of lymph node metastases. The diagnostic results were compared with the histological findings of the retroperitoneal lymph node dissection. In our evaluation computed tomography (n = 48) had an accuracy of 0.77, a sensitivity of 0.62 and a specificity of 0.89; sonography (n = 40) had an accuracy of 0.66, a sensitivity of 0.47 and a specificity of 0.81; lymphography (n = 54) had an accuracy of 0.56, a sensitivity of 0.59 and a specificity of 0.53. Our own results were compared with the results published elsewhere.

[Diagnosis of reflux in children with CO2 and ultrasound. Technic and preliminary results].

So far diagnosis and follow-up of vesico-uretero-renal reflux (VUR) is primarily based on radiological voiding studies. However this is accompanied by significant gonadal radiation doses. The goal of this study was to compare the diagnostic accuracy of real time sonography, following CO2 instillation into the bladder with that of conventional radiography in the diagnosis of VUR in children. By means of sonography 13 out of the 16 grade III, and all 16 grade IV and V VUR were seen. 4 out of 7 grade II VUR could be demonstrated. No grade I reflux has been diagnosed. In conclusion, sonography reflux study is a sensitive and specific screening and follow up procedure for VUR. At present 75 per cent of all VUR's can be diagnosed by sonography alone.

[Parameters of incurability in urinary bladder cancer].

Curative treatment of bladder cancer is limited to superficial tumors (Ta-T2). Advanced stages (T3a + T3b) can be treated only palliatively even when confined to the bladder (N0, M0). Nevertheless the differentiation between curable and incurable tumors is difficult. The classification of patients into the two different categories is usually done on the basis of tumor infiltration and lymph node involvement. In this retrospective study all available data of patients with bladder cancer were evaluated for their prognostic reliability in order to obtain a safe basis for further therapeutic approaches. The principal aim was to define parameters of reliable prognostic significance to prevent under- or overtreatment during the entire course of the disease.

Bulky germinal tumors: comparison of different induction regimens and significance of residual disease.

In order to study the efficacy of three different chemotherapy regimens and the prognostic significance of residual disease after chemotherapy, we analyzed 84 patients with bulky germinal tumors. Chemotherapy, consisting of vinblastine, bleomycin/adriamycin, cisplatin (VB/AP) was administered sequentially to 37 patients. 19 patients were treated with the cisplatin, vinblastine, bleomycin (PVB) regimen and 17 patients with cisplatin, vinblastine, bleomycin, ifosfamide (PVBI). The initial complete remission (iCR) was 71% (52 of 73) with a relapse rate of 29% (15 of 52). 10 of 73 (14%) patients achieved partial response and 11 of 73 (15%) showed progression due to multidrug resistance. There was no statistically significant difference in iCR, continuous complete remission (cCR), relapse and survival between the PVB, PVBI and VB/AP regimens. In 50 patients, the residual tumor after chemotherapy was examined histologically. cCR was achieved in 75% (15 of 20) of patients with necrosis or fibrosis, in 50% (5 of 10) with adult teratoma and in 25% (5 of 20) with malignant tumor. Comparison of survival curves according to residual histology revealed statistically significant differences (p = 0.02). Our findings suggest that in patients with germinal bulk disease PVBI-regimen and VB/AD-therapy are not superior to the standard regimen. Residual disease seems to be an important prognosticator.

[Therapy of stress incontinence recurrence: comparison of fascia sling-plasty and Zoedler band in 117 patients].

We report on 117 patients with urinary stress incontinence, 44 with previous attempts at surgical correction. The inguinovaginal fascial sling operation as modified by Narik and Palmrich was used in 33 women and had an early success rate of 91%. The Zoedler method used in 84 women was successful in 87%. 52% of women with fascial sling procedure (mean duration of follow-up 1.1 year) and 45% with Zoedler method (mean duration of follow-up 5.7 years) stayed completely continent. The most common reasons for failure are discussed.

[Bladder duplication. A case report].

Duplication of the bladder is uncommon and rarely occurs as an isolated anomaly. This anomaly is classified as complete duplication, incomplete duplication, complete sagittal septum and incomplete sagittal septum. We present the case of a 7 month old child which had recurring pyuria.

[Panarteritis nodosa--a urologic problem?].

A case of panarteritis nodosa with renal and intraabdominal symptoms is described. Diagnosis of this visceral form of this rare disease is extremely difficult, and in the absence of subcutaneous granulomata can only be made on angiography. Depending on the site affected, visceral disease can lead to life threatening internal haemorrhage which in most cases has to be treated surgically.

[Familial occurrence of testicular neoplasms (author's transl)].

Testicular malignancies in closely related family members are rare, 22 cases have been published before. Our report describes the eleventh case of testicular tumors in non-twin-brothers and the first case with histologically verified bilateral tumor in one brother. We also report about another case of testicular neoplasms in identical twin brothers.
