Experimental non-A, non-B hepatitis in chimpanzees: light, electron and immune microscopical observations.

Two chimpanzees (Pan troglodytes) were inoculated and cross-challenged with a fibrinogen and factor VIII preparation, respectively. Successful non-A, non-B (NANB) infection was documented by biphasic elevations of aminotransferases (ALT), concomitant hepatitic reactions and typical electron microscopic alterations, the most consistent being dilatation of the endoplasmic reticulum, as well as tubular and sponge-like cytoplasmic inclusions in the absence of nuclear virus-like particles. An anti-nuclear (anti-DNA) antibody of the IgM class in one of the chimpanzees simulating an antiviral antibody is described.

Effects of serial resin hemoperfusion in fulminant hepatic failure.

Resin hemoperfusion using an albumin coated Amberlite XAD-7 column was performed in 19 patients in coma due to fulminant hepatic failure. The procedure was clinically well tolerated, with good blood compatibility, platelet and white cell levels being 97.3 +/- SE 3.2% and 105 +/- 3.8% of the respective initial values after four hours hemoperfusion. No significant changes were observed in beta-thromboglobulin, screen filtration pressure, plasma electrolytes, calcium, protein or albumin. The total plasma bilirubin fell by a mean of 24 mumol/l, with a reduction in 21 of the 25 perfusions studied of up to 104 mumol/l during perfusion. Mean plasma levels of total bile acids were 137 +/- 19 mumol/l and 115 +/- 16 mumol/l respectively before and after four hours hemoperfusion. The amount of bile acids recovered by elution of the resin column was over three times greater than that apparent from the change in plasma levels. Column chromatography on Sephadex G-25 of material eluted from the resin column showed various peaks to be removed, including substances in the middle molecular weight range (1000-5000 daltons). Of the patients treated, eight (42%) survived to leave hospital.

Stability and reversibility of higher ordered structure of interphase chromatin: continuity of deoxyribonucleic acid is not required for maintenance of folded structure.

The organization of the higher order structure of chromatin has been examined in chicken erythrocyte. Chromatin solubilized during the time course of a gentle micrococcal nuclease digestion of nuclei shows a continuous variation in the distribution of molecular weights. Electron microscopy studies of large chromatin fragments solubilized at physiological ionic strength (0.14 M NaCl or KCl) suggest that the polynucleosome chain is folded in continuous compact structures of an average diameter of 23 nm in which the individual nucleosomes are difficult to distinguish. This compact structure is destabilized even at intermediate ionic strengths (e.g., 40 mM NaCl), resulting in looser fibers of similar diameter. At 5 mM NaCl the fiber is unraveled into a continuous filament of 10-nm diameter. These conformational changes are reversible as determined by hydrodynamic and biochemical parameters. The 10-nm leads to 23-nm transition of chromatin appears to be a cooperative process requiring the full complement of histones H1 and H5. Micrococcal nuclease cleaves the DNA in the compact chromatin structure to an apparent limit of digestion corresponding to an average of eight to nine nucleosomes with little effect on the size of the fiber. Thus, the continuity of the DNA is not required for the stability of the folded chromatin fiber. Histones H1 and H5 exhibit a binding preference to larger chromatin fragments regardless of the length of the DNA. This behavior is not observed with relaxed chromatin, suggesting that multiple stabilizing interactions involving H1 (H5) are possible only in the compact configuration.

[Hepatitis B markers in paediatric patients (author's transl)].

372 children with suspected liver disease were examined for serum HBsAg. Six (three boys, three girls) were found to be positive (1.6%). Further studies of these patients for up to three and a half years revealed elimination of HBsAg in one case only. Biopsies were performed in five patients. Three showed mild chronic hepatitis (two chronic persistent hepatitis, one unspecific reactive hepatitis). Chronic aggressive hepatitis was diagnosed in one patient. One child seemed to be normal on light microscopy, but the findings on electron microscopy were abnormal, the liver cell nuclei being filled with core particles. Two thirds of the family contacts of these children showed hepatitis B marker. Two pregnancies were observed in HBsAg-positive mothers. An infection of the babies was not demonstrable.

In vitro core particle and nucleosome assembly at physiological ionic strength.

Nucleosome core particles have been efficiently assembled in vitro by direct interaction of histones and DNA at physiological ionic strength, as assayed by digestion with DNases, supercoiling of relaxed circular DNA, and electron microscopy. Reconstitution was achieved either by the simultaneous addition of all core histones, or by the sequential binding of H3 . H4 tetramer and H2A . H2B dimer to DNA. Micrococcal nuclease digestion and electron microscopy studies indicated that there is heterogeneity in the spacings at which core particles are assembly on the DNA. Length measurements of oligomeric DNA produced during the course of the digestion suggest that the core histone octamer can organize 167 (+/- 4) rather than 145 base pairs of DNA, the extra 20 base pairs being quickly digested. Binding of histone H1 to core particles resulted in the protection of about 165 base pairs of DNA from nuclease attack. Because the core histone octamer is fully dissociated into H3 . H4 tetramer and H2A . H2B dimer at physiological ionic strength, our results would suggest that in vivo core particle assembly may also occur by interaction of these two complexes on the nascent DNA.

[Diagnosis experiences in 188 patients with primary liver cancer].

In Austria primary carcinoma of the liver (plc) is no rare malignancy. From 1973 and 1979, in our department, 188 patients with plc had been registered. By separate evaluation of the results of alpha-1-fetoprotein (AFP)-determination and 67-gallium-liverscintigraphy, a diagnostic accuracy of 81.6%, resp. 78. % could be demonstrated. By combination of both methods, a correct diagnosis of plc was obtained in 95%. Based on the promising results of lobectomies performed in patients with plc, therapeutic nihilism is not longer justifiable.

[Clinical significance of the combined determination of alpha 1 fetoprotein, lipoprotein X, and hepatitis B antigen and antibodies].

In a four-year prospective study 4000 patients of a gastroenterologic and hematologic department were screened for alpha1-fetoprotein (AFP), lipoprotein-X (LPX), and hepatitis-B antigen and antibody (HBsAg, HBsAb). Only histologically confirmed results were used for the evaluation of this study. Liverscintigraphy using 99m-Tc-S-colloid was performed as the morphological examination. 85% of focal liver diseases were seen as space-occupying lesions in the radioscan. The interpretation of their dignity was performed with success by means of 67-galliumcitrate with an overall diagnostic accuracy of 90%. With the methods, presented in this paper, positive results of AFP-LPX-, HBsAg- and HBsAb-determinations mean, without exception, an informational gain. In 1241 of 4000 patients, 1417 positive results were recorded. In combination with two-step radionuclide-scintigraphy as the morphological element of our screening procedure, focal diseases of the liver are no longer a diagnostic problem. Additional information of diagnostic relevance is expected in about 35% of all diffuse hepatocellular diseases.

[Lipoprotein-x determination in children beyond an age of 6 months (author's transl)].

In premature and newborn infants an abnormal lipoprotein (LP-X) was almost regularly detectable by examination up to the 24th week after deliver. The aim of the submitted study was to evaluate LP-X results in children older than 6 months. 250 sera of 158 infants of all age groups were tested for presence of LP-X. The frequency of LP-X positive results was 85.7% in the group of infants younger than 24 weeks (91 samples of 45 infants were tested) whereas in the group of infants of an age between 24 weeks and 14 years (159 samples of 113 children were tested) 16.35% positive results were obtained. LP-X positive results showed the same clinical significance in the second group of children as it was demonstrated in adults elsewhere.

[LP-X in the first month of life (author's transl)].

An abnormal lipoprotein can be detected in the sera of patients suffering from intra- and extrahepatic cholestasis. The specificity of LP-X detection for the diagnosis of cholestasis in adult persons was proved by extensive investigations. The aim of this study was to examine the specificity of LP-X with regard to cholestasis in the postnatal phase of premature and newborn infants. 1056 sera of 270 newborns were tested and in 75% of these cases LP-X was found to be positive. Three hypothetical explanations are given concerning the appearance of LP-X in the sera of premature and newborn infants. The semi-quantitative determination of LP-X also represents a possibility for differential diagnosis of obstructive jaundice in the neonatal period.

Evaluation of environmental polychlorobiphenyls and DDE in terms of mixtures of commercial preparations from peak heights of packed-column gas chromatograms using a programmable calculator.

A method is proposed for evaluating gas chromatograms of multi-component PCB mixtures and superposed single components simultaneously. Apparent concentrations relative to a calibration mixture are assigned to a number of suitable peaks, and the apparent concentrations are related to the true concentrations by a set of linear equations, which are solved by least-squares approximation. The results are provided with an estimate of their confidence intervals.

[Preliminary investigations of the serum alpha-1-fetoprotein level in mature and premature newborn infants (author's transl)].

Semiquantitative determination of the serum AFP concentration was carried out in 170 newborn infants (mature and premature) by means of crossover electrophoresis on admission to hospital and at 1 to 2 weekly intervals thereafter. The infants were divided into 3 groups according to their birth weight and also grouped according to gestational age into 3 further groups. The serum AFP level was correlated with the age in weeks of the infants for each group separately. Mature newborn infants with a birth weight of over 2500 g show a rapid fall in serum AFP titre; AFP is no longer detectable after the 9th week of life. In premature infants with a birth weight of 1500 to 2500 g, the fall in AFP titre is delayed and reaches zero between the 7th and 13th week of age. There is a markedly slower decrease in serum AFP concentration in the group of immature infants with a birth weight of under 1500 g; the limit of detectability lies between the 13th and the 17th week of age. An almost identical pattern is revealed when the infants are grouped according to gestational age. The possibility of diagnosing the small for date babies and severe liver disease by means of the serum AFP titre is discussed.

[Differential diagnostic procedures in localized liver diseases].

Liver scintigraphy by means of 99mTc-sulphur colloid (n = 430) and 67Gallium-citrate (n = 180) were carried out on selected patients, on a follow-up study. This study was undertaken to prove the possibility of early diagnosis of primary hepatic carcinomas. Only autoptically verified cases (n = 119) were used for the evaluation of the accuracy of liver scans. 99mTc-s.c. -scans predicted the presence or absence of focal liver diseases in 78% of the cases. Scan data were falsely positive in 10% and falsely negative in 28%.
