Neutron anatomy.

The familiar extremes of crystalline material are single-crystals and random powders. In between these two extremes are polycrystalline aggregates, not randomly arranged but possessing some preferred orientation and this is the form taken by constructional materials, be they steel girders or the bones of a human or animal skeleton. The details of the preferred orientation determine the ability of the material to withstand stress in any direction. In the case of bone the crucial factor is the orientation of the c-axes of the mineral content-the crystals of the hexagonal hydroxyapatite- and this can readily be determined by neutron diffraction. In particular it can be measured over the volume of a piece of bone, utilising distances ranging from 1 mm to 10 mm. The major practical problem is to avoid the intense incoherent scattering from the hydrogen in the accompanying collagen; this can best be achieved by heat-treatment and it is demonstrated that this does not affect the underlying apatite. These studies of bone give leading anatomical information on the life and activities of humans and animals-including, for example, the life history of the human femur, the locomotion of sheep, the fracture of the legs of racehorses and the life-styles of Neolithic tribes. We conclude that the material is placed economically in the bone to withstand the expected stresses of life and the environment. The experimental results are presented in terms of the magnitude of the 0002 apatite reflection. It so happens that for a random powder the 0002, 1121 reflections, which are neighbouring lines in the powder pattern, are approximately equal in intensity. The latter reflection, being of manifold multiplicity, is scarcely affected by preferred orientation so that the numerical value of the 0002/1121 ratio serves quite accurately as a quantitative measure of the degree of orientation of the c-axes in any chosen direction, for a sample of bone.

The orientation of the mineral crystals in the radius and tibia of the sheep, and its variation with age.

The direction of preferred orientation of the hydroxyapatite crystals in both the tibia and radius of the sheep is close to the long axis of the bone, notwithstanding the angle of about 30 degrees which, for the tibia, exists between the long axis and the direction of principal dynamic strain during locomotion. For both bones the orientation of the cranial cortex, which is a tension surface during locomotion, is about 40% larger than the caudal. The variation with age of the magnitude of the preferred orientation for the sheep bones is contrasted with what has been reported earlier for the human femur. Notably, for the sheep, both bones show substantial orientation at birth--having increased steadily during gestation--so that the animal is able to stand and walk at the outset.

Anabolic steroids: interest among parents and nonathletes.

To determine the nature and extent of public interest in anabolic steroids, questionnaires were sent to 1290 family physicians and pediatricians in Texas. Of the 517 respondents, 55% reported being asked about steroids or seeing possible steroid users in their practices during the previous 5 years; 261 physicians (51%) reported 1682 inquiries. High school aged boys were the subject of 60% of all inquiries, 26% of which were made by parents and were exclusively related to sports. Football and athletics in general were the most common reasons for inquiry (62%), but 36% of the adolescents were interested in steroids for psychosocial reasons. Nearly 60% of the inquiries by adolescents and 36% of those by parents were considered to be serious, as opposed to casual. All inquiries regarding adolescents were for boys, and less than 1% were nonwhite. On hundred seventy-seven physicians (34%) reported 802 possible steroid users. Only 17 patients fit the stereotype of a steroid user (ie, competitive bodybuilders or professional and collegiate athletes); football players comprised 12.4% of the total. Those thought to be using steroids for psychosocial reasons equaled almost 10% of the total. All those identified as steroid users were male and only nine were nonwhite. The survey results suggest that interest in and use of anabolic steroids are common, rather than the exclusive purview of competitive athletes. This study is the first to demonstrate parental interest in anabolic steroids.

The dependence of human bone texture on life style.

Neutron diffraction has been used to determine the preferred orientation of the hydroxyapatite crystals at the lower front edge of the tibia. The bones are compared for two Neolithic Orkney tribes. At Isbister, a muscular tribe on a hilly site engaged in heavy uphill work, whereas a tribe at Quanterness lived on level ground. The two sets are contrasted with the texture of present-day amputations. Values of 16, 8 and 3 are found for an 'index of manoeuvrability' for the Isbister, Quanterness and modern ankles, respectively.

Effects of peer-group intervention on metabolic control of adolescents with IDDM. Randomized outpatient study.

In children with insulin-dependent diabetes mellitus (IDDM), deterioration in metabolic control frequently occurs during early adolescence. To prevent this predictable increase in blood glucose levels, we randomly assigned young adolescents with IDDM to an intervention based on problem solving with self-monitoring of blood glucose (SMBG) integrated into standard outpatient care or to standard care only for an 18-mo period. At follow-up, 50% of the standard-care adolescents exhibited greater than 1% increase in glycosylated hemoglobin (HbA1) levels over baseline values, indicating a deterioration in metabolic control, compared to only 23% of the intervention group. Follow-up HbA1 means +/- SD were 10.10 +/- 2.00% for intervention and 11.04 +/- 2.28% for standard-care adolescents, indicating a significantly lower value in the intervention group (P = .04). At follow-up, a greater percentage of intervention than standard-care adolescents reported using SMBG information when they exercised (60.0 vs. 33.3%, chi 2 = 4.29, P = .04). Our data suggest that clinic-based problem-solving groups can be more effective with young adolescents with IDDM than conventional treatment in preventing the expected deterioration in blood glucose.

Anabolic steroids and growth hormone in the Texas Panhandle.

We conducted a survey of family physicians and general pediatricians in the Texas Panhandle to learn about patient interest in anabolic steroids and growth hormone. Responses were anonymous. Fifty-five percent of those surveyed responded. Of these, 48.5% had received inquiries about anabolic steroids, 29% had seen patients they felt were taking steroids, and 15% had received inquiries concerning growth hormone. All inquirers about steroids and growth hormone and all perceived steroid users were male, and almost all were white. Adolescents (from 13 to 17 years old) accounted for 48% of the inquiries, young adults (18 to 25 years old) 48%, and adults 4%. Twenty percent of the inquiries were made by parents. Adolescents accounted for 31% of the perceived steroid users, young adults 68%, and adults 1%. Weight lifters and body builders comprised 60% of this group, football players 26%, and those taking steroids primarily for psychological reasons 13%. Nearly all inquiries about growth hormone were made by parents, and 44% were made on behalf of children for purposes of augmenting their normal height or for athletic enhancement.

Alopecia areata, endocrine function, and autoantibodies in patients 16 years of age or younger.

Forty-five children with alopecia areata were prospectively studied by means of both clinical and laboratory evaluation for evidence of endocrine diseases and autoantibodies. Twenty-four percent had an abnormality as determined by one or more thyroid function studies (thyroxine, triiodothyronine, and thyroid-stimulating hormone) and/or elevation of microsomal antibody levels. In 16%, smooth muscle antibody was present, and in 4%, parietal cell antibody was present. Routine thyroid function testing is recommended for all children with alopecia areata.

Evaluation of glycosylated hemoglobin in the management of young patients with insulin-dependent diabetes mellitus.

Serial glycosylated hemoglobin (HbA1) concentrations were measured in 146 children and young adults with insulin-dependent diabetes mellitus. Patients were randomly divided into two groups. The HbA1 results were made available to the physicians and patients of group A but they were withheld from group B for 12-16 months. The groups were then alternated for an equal time period, with group A being blinded and group B unblinded. When the patients were further subdivided into those with initial HbA1 levels above or below the group mean (12.7%), it was found that the metabolic control of subjects with high HbA1 concentrations tended to improve when the values were made available and to remain unchanged when the results were withheld. Knowledge of HbA1 concentrations by physician and patient/parent appears to be beneficial in patients with relatively poor control. In contrast, no improvement was noted in patients with initial HbA1 less than 12.7% whether or not the results were available to the physician and patient/parent.

Cognitive abilities of adolescent Turner's syndrome patients.

The association of deficits in spatial abilities and lower Performance IQ (PIQ) scores in Turner's Syndrome Patients (TSPs) has been studied since 1962. Conflicting results have been reported with regard to the nature of their spatial ability deficits. This study examines the nature of the cognitive and spatial abilities in TSPS of normal intelligence. A neuropsychological battery of standardized tests was administered over a two-month period to 14 TSPs, aged 13-19 years. These patients had been served medically in the Pediatric Endocrinology Unit, University of Michigan Medical Center, since childhood. Patient scores were compared to the test battery's standardized norms by univariate t-tests. No differences were found in the Wechsler subtest scores previously used to measure spatial abilities (Block Design, Object Assembly, WISC-R). TSPs excelled in verbal abstract reasoning and verbal comprehension (p less than 0.01) and demonstrated superior social judgment (p less than 0.05). Pairwise t-tests did not reveal differences in these spatial measures when the sample was divided into Bright (110+) and Average (109-80) Verbal IQ (VIQ) groups. Although their PIQs were within the average range, the TSPs VIQs were significantly higher (p less than 0.01). Verbal-performance (V-P) differences are discussed in the context of normal V-P discrepancies in Wechsler's original standardized sample. Patients did have a significant (p less than 0.01) impairment on measures that involved the cognitive process of visual memory. Findings suggest that a specific spatial deficit is not present in TSPs who have average or above intelligence. Patients in this sample did, however, demonstrate an impairment in visual memory.

Texture, stress and age in the human femur.

The way in which remodelling of human bone is determined by the mechanics of life is explored in a neutron diffraction study of the femur. The hydroxyapatite crystals provide ability to resist vertical stress by a preferred alignment which is low at birth, increases to a maximum at an age of about three years and then falls away steadily in later life. This vertical alignment is greater at the front of the femur than at the back.

A neutron diffraction study of the bones of the foot.

The preferential orientation of the apatite crystals in the lower tibia, talus and calcaneus, as determined by neutron diffraction, serves as an indicator of the lines of stress in the foot. The main stress flows down from the tibia to the heel and the ball of the foot and there is noteworthy orientation along the line of the plantar aponeurosis which acts as a tie. Orientation is particularly marked above the point of attachment of the Achilles tendon, where the edge of the calcaneus functions as a beam. The centre of the talus serves as a junctional region for forces and is relatively unoriented.

Hormonal and clinical responses to prednisone treatment in adolescents with congenital adrenal hyperplasia.

The effects of prednisone therapy and the withdrawal of prednisone for 3 days on hormonal relationships was investigated in 6 adolescent patients, age 10--19 years with congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency. Prednisone continuously suppressed adrenal release of 17 alpha-hydroxyprogesterone (17-OHP) in 4 patients and adequately controlled 17-oxosteroid (17-OS) excretion in 5. Serum dehydroepiandrosterone sulfate (DS) concentrations were not elevated during or 3 days after withdrawal of medication. Growth hormone (GH) secretion was not suppressed by prednisone. Withdrawal of treatment was associated with a decrease in the follicle-stimulating hormone (FSH) to luteinizing hormone (LH) ratio in 5 patients and a rise in serum testosterone in 4. The clinical courses of these patients emphasize the difficulty in achieving optimum control.

Stress distribution in the scapula studied by neutron diffraction.

High intensity neutron beams provide a method of measuring the preferred orientation of apatite crystals in bulk samples of bone. Measurements at seven different sites on the scapula show that the c axes of the crystals lie preferentially along the directions of pull of the attached muscles. The highest orientation is found at positions under the influence of only a single group of muscles, such as M. teres major or M. infraspinatus. In intermediate regions a multiple distribution of crystals is found, able to withstand stresses in more than one direction. The technique provides a method of assessing the distribution of stress in bones.

Suppressed responsiveness to gonadotropin-releasing hormone in girls with unsustained isosexual precocity.

Eleven girls, ages 10/12 to 76/12 years, were evaluated because of early and rapid breast development. Initial clinical presentations and serum gonadotropin or estradiol determinations did not differentiate patient types. However, patients could be divided into two groups based on their responses to synthetic gonadotropin-releasing hormone: Group A consisted of seven girls with suppressed or prepubertail-type responses, and Group B consisted of four girls with pubertal or adult-type responses. Subsequent evaluation revealed that Group A patients had intermittent or unsustained isosexual precocity, whereas Group B patients had isiopathic prococious puberty. During initial evaluation, increased serum or urinary estrogen values were noted in ten of ten patients who were studied. The greatest serum E2 values (162 and 117 pg/ml) were noted in two Group A patients; three months and two years later, those patients had normal prepubertal responses to GnRH and serum E2 values of less than 4 and 14 pg/ml, respectively. Unsustained sexual precocoity in girls may be secondary to autonomous ovarian production of estrogens, and the GnRH test may be useful in evaluation of girls with isosexual precocity.
