[Total and specific IgE concentrations for early diagnosis of respiratory allergy in children less than four years old (author's transl)].

Serum total IgE concentrations were determined according to paper-radio-immunosorbent test (Prist, Phadebas) in 73 infants aged 2 months to 4 years, with recurrent asthmatoid bronchitis. Specific IgE against multiple inhalant allergens (Dermatophagoides pteronyssinus d1, house dust h3, pollens, moulds) were quantified by radio allergosorbent test (Rast, Phadebas). Normal total IgE values were determined in a control group of 111 infants aged 0 to 4 years. Total IgE were slightly elevated in 8 cases, strongly in 35 cases (id. e in 47% of the patients' group). Serum IgE are significantly higher in infants whose first degree relatives are atopics. Specific IgE against d1 are detected in 44% of our cases. Fourteen infants with dl-specific IgE have had an "ecologic treatment" by Paragerm AK. The clinical results are good in twelve of these patients. Total IgE and d1-specific IgE decrease in 5 cases at the end of the Paragerm application.

[Neonatal screening for hypothyroidism by simultaneous determination of T4 and TSH on filter paper (author's transl)].

A simultaneous determination of T4 and TSH has been done on 48 000 new borns since January 1st of 1977 in the Midi-Pyrénées district. 15 cases of hypothyroidism have been detected, an incidence of 1/3 200 new borns. TSH values always were above 80 microU/ml; T4 values are under m-2 sigma in 11 cases, normal in 2 cases (8,8 and 9 micrograms/dl) and between -1 and - 2 sigma in 2 cases. A thyroid scanning with 123 odine or 99 Tech, has shown the absence of thyroid in 6 cases, an ectopic gland in 7 cases and a thyroid in a normal position in one case. A treatment has been possible before the first month of life in each case. The average D.Q. for the seven oldest new borns (m = 11 months) is 96. The simultaneous determination of T4 and TSH is a sensitive and specific screening method for hypothyroidism. False-negative are avoided and the number of false-positive results is reduced. All cases of hypothyroidism can be detected (primary hypothyroidism, hypothalamo-hypopituitary hypothyroidism, TBG deficiency, etc...) Then, an early and certain diagnosis is made possible (T4 low and TSH increased).

[Normal values of total serum IgE in children from birth to 16 years of age].

Total serum IgE values were measured by solid phase radio-immunoassay in 250 normal children, 129 females and 121 males, whose ages ranged from birth to 16 years. IgE is not present in cord blood in contrast to the high maternal values but thereafter levels increase in a linear manner (y = 2.28 + 5.43 x, n = 250, r = 0.57, alpha less than 0.01). There was no significant difference between the sexes, both for the group as a whole as well as within age groups. There was no circadian variation. The distribution of the values in the age groups was log Gaussian and therefore the upper limit of normal should be defined as the 95% confidence intervals calculated from the log Gaussian distribution.

[Somatostatin in a case of nesidioblastosis (author's transl)].

The authors report the case of a three-month-old infant presenting with hypoglycaemia. The etiology, i.e. nesidioblastosis, was asserted by simultaneous dosage of insulin and glucose and by histologic study of the pancreas. Initial therapy was a combination of diazoxide, continuous enteral alimentation and glucose injections. It's ineffectiveness led to an operative decision and 4/5 ths of the pancreas were removed. After a transitory improvement symptoms recurred. Data provided by studies of baseline levels and response to exogenous somatostatin then allowed to determine the correct dosage for treatment with long-acting somatostatin. The authors also discuss the importance of somatostatin in pathogenesis as well as for diagnosis and therapy of nesidioblasosis.

[Results of psychomotor development in hypothyroidism detected at birth].

Thirty-five children in whom hypothyroidism was detected at birth were studied with respect to psychomotor development. A 5 year-follow-up was obtained in a few cases. The I.Q. was established using Brunet-Lezine's test or Griffith's test after age 30 months. Global quotients where normal: 98 + 10 at 6 months, 96 + 7 at 1 year, 99 + 7 at 3 years, 97 + 7 at 4 years and 97 + 4 at 5 years. Partial development quotients were normal concerning coordination and sociability. A transitory decrease was found at 1 year concerning posture and at 2 and 3 years concerning language, both returning to normal by age 4. These results were correlated with several factors: there was no significant difference between athyroidism and ectopia at the different ages but a difference appeared for partial development quotients at 2 and 3 years. There was no correlation between development quotients, clinical scores and dates of diagnosis. On the contrary, a significant correlation was found global development quotients at 6 months and the surfaces of epiphyses. Finally, a precise neurological evaluation showed an impairment of posture, coordination and fine movements of the extremities.

[The Stein-Leventhal syndrome. Clinical, biological and anatomic study of 6 cases in adolescent girls (author's transl)].

Clinical, biological and anatomic studies were performed in 6 adolescent girls with the Stein-Leventhal syndrome. The rare occurrence of this syndrome in pediatrics is emphasized. The main clinical signs consist of obesity, hirsutism and menstrual irregularities. Basic LH and plasma testosterone are useful diagnostic means, as well as -- in children -- the suppression-stimulation and LH-RH tests. However, coelioscopy is necessary to allow a correct diagnosis. Histological examination might not be absolutely necessary since a large biopsy could be responsible for untimely ovulation. Estrogens and anti-androgens are the only logical treatment at the time of puberty.

[Neonatal detection of hypothyroidism in the South-Pyrenean region. Results of 14,000 determinations of T4 in blood eluates collected on blotting-paper].

Screening for neonatal hypothyroidism has been undertaken in Southern France by the estimation of T4 in the eluates of blood spots on filter paper. Since March 1976, 14,000 estimations have been made and four cases of hypothyroidism with very low levels of T4 have been detected (less than 38 pg/2 spots). Two cases had high TSH levels (greater than 200 microunits/2 spots). Blood was taken from the hypothyroid babies on the 20th day of life, to check the findings and in all cases the thyroxine was less than 2.8 microgrammes/100 ml and the TSH greater than 40 microunits/ml. T3 was reduced in three cases (less than 75 pg/ml) and normal in one case (150 pg/ml). In all cases no thyroid tissue could be demonstrated with technitium scan. In the first month of life there were practically no symptoms except for mottling of the skin in two cases, a large posterior fontanelle in two cases and in one baby a prolonged neonatal jaundice.
