[The concept of tic in the history of abnormal movements].

History of abnormal movements started during the 14th century. At that time the St Vitus' Dance was described, but the nosology of dyskinesias remained confusing during the next five centuries. The concept of tic was elaborated in France during the 18th century. It remained too large a concept however. Definitive semiologies appeared at the end of the 19th century, thus allowing tics to emerge from the "chaos of choreas". The etymology of the word "tic" still remains mysterious. In 1905, Meige thought that the word tic was used for the first time by reference to horses. He referred to the tic of the bear in the horse described by Rudler and Chomel at The Société de Neurologie de Paris in 1903. Veterinarians were thus probably the first to describe the word. If so, however, the horse must leave anteriority to the goat. The word Ticq was used in 1611 as mentioned by the French dictionary Robert. The word is said to be an onomatopea and is compared to the italian word ticchio which means caprice. Another dictionary (Littré) suggest the german word "ticken", which means "to touch slightly", the galic word tacaid (sudden pain) and the german ziki (young goat), a word which could have lead to ticchio as capra, goat in italian, gave capricio.

Simultaneous determination of metapramine and its demethylated metabolites in plasma by gas chromatography-mass spectrometry.

A capillary column gas chromatography--mass fragmentographic method for metapramine and its three major demethylated metabolites is described. Compounds are extracted from plasma using a double-extraction procedure and transformed into N-trifluoroacetyl derivatives. The detection is performed by monitoring specific ions for metapramine and for its metabolites with a mass detector. In spite of extensive metabolism in the liver and rapid elimination of metapramine, plasma concentrations of both metapramine and its metabolites can be simultaneously followed over 24 h after a single 150-mg oral dose, because of the sensitivity and selectivity of the method. This method has been successfully applied to the analysis of samples obtained from patients who were at steady state with metapramine and to a pharmacokinetic study in a healthy volunteer.

Acute migraine attack therapy: comparison of naproxen sodium and an ergotamine tartrate compound.

The efficacy of safety of naproxen sodium and ergotamine tartrate were compared for the treatment of acute migraine attack in a randomized, parallel trial with 114 participating patients. At the start of symptoms, patients took either three tablets of naproxen sodium (275 mg each) or one of an ergotamine combination (containing 2 mg ergotamine tartrate, 91.5 mg caffeine, and 50 mg cyclizine chlorhydrate). Patients were followed for three months or until six attacks were monitored, whichever came first. Both medications substantially shortened the duration of migraine attacks and reduced the severity of symptoms. When the test medications were taken within 2 h of onset of attack, naproxen sodium was statistically significantly more effective than the ergotamine combination in reducing the severity of headache pain, nausea, and lightheadedness. The ergotamine combination was associated with significantly more vomiting, need for rescue medication, and side effects than was naproxen sodium. Four patients required discontinuation of the ergotamine combination and one of naproxen sodium. Both patients and investigators rated tolerance for naproxen sodium as superior to tolerance for the ergotamine combination. Naproxen sodium seems to be an effective and safe treatment for migraine attacks.

[Acute parasellar compression syndrome disclosing hypophyseal adenoma. 3 cases].

Three cases of acute necrosis of a hypophyseal adenoma are reported with a review of the medical literature. The clinical presentation as an acute parasellar compression syndrome was the result of aseptic meningitis in 2 cases and of cavernous sinus thrombosis in the other; the usual symptoms of headaches, oculomotor paralysis and impairment of consciousness were observed. The diagnosis was suspected on clinical grounds, by the radiological changes of the pituitary fossa and confirmed by computerised axial tomography. All three patients had a favourable outcome. Surgery was only required in one case with threatening visual complications. Dissociated anterior pituitary deficiency persisted in all cases; two patients also had diabetes insipidus. In one case, the pituitary necrosis stabilised an acromegaly for a two year period.

[Association of autoimmune dementia, Basedow's disease, and Gougerot-Sjögren syndrome].

The authors observed one case of myoclonic dementia reversible with steroid therapy, associated with Grave's disease and Sjögren's syndrome. A circulating IgG fraction was demonstrated reacting specifically with cerebral micro-vessels and which evolved in parallel with the neuro-psychiatric disturbances. These results support the hypothesis of an autoimmune vasculitis causing the dementia. The persistence of abnormalities one year after clinical cure showing an alteration of the blood-brain barrier associated with the appearance of circulating antibodies against the hypophyseal cells, and in view of the predictive value of these antibodies, suggests a possibility of the eventual apparition of an Alzheimer type dementia.

[Secondary prevention of cerebral infarction caused by atherosclerosis. 3-year comparison of flurbiprofen and an ergot derivative].

A multicentre controlled trial was conducted on 383 patients to compare the effectiveness of flurbiprofen (100 mg/day) and an ergot derivative in preventing cerebral infarction. The end-point of the trial was recurrence within 3 years of treatment with one or the other drug of a fatal or non-fatal cerebral infarction. Only those patients who had already experienced an established and disabling ischaemic accident in the carotid or vertebro-basilar territory were included; 88% of these entered the trial less than 6 weeks after the accident. Sixty-one patients had recurrent cerebral infarction during the 3-year observation period: 22 in the flurbiprofen group and 39 in the ergot derivative group (p less than 0.02 log rank test adjusted by centre). The actuarial recurrence rates in the two groups were 13% and 21% respectively. These results concur with those of other trials involving aspirin to demonstrate the value of antiplatelet aggregation drugs in the secondary prevention of atherothrombotic cerebral infarction.

Effect of a reversible and selective MAO-A inhibitor (cimoxatone) on diurnal variation in plasma prolactin level in man.

Prolactin (PRL) secretion is stimulated by serotonin (5-HT) and inhibited by dopamine (DA). 5-HT is generally recognized as a substrate for type A monoamine oxidase (MAO), whereas DA is considered as a substrate for either A or B, or both forms of MAO, depending on the species and tissues used. The effect of cimoxatone, a reversible, selective MAO-A inhibitor, on diurnal variation in plasma PRL level was investigated in healthy adults after a single 40 mg oral dose, as an indirect approach to investigating whether DA is preferentially a substrate for Type A or B MAO in man. The circadian rhythm in PRL, stress conditions and diet were taken into account in the present study, which was placebo-controlled. There was a slight but significant reduction in circulating PRL in the six subjects, which persisted for at least 9 h after cimoxatone. However, the duration of the decrease in plasma PRL was shorter than the inhibition of MAO-A. The results are not inconsistent with the presence of both forms of MAO in the human hypothalamus and with DA as a substrate for both forms in this region, if it is assumed that the hypothalamic concentrations of the drug during the period 0-9 hours was sufficiently high to inhibit DA deamination by both forms of MAO.

[Chronic paroxysmal hemicrania sensitive to indomethacin].

Chronic paroxysmal hemicrania is a new entity described in 1974. It appears to be rare, since 34 cases only have been reported in foreign literature. It is characterized by severe unilateral pain occurring several times a day for months or years and is resistant to all treatments, except indomethacin. The first 3 cases observed in France are reported and data from the literature are reviewed.

A comparison of the effects of vitamin B12 and aspirin in three experimental pain models in rats and mice.

The effects of vitamin B12 are compared to those of aspirin and 0.9% saline in three different experimental pain models in rats and mice; an electric shock test, the hot-plate test, and the phenylbenzoquinone writhing test. Neither cyanocobalamin nor hydroxocobalamin showed any antinociceptive activity in any of the three tests at any of the doses used, whether the drug was administered as a single injection or as a series of daily injections lasting 7 days. These results run counter to those of clinical trials in man, though close scrutiny of the experimental procedures used in these trials often reveals a lack of rigor which casts doubts on their validity. This consideration, together with the negative findings reported here, suggests that the clinical use of vitamin B12 as an analgesic may be inappropriate.

[Controlled trial of isaxonine in traumatic or ischaemic (compression) neuropathies (author's transl)].

Fifty patients showing one or more impairments to large nerve trunks due to trauma or ischaemia received, after drawing lots, either isaxonine or a placebo. The evaluation criteria of the recovery were both clinical (testing of muscles, sensory disorders, vegetative disorders, tendon reflexes, tonicity, amyotrophy) and electrophysiological (electromyogram, conduction velocity, distal latency time, H reflex and H/M index). This study shows two major elements: the action of isaxonine shortens healing time (45-90 days instead of 120-150 days) and isaxonine allows neurological recovery to resume at neurologic level whereas the situation was dating back more than three months.

[Abdomino-crural contracture disclosing panhypopituitarism with malnutrition].

Another case of flexion contracture in a 60 year-old patient with hypopituitarism, chronic alcoholism and starvation is reported. The contracture, as in the other cases, involved the lower limbs, but in this particular patient it extended to the abdominal and extensor neck muscles and neurological signs were associated: cerebellar atrophy and peripheral neuropathy of the lower limbs probably due to alcoholism. As in the other reported cases, severe hyponatremia was observed (105 mmol/l) the physiopathology of which is discussed: starvation and glucocorticoid deficiency alone or associated with aldosterone deficiency. The special feature of the anterior pituitary deficiency is that it is partly functional, at least with regards to the gonadotrophic secretion, perhaps related to the denutrition. The authors emphasize the necessity of searching for adrenal insufficiency in cases of abdomino-crural contracture because of the beneficial effects that result from hydrocortisone replacement therapy.

Triazolam compared with nitrazepam and with oxazepam in insomnia: two double-blind, crossover studies analyzed sequentially.

1 In two double-blind, crossover studies, general practitioners compared the hypnotic activity of triazolam 0.50 mg with that of nitrazepam 5 mg or oxazepam 50 mg in outpatient insomnias. 2 The patient's preferences served as the main criterion, and were processed by sequential analysis. 3 More patients (P less than 0.05) preferred triazolam to nitrazepam (triazolam 28; nitrazepam 11; no preference 9). 4 The numbers of patients who preferred triazolam or oxazepam did not differ significantly. 5 Triazolam and oxazepam affected sleep parameters similarly except for onset of sleep and feeling in the morning, for which triazolam seemed to be superior to oxazepam.

[Subacute sclerosing panencephalitis. A case with a prolonged course in an adult. Isolation and characteristics of a "defective" measles virus (author's transl)].

A 33-year-old man had a 6-year history of clinical signs suggesting multiple sclerosis : visual lesion at 27 years of age, cerebellar and visual disturbances at 31, which partly regressed, lymphocytosis and increased-gammaglobulin levels in the cerebrospinal fluid. Biological and anatomical (optical and ultrastructural) examinations gave results typical of a subacute sclerosing panencephalitis. A cytopathogenic measles virus was isolated from a cerebral biopsy specimen. The agent was transmissable to vero cells by co-culture but infectivity was always related to the cells and was therefore an incomplete viral infection. Virus-like particles were found in the nucleus and cytoplasm after electron microscopy examination of the co-cultures. Biochemical tests demonstrated that the viral proteins were all synthesized except hemagglutinin, which is a characterist abnormality of a "defective" measles virus.

[Meningo-radiculitis following tick bite].

The authors report nine cases of acute inflammatory polyradiculoneuritis developing one to eleven weeks after a tick bite, regressive, painful and asymmetrical, with an albumino-cellular reaction in the CSF. They stress the very particular physiognomy of this type of meningo-radiculitis, its seasonal occurrence and the uncertain nature of its pathogenesis.
