Indications, procedures and results in the surgical treatment of hyperthyroidism: a follow-up of 336 cases.
A personal experience with the surgical treatment of hyperfunctioning thyroid diseases from 1970 throughout 1982 is reported. Data on 336 controlled cases are analyzed. The series reported is homogeneous as for indications, surgical procedures, pre and postoperative management. Total lobectomy has proved to be the therapy of choice for uninodular lesions: all followed patients (208) are euthyroid. Subtotal or "near-total" thyroidectomy was curative in 71,8% of multinodular and basedowified goiters, as well as in Graves' disease. For the latter, indications of total thyroidectomy in selected cases are being evaluated. Further studies to complete the follow-up of the series are in progress.
Primary malignant tumors of the thyroid gland. Histology, age and sex distribution and pathologic correlations in 139 cases.
The classification proposed by Woolner et al. (1961, 1971) has been applied to 139 cases of primary malignant tumors of the thyroid gland diagnosed in one of the largest hospitals of Rome, Italy, over a period of 5 years, from 1977 to 1981. These cases come from 1418 patients with enlargement of the thyroid surgically treated at the 5th Surgical Clinic of the University of Rome. All the histologic slides were re-examined, and the pathology records were reevaluated. Of these tumors, 56.8% were papillary, 30.9% follicular, 9.3% anaplastic or undifferentiated, and 2.1% medullary. There was a female predominance in all age groups and for all types of tumors, reflecting a total female to male ratio of 1.9:1. Papillary carcinoma occurred most commonly in young and young-adult patients, follicular in the middle-age group, and anaplastic in the elderly. The extent of the primary tumor, the presence of regional metastases, and the association with other thyroid nonneoplastic diseases such as lymphocytic and Hashimoto's thyroiditis, and nontoxic and toxic goiter were also recorded. Multicentric tumors were found in 38.9% of cases principally represented by papillary carcinomas. Regional lymph node metastases were observed mainly in young patients, and no correlation was found between thyroid cancer incidence and other nonneoplastic thyroid diseases.

[Choledochoduodenostomy. What are the indications in non-neoplastic pathology of the hepatic and common bile ducts?].
Questions connected with choledochoduodenostomy, particular as regards indications, are discussed on the basis of a personal series and of a critical examination of concepts reported in the literature. The operation should certainly be considered in non-neoplastic bile duct pathology, although there are some fairly rigorous limitations. The disadvantages of the method are minimised to some extent and the surgeon is urged, in certain cases, to enhance the positive aspects so as not to lose opportunities which might otherwise not have been grasped.
Glycosidases in serum of cystic fibrosis patients.
In a study of eight glycosidases in serum samples from 72 cystic fibrosis patients, 85 cystic fibrosis parents and 34 healthy and diseased controls, significant elevations of mean alpha-glucosidase levels were found in cystic fibrosis patients. All other glycosidases did not show any significant change. Mean alpha-glucosidase levels in obligate heterozygotes were the same as in control individuals. Moreover, alpha-glucosidase levels in cystic fibrosis patients correlated with the degree of clinical impairment as measured by the Schwachman score.
[Extension of total thyroidectomy to the cellulo-lympho-thymic tissue of the anterior superior mediastinum].
Stress is laid on the importance of the cellulo-lympho-thymic tissue of the anterior-superior mediastinum in demolition surgery for thyroid cancer. For this reason, a straightforward, reliable technique for removing the thyrothymic lamina by the cervical route has been prepared. Experimented on cadavers, this method has been used for superior mediastinic lymphnode exeresis in all cases where it was indicated.

Mitochondrial and nuclear glutamate dehydrogenases in Chinese hamster ovary cells in culture.
Nuclear glutamate dehydrogenase (EC 1.4.1.3) activity has been demonstrated in Chinese hamster ovary cells. Some characteristics of this enzyme have been examined and compared with those of the mitochondrial glutamate dehydrogenase from the same source. Differences were detected in the extent of the activation by inorganic phosphate, in the pH versus activity curves, in the affinity of the two enzymes for the cofactor NAD+ and in the electrophosretic mobility. A different rate of decay of the two enzymes has been observed in cells grown in the presence of chloramphenicol. Immunological studies show that, as in ox liver, the nuclear enzyme has specific antigenic determinants besides those in common with mitochondrial glutamate dehydrogenase. Finally, experiments of thermal inactivation indicate a higher stability of the mitochondrial enzyme.
Human alpha-fucosidase. Purification and properties.
Human placental alpha-fucosidase (EC 3.2.1.51) has been extensively purified and partially characterized with respect to kinetic and structured properties. Although the enzyme seems to be separated by DEAE-cellulose chromatography in two forms which differ in their molecular weight and thermostability, an interconversion between the two forms takes place during storage and/or electrofocusing so that the same peaks of activity, revealed by the latter technique, are found before and after DEAE-cellulose chrome. The heterogeneous peaks of activity revealed by isoelectrofocusing show a reproducible pattern in the different tissues examined, except in serum where their pI values are consistently more acidic.
Human alpha-fucosidase. Single residual enzymatic form in fucosidosis.
Four major forms of alpha-fucosidase (EC 3.2.1.51) activity were separated by isoelectrofocusing from sera of normal control individuals. All forms shifted towards less acidic pI values after neuraminidase treatment. In two patients affected with fucosidosis, only a single major acidic peak was observed and this was affected to a lesser degree by neuraminidase treatment. The kinetics of heat inactivation of the residual activity found in these two patients showed two decay rates while the controls showed only one rate. These data are considered in relation to the hypothesis of the existence of interconvertible thermolabile and thermostable forms of the enzyme which has been discussed in the preceeding paper. The residual alpha-fucosidase found in patients could be structurally altered so that its ability to form the thermostable higher molecular weight aggregates is impaired.
Characterization of human alpha-galactosidase A and B before and after neuraminidase treatment.
It has been previously reported that following neuraminidase treatment alpha-galactosidase A is converted into the B form, as revealed by electrophoresis. By a variety of techniques such as isoelectrofocusing, DEAE-chromatography and by enzyme kinetic parameters, no conversion of alpha-galactosidase A into B, or the reverse, could be detected after neuraminidase treatment. Only an apparent transformation of alpha-galactosidase A into B was revealed by Cellogel electrophoresis. In addition, a discrepancy was noticed between the pattern of electrophoretic migration on starch gel and Cellogel and the net electrical charges of the two alpha-galactosidases as deduced by isoelectrofocusing and DEAE-cellulose. Neuraminidase treatment did not affect the activity of alpha-galactosidase A towards the natural substrate, ceramidetrihexoside, but the activity of alpha-galactosidase B decreased by about 30% under the same conditions. The two forms of alpha-galactosidases A and B used in this study were extensively purified by classical procedures.
