Ano-rectal lesions in patients taking suppositories containing non-steroidal anti-inflammatory drugs (NSAID).

We report on 3 cases in which ano-rectal lesions were associated with chronic administration of suppositories containing non-steroidal anti-inflammatory drugs (NSAID). Lesions were characterized by erosions or ulcer in the rectum and by stenosis of the anal verge. Erosions or ulcers were observed in patients taking the drugs for a relatively short time. The lesions remitted after suspension of the treatment. The cases observed indicate that suppositories containing NSAID could induce ano-rectal lesions similar to those observed after the use of suppositories containing ergotamine.

Comparison of a new colon lavage solution (Iso-Giuliani) with a standard preparation for colonoscopy: a randomized study.

Iso-Giuliani is a new improved-taste isoosmotic PEG-electrolyte solution for colon cleansing. In a randomized study, we have compared the effectiveness, tolerance and patient acceptance of 3 dosage volumes of this solution with a standard preparation in 191 patients undergoing colonoscopy. Iso-Giuliani 3 1, and 4 1 proved better colonic preparations as compared with the standard preparation (P less than 0.01). However Iso-Giuliani, 21 was sufficient for satisfactory cleansing, especially in the rectosigmoid. Patients in the standard preparation group experienced more cramps than did those in Iso-Giuliani groups. Patient acceptance was significantly better for Iso-Giuliani than for the standard preparation. The ingestion of Iso-Giuliani represents a safe, effective and well-accepted method of colon cleansing, and is our elective method of preparation for colonoscopy.

Prevalence of autoimmune atrophic gastritis in vitiligo.

Gastric biopsies, and measurements of fasting serum gastrin levels and titers of antihuman parietal cell antibodies have been performed in 65 unselected patients with vitiligo. Histologic evidence of autoimmune atrophic gastritis has been obtained in 10 cases (15%), who were all positive for the antibodies and who had elevated gastrin levels. The study of gastric secretion after pentagastrin stimulation, performed in 7 of these patients, showed a markedly reduced acid output. The present study provides definite evidence of the association of autoimmune atrophic gastritis with a proportion of vitiligo cases and suggests the need for surveillance of these patients in terms of gastric neoplasia.

Candida albicans infection of gastric ulcer frequency and correlation with medical treatment. Results of a multicenter study.

This paper reports the results of a multicenter prospective study of 188 consecutive patients affected by gastric ulcer, verified by endoscopy, in whom the frequency of a mycotic infection of the lesion was evaluated as well as the eventual influence of such pathology on the efficiency of medical treatment, the healing rate, and the healing time. A mycotic infection, defined as penetration of the periulcerous mucosa by the fungi, was found in only 13 patients (6.9%). No significant differences were found in the healing rate and healing time among these patients treated with H2-receptor antagonists and a control group of 43 matched gastric ulcer patients treated in the same period with the same therapy. It would appear from the data that mycotic infections of the gastric ulcer do not modify the efficiency of medical treatment.

Colonic involvement in adult patients with Henoch-Schoenlein purpura.

Henoch-Schoenlein purpura is a haemorrhagic syndrome, rare in the adult but well known in paediatric pathology. The clinical manifestations are usually: arthralgia, skin rash, renal and gastrointestinal involvement. Although the small bowel is more frequently involved, cases of oesophageal, gastroduodenal and colo-rectal localization have been rarely reported. We describe endoscopic features of two cases of Henoch-Schoenlein syndrome with extensive colonic involvement. No previous endoscopic documentation of this localization has been reported in our available literature.

[Our experience with the effectiveness and tolerance of ibuproxam in rheumatology].

The tolerance and effectiveness of Ibuproxam were investigated in 54 patients with arthrosis, R.A., ankylosing spondylitis, and fibrositis. Statistically significant reduction of pain at the end of the treatment was accompanied by decreased limitation of joint movement and morning stiffness. Simultaneous investigation of clinical and biological tolerance showed that the drug is infrequently responsible for side-effects that are always of negligible importance. Oesophagogastroduodenoscopy in subjects with a prior history and/or signs of active pathology of the upper digestive tract did not reveal exacerbation or recrudescence of lesions, nor the appearance of acute lesions.

Antibiotic-associated colitis: always pseudomembranous?

Among the 14 cases of antibiotic-induced colitis, endoscopic features were: in 9 cases typical pseudomembranes, in 4 cases petechiae or ecchymosis or suggillation-like redness with near-to-normal surrounding mucosa. In one case induced by Spiramycin, a macrolide that had never been previously indicated as being responsible for the disease, the endoscopic picture was 'atypical' and directed us towards a severe ulcerative colitis. These forms are very rare and usually limited to the right or transverse colon which suggests, if possible, a total colonoscopy. The nosographic picture of these cases is not quite clear, since they could represent both the most severe stages within 'non-pseudomembranous' antibiotic-induced colitis and autonomous forms whose evolution does not contribute in any way to the formation of pseudomembranes.

Unusual case of colonic sub-obstruction by giant pseudopolyposis in Crohn's colitis.

Giant pseudopolyposis of the colon is a very rare local complication of ulcerative and Crohn's colitis. 10 cases have been reported in the literature available to us. All the patients were surgically treated for clinical symptoms of obstruction or radiological stimulation of carcinoma. In this paper we present a peculiar case diagnosed by colonoscopy and biopsy which regressed after eight months of medical treatment.

[Diagnosis of chronic reflux esophagitis. Role of endoscopic and histological examination].

Fibroendoscopic and histobioptic study of the distal oesophageal mucosa has been carried out in a series of patients suffering from gastro-oesophageal regurgitation. The regurgitation condition was verified and its extent established by means of anamnestic, spot-fluorographic and manometric-pHmetric investigations. On the basis of the analysis of relations between the extent of the oesophagitis assessed on the basis of endoscopy and that arising out of microscopic examination of the biopsy, it is concluded that there is no complete correspondence between the two techniques insofar as endoscopy can give false positive or false negative results. On the basis of these results and of those of functional investigations, histology is considered desirable even when the oesophageal mucosa is normal, if there are clinical and/or laboratory signs of gastro-oesophageal regurgitation.

[2 cases of Apud cell tumors of the rectum and colon].

Two cases of Apud cell tumour of the rectosigmoid are presented. The question of gross diagnosis and the therapeutic efficacy of polypectomy are discussed. Lastly, an account is given of the morphological, functional and embryogenetic features of these tumours.
