Low-renin primary hypertension in a young patient treated with dexamethasone.

We have studied a young untreated patient with low-renin hypertension. All the clinical tests were in normal range, except plasma renin activity, which was low. Also all traditional tests evaluating the adrenal function were normal. A test of supraphysiological ACTH stimulation revealed a partial enzymatic defect of adrenal 11-hydroxylase. The patient has been treated with dexamethasone (0.75 mg once daily), during 4 weeks. At the end of the treatment blood pressure reached normal values. The traditional adrenal function tests are not sufficient to exclude enzymatic abnormalities and their possible involvement in the pathogenesis of hypertension.

Gold storage in the liver: appearance on CT scans.

The authors present one case in which high attenuation values in the liver parenchyma on computed tomographic scans were found in a patient who had been previously treated with colloidal gold for rheumatoid arthritis. Reticuloendothelial storage of gold was proved by biopsy.

[Quantitative evaluation of the phase image in radionuclide ventriculography ].

A functional image is a representation that enhances a particular characteristic of organ behaviour (feature enhancement) with a concomitant reduction or elimination of other informations. The functional image of phase (PI), obtained by Fourier analysis, may be interpreted to represent the onset of counts reduction or contraction in each region of the heart. Phase Spreading (PS), is a parameter derived from PI and represents ventricular temporal behaviour including some spatial information. Phase analysis and PS are very sensitive tools in the diagnosis and characterization of cardiac pathology. PS is a specific indicator of CAD, allows the introduction of normal ranges and is an example of quantitative evaluation of functional images.

Right oblique subxiphoid view for two-dimensional echocardiographic visualization of the right ventricle in congenital heart disease.

Two-dimensional echocardiography in the right oblique subxiphoid view (ROSV) was used to visualize all right ventricular components in 351 patients younger than 2 years of age (100 normal and 251 with various types of congenital heart disease). The ROSV, which is anterior and parallel to the ventricular septum, displays all right-sided cardiac components, the left atrium and the aorta. This view provided the correct diagnosis in patients with pulmonary stenosis or atresia with intact ventricular septum, Ebstein's anomaly, tricuspid atresia, infundibular ventricular septal defect, tetralogy of Fallot and truncus arteriosus. In all patients studied with the ROSV, an excellent definition of morphologic characteristics, equivalent to that in the right oblique axial angiocardiographic view, was achieved. The ROSV should be used routinely to delineate the right ventricular anatomy in all infants with congenital heart disease.
