["Canadian" hernioplasty with embrication. Anatomy, surgical technic, and personal experience].

Among the numerous operations proposed for the treatment of inguinal hernia, the authors utilised Shouldice's method of hernioplasty in over 400 cases of non-recurrent hernia. After drawing attention to a few anatomical type specifications regarding the inguinal canal structures, the "Canadian" surgical technique is described in detail. The authors analyse the various methods of hernioplasty now used and conclude that Shouldice's method should be considered elective in the treatment of primary direct inguinal hernias and voluminous indirect hernias in adults.

Linear accelerator radiosurgery of cerebral arteriovenous malformations: current status.

228 patients affected by cerebral arteriovenous malformations (AVMs) underwent linear accelerator radiosurgery. Follow-up ranges from 1 to 100 months (mean 42 months). Complete angiographic obliteration was achieved in 47% of treated patients at one year and 80% at 2 years. 17 haemorrhages were observed after treatment and 6 patients died from them. No bleeding took place after complete angiographic obliteration. 11 patients suffered for radionecrosis. In 6 patients complete recovery was obtained with corticoid medication. The aim of this study is to present our results and to evaluate the effect of irradiation on the risk of bleeding after radiosurgery. Patients were considered at risk in the time lapse after irradiation and before angiographic obliteration or other definitive treatment or death. Patients were followed from the date of radiosurgery and the number of haemorrhages were recorded every six months. In our series the bleeding risk in patients harbouring incompletely obliterated AVMs decreases from 8% in the first year after radiosurgery to 0% starting from the 24th month of the follow-up.

Exploring visuospatial short-term memory defect in Alzheimer's disease.

The present study aimed at controlling two variables that may affect the visuospatial short-term memory of Alzheimer patients: visuospatial coding efficiency and response modality. Thirty patients affected by Alzheimer-type dementia with relatively spared visuo-perceptual functions were tested under three conditions, all of which employed the same kind of stimuli (visuospatial patterns). At all memory tasks, patients achieved scores significantly lower than those of 30 age- and education-matched normal subjects. Patients did not benefit from longer presentation time, nor did their performance improve with pointing response modality, although they performed perceptual pattern recognition as well as did controls. These data confirm that visuospatial immediate memory capacity is reduced in dementia.

Effect of testing procedure on Corsi's block-tapping task in normal subjects and Alzheimer-type dementia.

Corsi's block-tapping task was given to 30 normal subjects and 38 Alzheimer-type demented patients following two different procedures. The first is the most widely standardized (scoring criterion: 3 correct reproductions out of 5 sequences), while the second is more lenient since it does not require subjects to replicate a certain performance three times. Demented patients' scores were lower than those of controls in both conditions, and scores on the two tasks were significantly correlated for patients and controls. However, the requirement of replicating the visuospatial memory performances was more detrimental for demented patients than for controls so the two procedures cannot be considered equivalent.

Influence of thyroid hormone on androgen metabolism in peripuberal rat Sertoli cells.

The aim of the present study was to investigate the influence of thyroid hormones on androgen metabolism in Sertoli cells isolated from 3- and 4- week-old rats. Hypothyroidism was induced by the oral administration of 0.025% methimazole (MMI) from birth until the rats were killed at 3 and 4 weeks of age. Half of the MMI-treated animals were injected i.p. with L-triiodothyronine (T3 3 micrograms/100 g body weight) during the last week before death. Sertoli cells from all groups were initially cultured under basal conditions for the first 24 h and subsequently in the presence of testosterone with or without T3 for an additional 24 h. Hypothyroidism was associated with severe impairment of body as well as testicular growth. Indeed, body and testicular weights were similar in 4-week-old hypothyroid animals to those in 3-week-old control rats. Testosterone metabolism in Sertoli cells isolated from 3- and 4-week-old hypothyroid rats was mainly expressed by the lowering of 5 alpha-dihydrotestosterone + androstane 3 alpha, 17 beta-diol and an enhanced formation of 5 alpha-reduced steroids with poor androgenic properties (e.g. 5 alpha-androstane, 3, 17 alpha-dione (androstanedione), 5 alpha-androstane, 3-ol-17-one (androsterone)). Treatment of the same group of animals with T3 in vivo and in vitro did not influence the pattern of 5 alpha-reductase steroids substantially. The most striking finding in the Sertoli cells of 3-week-old hypothyroid rats was the dramatic enhancement of oestradiol formation which persisted to a lesser extent 1 week later.(ABSTRACT TRUNCATED AT 250 WORDS)

Brain stem and motor evoked responses in "locked-in" syndrome.

A patient suffering for an ischemic "locked-in" syndrome following a subarachnoid hemorrhage, was evaluated with brain stem acoustic evoked potentials (BAEPs), short latency somatosensory evoked potentials (SEPs) and motor evoked potentials (MEPs). Neuroradiological findings failed to reveal any lesion in the brain stem, while a transcranial Doppler showed increased flow velocity upon the basilar artery, suggesting vascular spasm. BAEPs were normal and SEPs showed to be slightly impaired while MEPs upon magnetic cortical stimulation were heavily deranged. Neurophysiological investigations appeared remarkable for the early diagnosis of the disease, when neuro-radiological findings were still negative.

Linear accelerator radiosurgery of cerebral arteriovenous malformations: an update.

One hundred eighty patients affected by cerebral arteriovenous malformations (AVMs) underwent radiosurgical treatment in our department. One hundred fifty-three patients have been treated with complete irradiation of the entire AVM nidus. In 27 patients (with large and/or three-dimensional irregular target volumes), only part of the nidus was covered with a dose adequate for obliteration. Follow-up ranged from 88 to 1 months (mean, 43.1 mo). Angiographic control was performed at 12, 24, and 36 months until complete obliteration was attained. The complete obliteration rate was 46% at 1 year and 80% at 2 years. We observed 15 hemorrhages after treatment, and five patients died from them. No bleeding took place after complete angiographic obliteration. The aim of this study is to evaluate the effect of irradiation on bleeding risk after radiosurgery and before complete obliteration. Inclusive parameters of patients considered at risk were as follows: 1) all patients in the time lapse between irradiation and demonstrated complete angiographic obliteration; 2) all patients in the time lapse between irradiation and definitive treatment either by surgery or embolization; and 3) all patients in the time lapse between irradiation and death. These groups include all irradiated patients who still had incompletely obliterated AVMs. They were stratified starting from 0 time (the date of radiosurgery), and the hemorrhages were evaluated every 6 months. In totally irradiated cases, the bleeding risk decreased from 4.8% in the first 6 months after radiosurgery to 0% starting from the 12th month of the follow-up.(ABSTRACT TRUNCATED AT 250 WORDS)

Modifications of proteoglycans produced by human skin fibroblast cultures during replicative senescence.

The properties of proteoglycans (PGs) produced by normal human skin fibroblasts were investigated with increasing passage. The increase of subculture number was associated with a constant increase in PG molecular size, which was particularly evident in cell layer extracts. In the cell layer, the ratio of DS-PGs/HS-PGs was markedly higher in early passage cultures. Moreover, the cell layer from young cells contained lower amounts of radioactivity incorporated into the most hydrophobic PG populations, suggesting that the PG core protein might also undergo significant modification with increasing subcultures. There was no significant difference in energy charge value between early and late passage cultures, whereas the NAD/NADH ratio was found to decrease markedly in senescent cells.

Arcobacter cryaerophilus and thermophilic campylobacters in a sewage treatment plant in Italy: two secondary treatments compared.

Microaerophilic organisms were monitored in sewage effluent undergoing two secondary treatments: air and oxygen-activated sludge. The mean numbers of Arcobacter cryaerophilus and thermophilic campylobacters detected in incoming sewage were 5639/100 ml and 1720/100 ml respectively. Secondary treatment in air tanks reduced the population of A. cryaerophilus by 97.1% and of thermophilic campylobacters by 99.08%, whereas treatment in oxygen tanks reduced the bacteria 97.8% and 99.63% respectively, showing that oxygen-activated sludge treatment was more effective. Subsequent tertiary treatment with 2 p.p.m. chlorine dioxide evidenced the removal of A. cryaerophilus to 99.9% and eliminated thermophilic campylobacters. Campylobacter jejuni and C. coli constituted 54.1% and 45.9% of 74 thermophilic campylobacter strains isolated. In air-activated sludge effluent C. jejuni was found more often, thus appearing more sensitive to oxygen. The most probable number assay used for detection of campylobacters, blood medium for enrichment and blood-free medium for plating, also appeared to be fit for A. cryaerophilus, the high density of which in urban sewage may be due to inflows from slaughterhouses.

Effect of oxygen tension and lactate concentration on keratan sulphate and chondroitin sulphate biosynthesis in bovine cornea.

Calf cornea slices were incubated with [U-14C]glucose, in varying pO2 or lactate concentrations. Acid glycosaminoglycans were separated by ion-exchange chromatography after papain digestion. The percentage radioactivity incorporated into keratan sulphate increased markedly with decreased oxygen tension, whereas a concomitant relative decrease of the biosynthesis of glycosaminoglycuronans occurred. Similar results were obtained with increased lactate concentration. Our findings support the idea that keratan sulphate is a functional substitute for chondroitin sulphate in conditions of oxygen lack (Scott, J.E. and Haigh, M. (1988) J. Anat. 158, 95-108).

Modifications of adhesion properties and proteoglycan structure in rat embryo fibroblast cultures with increasing passages.

Adhesion properties of rat embryo fibroblast cultures and proteoglycans (PGs) produced both in the growth medium and in the cell layer were investigated with increasing passages. Both cell-cell and cell-substrate adhesion increased with increasing subculture number. Cell adhesion properties were improved by cell treatment with chondroitinase ABC. The increase in subculture number was coupled with a constant increase of PG molecular size, which was particularly evident in cell layer extracts. The ratio HS-PGs/DS-PGs increased with increasing passages. PG modifications are likely to represent evidence of changes in extracellular matrix organization and could play a role in the increase of cell adhesion properties.

A pilot model of vaccination against hepatitis B virus suitable for mass vaccination campaigns in hyperendemic areas.

A hepatitis B vaccination campaign was carried out in a town of 60,000 inhabitants, Afragola, Campania, Italy, a hyperendemic area for hepatitis B where HBsAg prevalence was 13.4% and anti-HBc prevalence was 64.7%. This experimental pilot project aimed to reduce the incidence of both acute and asymptomatic viral hepatitis B and of related chronic liver complications. From 1983-1989, 8,400 subjects were vaccinated: 6,900 children up to 10 years of age and 1,500 subjects from 11-60 years of age. High seroconversion rates were observed: 99.0% in all children under one year of age, 96.0% in the older children, and 86.7% in adults. The rate of infection in Afragola has diminished from 63/100,000 in 1983 to 10/100,000 in 1989. Carriers of HBsAg decreased in the general population (7.3% compared to 13.4%), especially in children up to 10 years of age (1.0% compared to 9.0%). In babies who received hepatitis B vaccine at the same time as compulsory vaccinations compliance was 98% while it was 80% in babies who were vaccinated separately. In June 1991 the Italian Parliament promulgated a decree which imposes hepatitis B vaccination for all newborn babies at 3, 5, and 11 months of age, at the same times as the mandatory childhood vaccinations (diphtheria, tetanus, and polio) according to a new protocol (Piazza scheme) which has been in use since January 1987 in our pilot vaccination campaign in Afragola.

[A rare case of intramedullary epidermoid cyst diagnosed by MRI].

Intramedullary epidermoid cysts are rare entities. A case recently seen on MRI led us to review the literature on the subject. A 9 year old boy developed in a four month-time a spastic paraparesis with a T7-T8 sensitive level. The MRI showed a widening of the spinal cord at the T6 level with an area of decreased signal on T1 weighted images, and a homogeneous round shaped area of increased signal on T2 weighted images. The authors discuss the contribution of MRI to the diagnosis of this lesion. The number of cases of intramedullar epidermoid cysts, like dermoid cysts, diagnosed with this neuroradiological method, is still very limited and the literature only mentions isolated cases.

[Arrhythmias from swallowing].

We describe the case of a 51-year old, non cardiopathic patient, with recurrent attacks of supraventricular tachycardia induced by swallowing. In the existing literature we found several descriptions of hypokinetic arrhythmias, easily explained by a mechanism of vagal inhibition. The cases of predominantly hyperkinetic arrhythmias, however, are much less common. In these patients the origin of the disease seems to be due to sympathetic oesophageal fibers and superior and medium cardiac nerves. In the present case, as in the others reported in the literature, the drug of choice seems to be Amiodarone which appears to be the most effective in preventing tachyarrhythmias caused by swallowing.
