Malignant disease of the kidney. An appraisal of diagnosis and management.

The diverse systemic effects associated with malignant renal tumours are described. It is emphasized that their recognition is essential for the early diagnosis of the tumour and that many of these effects may be overlooked unless the clinician is alert to their significance. Many of these early diagnostic clues also have a prognostic value.Although the basic management of a patient with a renal tumour continues to be a nephrectomy, the importance of tumour staging in relation to radical surgery is emphasized. Adjuvant therapy by radiotherapy, drugs, or immunotherapy is described and evaluated.

Testicular biopsy in evaluation of male infertility.

Testicular biopsy findings in 100 infertile men were correlated with the clinical findings. Mild or moderately severe tubular lesions were seen in 57 cases and severe changes in 43. Clinical examination and semen analysis were no guide to the severity of the testicular lesion. Though patients with normal sized testes more commonly had mild tubular lesions, many were severe. Patients with small testes more often had severe lesions but some had only mild tubular changes. Biopsy findings in both aspermic and oligospermic patients ranged from normal to a complete loss of germinal tissue.Testicular biopsy is advocated in infertile men for the complete assessment of the case and for identifying those which are potentially treatable. Patients with a severe lesion can be spared further investigations. The choice and results of treatment are discussed, particularly the surgical treatment of varicocele or obstruction. Only patients with a mild or moderate testicular tubular lesion should participate in future trials with drugs for male infertility.

Concentration of antibacterial agents in interstitial tissue fluid.

The concentration of antibacterial agents in the interstitial tissue fluid has been studied in an experimental model using implanted perforated Silastic capsules (tissue cages). Tissue fluid concentrations were always lower than the initial peak concentration in the serum, but for those drugs that were more slowly excreted the tissue fluid was similar to the serum concentration after six hours. In contrast the concentration in the tissue fluid for those drugs that are excreted rapidly was unpredictable, being either negligible or maintained at concentrations significantly better than in the serum. There was no evidence of accumulation in tissue fluid with regular dosage. These results indicate that the tissues may never be exposed to an adequate concentration of some antibacterial agents. This may be of clinical importance in tissue infections. Antibiotics which are rapidly excreted should be given in large enough doses and often enough for adequate blood levels to be maintained, so allowing equilibrium with tissue fluid to be achieved.

Retroperitoneal fibrosis associated with malignant disease.

The clinical features of 19 patients with malignant infiltration of the retroperitoneal tissues are described. These patients usually presented with unexplained uraemia and nonspecific symptoms; only a few had other evidence of malignancy. The diagnosis was resolved only by histological examination of multiple biopsy specimens. Since the prognosis is usually very poor it is essential to distinguish this condition from non-malignant causes of retroperitoneal fibrosis.
