[The surgical treatment of hepatocarcinoma].

At the National Cancer Institute of Milan, 118 patients underwent surgical resection and 35 had liver transplants for HCC (hepatocarcinoma). Postoperative mortality was 14% and 17% after resection and transplantation respectively. The five-year survival was 35% for resected patients and 83% at 30 months for transplanted patients. Analysis of the literature shows that patients receiving a liver transplant for HCC stage I-II survive longer than those resected at the same stage.

[The determination of polyamines in tumors of the maxillofacial area].

Putrescine, spermidine and spermine are polyamines deriving from ornithine. These are vital molecules for cell duplication processes; in fact, enzyme inhibitors responsible for synthesis are able to block cell multiplication. It is interesting to observe that there is an increased concentration of the polyamines and enzymes responsible for synthesis in solid tumours in man and in biological fluids in subjects carrying tumours. The authors assayed the polyamine concentration in tumour tissue taken from 10 cases of parotid carcinoma, noting a considerable increase in their levels compared to healthy control tissues. The hypothesis of being able to use the level of polyamines present in biological fluids in order to make an early diagnosis of tumours has now been definitively abandoned, both due to the scarce specificity and sensitivity of the available methods and due to the fact that increased polyamine concentrations were also found in physiological conditions and in the presence of non-tumour diseases; however, it is now being examined whether it would be possible to use polyamine levels to assess the degree of biochemical tumour malignancy or to evaluate the response to surgical or pharmacological treatment in patients with malignant tumours.

Reappraisal of the role of liver transplantation in the treatment of hepatocellular carcinoma arising in cirrhosis.

Several retrospective analyses have recently shown the advantage of liver transplantation (OLT) for patients with hepatocellular carcinoma (HCC) at early tumor stages. Preliminary results of a prospective series of OLT for unresectable small HCC arising in cirrhosis are reported. Eighteen out of 22 patients (82%) are alive without evidence of HCC recurrence after a median follow-up of 11 months.

[The psychopathological aspects of oral lichen planus (OLP)].

The etiopathology of oral lichen planus (OLP) is still uncertain. It is probably caused by a T lymphocytes cell-mediated autoimmune reaction. This reaction, caused by different pathogenic noxae, develops in subjects with a specific genetic pattern (HLA-DR on the keratinocytes) and some psychic attitude (higher Hamilton anxiety scale (HAS) and Hamilton depression scale (HDS) scores. In literature a psychic component has been often mentioned in OLP etiopathology. This work is a case-control study of replication: it aims to evaluate the importance of psychopathologic trait in patients affected by OLP. The study has been carried out on 16 patients (7 male and 9 female--average age 56) with OLP histologically confirmed. Before biopsy, they had been tested with: General health questionnaire (GHQ). Hamilton anxiety scale (HAS). Hamilton depression scale (HDS) with melancholia scale (MES). We have also studied a survey group of 14 patients affected by leukoplakia and neoplasms of the oral cavity (12 males and 2 female--average age 51). Before biopsy the survey group has been tested with the same questionnaire faced by the studied group. We chose the survey group as follows. Patients with leukoplakia show such similar lesions to OLP that the diagnosis is often histological; as far as patients with oral cavity tumours are concerned we hypothesized a more than usual anxious and depressed behaviour. We compared the score of the OLP patients to the one of the survey group patients using the "One-way Analysis of Variance" (ANOVA) method. The results confirmed that the patients with OLP showed very high scores in HAS and HDS, while the survey group scores were significantly lower.(ABSTRACT TRUNCATED AT 250 WORDS)

[The clinical applications of biomaterials].

The more and more availability of new alloplastic materials in oral and maxillo-facial reconstructions causes the necessity of judging if the biomaterials satisfy the surgical needs. The authors re-examine the main chemical, physical and biological characteristics of following materials: hydroxylapatite, polytetrafluoroethylene, polyglactin 910, polyfluorocarbons, polypropylene and report their experiences.

[Malignant mesenchymal neoplasms of the oromaxillofacial area. The diagnostic problems].

Oral and maxillo-facial mesenchymal malignant neoplasms are rare. They involve nosologic, diagnostic and therapeutic problems. Clinical cases of soft and bone tissue are reported. They share difficulties in clinical, roentgenographic and histopathological diagnosis and often the disagreement between the clinical evidence and the histopathological report. The rule of the immunohistochemistry for a correct characterization of mesenchymal malignant neoplasms and the necessity for correct and early diagnostic definition in order to plain the treatment are emphasized.

Autoradiographic and flow-cytometric assessment of cell proliferation in primary colorectal cancer: relationship to DNA ploidy and clinico-pathological features.

Two cell-kinetic parameters, the [3H]thymidine-labeling index [3H]dT LI and the flow-cytometric S-phase cell fraction (FCM-S), and DNA ploidy were determined for a prospective series of 110 primary colorectal cancers. Aneuploidy was observed in 66% of tumors and more than one aneuploid peak was present in 12%. The frequency of aneuploid tumors was higher in rectal (80%) and left-colon (70%) cancers than in right-colon cancers (51%), and multiple aneuploid clones were detected more frequently in men than in women (p = 0.03) and more frequently in advanced Dukes' D-stage patients (p = 0.08). The median [3H]dT LI value (17.4%) was similar to the FCM-S value determined by a planimetric model (16.2%) and somewhat higher than the FMC-S value obtained by an optimization procedure (11.2%). However, there was no significant relationship between the [3H]dT LI value and either FCM-S value for individual tumors. Moreover, FCM-S values were higher in aneuploid than in diploid tumors, whereas [3H]dT LI values were independent of DNA-ploidy status; [3H]dT LI and FCM-S were also related differently to some clinical and pathological features such as tumor site and histology. These findings suggest different biological meanings for these 2 cell-kinetic parameters, which should not be used interchangeably.

[A case of a carcinoma arising in lichen planus in a subject with diabetes mellitus and arterial hypertension (Grinspan's syndrome)].

The authors describe a clinical case observed at the Inst of Oral & Maxillofacial Surgery characterized by three simultaneous symptoms: arterious hypertension, diabetes mellitus, oral erosive lichen planus. Grinspan & his co-workers observed this syndrome. The results of their research were exposed at the Congress on Dermatology in Buenos Aires, 1963. In 1965 in a repetitive study Grupper & Avul seemed confirm the existence of this symptomatological triad; therefore the authors defined this complex as "Grinspan's syndrome". Further researches conducted by other authors (Jolly, Powell. Howell e Rick) confirmed the association hypertension-diabetes-"erosive" lichen. However there were other worthwhile studies (Lozada-Nur, Christensen) which emphasized a not significant high frequency of patients with diabetes and high blood pressure and the oral sugar tolerance like random specimens of common people. Besides the authors notice that their clinical case is interesting not only because of their simultaneous presence of the three symptoms but also because of the bioptic appearance of a cancer likely appeared on a pre-existing inveterate lichenoid lesion.

Changes in alpha-1 and beta-2 adrenoceptor density in human hepatocellular carcinoma.

Catecholamines are involved critically in the mechanisms of liver cell proliferation by acting on hepatic alpha-1 and beta-2 adrenoceptors. To identify the role of these receptors in human hepatocellular carcinoma (HCC), the density was examined of alpha-1 and beta-2 adrenoceptors with their affinity and coupling of beta-2 adrenoceptors to adenylate cyclase in HCC tissue and in nonadjacent/nontumor tissue from the same livers. Studies were also done on healthy livers from age-matched and sex-matched patients undergoing abdominal surgery for nonhepatic diseases. Twenty-two HCC had a decrease of about 72% in alpha-1 adrenoceptor density compared with their nonadjacent/nontumor tissue and a decrease of about 40% compared with healthy controls. Nonadjacent/nontumor tissue from HCC patients had a 125% increase in alpha-1 adrenoceptor density compared with healthy livers. Twenty-three of 24 HCC had an increase of about 180% in beta adrenoceptor density compared with their nonadjacent/nontumor tissue and healthy controls. Beta adrenoceptors were coupled to adenylate cyclase, as evidenced by a guanosine triphosphate-mediated right shift in (-)-isoproterenol competition isotherms and by cyclic adenosine monophosphate (cAMP) production after stimulation with (-)-isoproterenol. The HCC tissue yielded a larger increase in cAMP than nonadjacent/nontumor tissue and healthy controls. The authors conclude that a higher density of alpha-1 adrenoceptors in nonadjacent/nontumor tissue from HCC characterizes the "healthy" part of the liver in HCC patients and that an increase in beta-2 and a decrease in alpha-1 adrenoceptor densities characterize the tumor part of the liver in human HCC.

Role of estrogens and epidermal growth factor in hepatocellular carcinoma (HCC).

Estrogen (E) and epidermal growth factors (EGF) receptors were assayed in the liver of nine patients with hepatocellular carcinoma (HCC). Total E and nuclear E receptors were decreased significantly in neoplastic tissue as compared to the levels found in surrounding nonneoplastic tissue. The EGF receptor was decreased also in neoplastic tissue. On the basis of binding data, a decrease in the number but not in affinity of both the E and EGF receptors was found.

Quantitation of estrogen and androgen receptors in hepatocellular carcinoma and adjacent normal human liver.

Sex hormones have been shown to influence the development and course of several liver diseases. The worldwide predominance of hepatocellular carcinoma (HCC) in males has led to the suggestion that this disease might be hormone-responsive. Therefore, the hepatic estrogen (ER) and androgen receptor (AR) status of liver specimens from such patients was investigated. Samples were obtained from three female and six males patients undergoing liver resection; in each case, a small sample of both the tumor and adjacent normal tissue was collected. All patients had primary hepatocellular carcinoma without cirrhosis. In most cases, the tumor and the normal specimen had an equivalent content of cytosolic ER; however, three of the tumor samples (one female and two male) displayed considerably elevated cytosolic ER levels as compared to that of the normal tissue. In every sample, the tumor contained less nuclear ER than did the normal liver. When AR was measured, tumors of three patients (one female and two male) demonstrated a twofold elevation in cytosolic AR as compared to adjacent normal tissue. In the two male patients, an approximately twofold greater nuclear AR was found. Two other samples from male patients showed a modest elevation of cytosolic AR in the tumors. The patients whose tumors showed elevations in ER were not the same patients as those in whom the AR was elevated. Thus, these studies indicate that certain, but not all, specimens of HCC demonstrate either elevated ER or AR and suggest that a determination of receptor content might be useful prior to initiation of certain antihormone therapies.

[Lingual innervation in the buffalo].

The lingual and papillary mucosa innervation has been studied in the buffalo by histochemical and immunohistochemical techniques and by gold impregnation method of Ruffini. In lingual mucosa the presence of ganglionic structures has been noticed in which there are numerous cellular bodies and bundles of fibres variously directed. Particularly in the vallate papillae, the papillary stroma contains myelinic and unmyelinated fibres and terminal nerve corpuscles. Along stromal myelinic fibres the presence of the proteins S-100 and NF has been demonstrated.
