[Sarcoma phyllodes of the breast. Apropos of a case withan ultrastructural and histoenzymological study].

One case of cystosarcoma phyllodes is reported. Histologically, the neoplasm was composed of two elements: benign epithelial cells and malignant stromal cells. The stromal cells, spindle shaped, grow in solid sheets or in sparcely cellular, myxomatous and alcianophilic areas. Scanning electron microscopy demonstrates a mucous secretion, showing porelike openings and mucous droplets on the surface of tumoral cells.By transmission electron microscopy, sarcomatous cells look like very polymorphous. Secretory cells (showing dilated granular endoplasmic reticulum and abundant lysosomes) are mixed with primitive mesenchymal cells and with intermediate, myoepithelial cells. The myoid origin of these cells is demonstrated by the bundless of myofilaments terminated in marginal plaques on the plasma membrane, the numerous pinocytic vesicles and basal lamina investing irregularly the cells. Histoenzymology corroborates these findings, disclosing a high activity of alkaline phosphatase. Discovery of ambiguous myoepithelial cells associated with undifferentiated cells, likewise reported in epitheliomas, is of a great histogenetic interest. It suggests development of mammary neoplasm from stem cells coming under local of general influences, unknown today, but perhaps responsible of epithelial or conjunctive differentiation. The presence of a contralateral carcinoma in our case is consistent with this hypothesis.

[Classification of benign tumors and cysts of the jaws].

On the basis of 324 cases of maxillary tumours, the authors review the classification of these neoplasms. The most frequent tumours are benign epithelial odontogenic tumours, which represent 55% of all tumours. The most common of these tumours is the ameloblastoma, followed by the odontomas. Most of the tumours of non-dental origin are benign (72% of this group). Amongst the malignant tumours, osteosarcomas are much more common than chondrosarcomas. The authors also discuss the various types of cyst and their respective relative frequency.

[Angiomas of the jaws. Diagnostic and therapeutic problems (apropos 12 cases)].

Difficulties encountered during surgery on maxillary angiodysplasias were studied in relation to case-reports of patients treated in the stomatology and maxillofacial surgery unit directed by professor Vaillant, Hôpital Salpêtrière, Paris. Findings allowed certain conclusions to be drawn with respect to diagnostic and therapeutic features and to compare these with those reported in the international literature.

[Mandibular mucoepidermoid tumor].

One case of mandibular mucoepidermoid tumour was observed in a 57 years old man. A progressive swelling of the mandible was shown with an osteolytic feature on roentgenograms. It was treated by hemimandibulectomy. Grossly, the neoplasm was partly cystic. Histologically, it was mainly composed of intermediate clear cells. Its ultrastructure was quite similar to that of mucoepidermoid tumours in other sites. Heterotopic salivary lobules, though lacking in our case, seem to play the major role in the genesis of these tumours.

[Proprioceptive sensitivity and orofacial functions].

Proprioceptive sensibility from stimulation of muscle, ligament articular and vestibular receptors plays a determining role in the regulation of tone, of the resting position of the mandible, of head posture and of the closure pathway of the mandible. Studies conducted on temporomandibular joints of fetuses and adult subjects failed to demonstrate the specialized corpuscles (a fact previously noted by Ramfjord) in the temporomaxillary joint capsule, described typically in other joints. In contrast, however, histology showed a particularly rich population of muscle receptors adjacent to this joint, this being only one of several particular characteristics.

[Parotid sialadenosis. A histoenzymologic and ultrastructural study].

A case of sialadenosis was studied by means of several morphological methods (light microscopy with micrometric measurement, transmission and scanning electron microscopy, immunopathology, histoenzymology). In that disease, these investigations seem to corroborate the hypothesis of a dyscrinism, true perturbation of the salivary excretion with storage of secretory granules in the cells and consecutive hypertrophy. This dyscrinism should result from a deterioration of branches of autonomic nervous system. In the case reported here, no etiology was proved but the hypothetic part of a recent antiulcerous treatment by Tagamet was suggested.

[Primary malignant tumors of the heart. Anatomo-clinical study of 12 cases].

Five of the 12 cardiac sarcomas reviewed in this study were recruited from 7,200 autopsy studies, including 2,980 performed on patients dying from malignant disease (overall incidence of 0.07 p. 100; 0.16 p. 100 in the group with malignant disease). These tumours are fifty times less common than cardiac metastases which were observed in 12.5 p. 100 of patients in the group with malignant disease. The other 7 cases of cardiac sarcoma were recovered from our surgical pathology archives over a 16 years period. The sarcomas were of the following histological types: angiosarcoma (6 cases), rhabdomyosarcoma (3 cases), fibrosarcoma (2 cases) and mesothelioma (1 case). The clinical presentation was very variable. The diagnosis was often made only at surgery or autopsy. The prognosis was very poor, the majority of patients dying of cardiac constriction or visceral metastases, within one year of diagnosis.
