Human spumaretrovirus-related sequences in the DNA of leukocytes from patients with Graves disease.

Viruses, and more particularly retroviruses, have been postulated to play a role in the pathogenesis of autoimmune diseases. In a search for spumaretrovirus infection markers, we screened a group of 29 patients with Graves disease and a representative healthy population (23 subjects) as a control. Southern blot hybridization under stringent conditions, of patients' DNA extracted from peripheral blood lymphocytes, with a spumaretrovirus-specific genomic probe derived from the human spumaretrovirus (HSRV) prototype, gave a positive signal in 10 cases. Moreover, by PCR, HSRV-related sequences were detected in the DNA of 19 patients (66%). Positive DNA samples in Southern blots were also positive in PCR for all regions tested (gag, bel1, bel2, long terminal repeat). Amplified (gag and bel2) products were cloned and sequenced; they showed high homology with HSRV. On the other hand, all 23 control subjects were negative by both procedures. Sera from both populations were examined for the presence of antibodies reactive with antigens of the spumaretrovirus family. These sera were negative by several immunodetection techniques: ELISA, indirect immunofluorescence, serum neutralization, and Western blotting. These results strongly suggest the existence of an association between Graves disease and the presence of HSRV-related infection markers.

Insulin-dependent diabetes and HLA.

The study of a hundred and fifteen unrelated insulin-dependent diabetes and eight families with at least two insulin-dependent diabetes members made it possible to confirm the higher frequency of HLA-B8 and B18 (p less than 0.001) among patients, producing a RR of 2.24 and 2.47 respectively. The increased B15 frequency did not achieve statistical significance. B18 whose gametic association (delta = 0.0438) was significant only in diabetic patients was often related to Aw19-2 (Aw30 + Aw31). The B8/B18 genotype gave a relative risk (RR = 4.98) which was significantly higher than that of B8, B18 and B15 heterozygotes (1.50, 1.24 and 1.39 respectively). Pairs of diabetic siblings were more frequently HLA identical than would be expected by chance, and distribution of the pairs of affected sibs into the three categories, identical, semi-identical and different, was closer to the recessive model than to the dominant one. The fact that the B8/B18 individuals had a RR slightly higher than the B8 and B18 homozygotes and distinctly higher than the heterozygotes for only one of these genes, favours the hypothesis of two dominant genes, giving the appearance of recessivity. The gene associated with B18 in Southern Europe seems to play the same part as that of the gene associated with B15 in Northern Europe.

[A case of mediastinal parathyroid adenoma visible on standard pulmonary radiography (author's transl)].

Mediastinal adenomas form about 10 % of all parathyroid adenomas, and are only very rarely visualized in standard pulmonary radiograms. This was the case however in a 54-year-old woman with hyperparathyroidism, chronic renal insufficiency, and an anterosuperior mediastinal tumor. After surgical excision by sternotomy the mass was found to be a large parathyroid adenoma weighing 105 g and measuring 8 cm in diameter. The authors discuss the problems raised by mediastinal adenomas and review the cases reported in the published literature.

Adenoma

[Intrathyroidal parathyroidal adenoma associated to a cold thyroidal nodule (author's transl)].

Parathyroid adenomas may have ectopic localizations, for example, intrathyroidal, which diagnosis is difficult. The present report illustrates this fact. A patient, 45 years old, euthyroidal, with a cold thyroidal nodule, underwent a right thyroidal lobectomy. Immediately after the operation, a tetany crisis occured with a Chovstek's sign and a 65 mg/l blood calcium level. The possibility of an accidental parathyroidectomy was evoked, and a second histological exam of the tumor showed the presence of a trabecular parathyroidal adenoma in the back of a vesicular thyroidal adenoma. The presence of this parathyroidal adenoma should have been suspected because of the occurence of nephretics colics in the antecedents, but there was a high blood uric acid level and the diagnosis of uric concretion was retained; no dosage of blood calcium level has been done.

Adrenal Insufficiency

[Contraception in diabetic women (author's transl)].

Contraception in diabetic women is necessary in order to plane pregnancies. Patients with chemical diabetes or family story of diabetes, babys with birth's weight over four kilogrammes, pregnancy's diabetes, may not use oestroprogestative pills. In this group progestins minipills can be used. Insulin-treated women may use oestroprogestative minipills as well as progestins minipills. In both groups, metabolic-controls must be performed every three months. It is also necessary to look at the funduns regarding to the unknown effects of contraceptives on diabetic microangiopathy. One or more other contrindication to oral contraceptives obliged to use intra-uterine device.

Continuous infusion of insulin (4 u/h) was performed in 9 insulin treated adult diabetics before and after CB 154 (2.5 mg/24 h). In 4 patients GH secretion do not exist under insulin infusion before CB 154 in spite of mean blood glucose level decrease of 1.5 g/l. In the same group GH secretion appears after CB 154. In the 5 others patients CH secretion is induced by blood glucose level decrease and became higher after CB 154.
