[Dual mesh-plus for wall reconstruction in incisional and umbilical hernia in the aged].

The article reports the author's experience about the use of Gore-Tex Dual Mesh Biomaterial in large incisional hernia operations in patients over seventy. From January 1996 at present day we operated on 23 patients for treatment of abdominal wall defect. In all patients we used a Dual Mesh. Overall mortality was O. Morbility was 9.5%. Follow-up is too short for definitive considerations about the incidence of recurrences but our initial experience with this material encourages us to use it again for replacement of abdominal wall defects especially in the old patient.

[Clinical and electrophysiological findings in various hereditary sensory neuropathies].

An electrophysiological study, comprehensive of peripheral sensory and motor conduction velocity (SCV, MCV), motor cortical stimulation (CS), median nerve somatosensory evoked potentials (SSEPs), brainstem evoked potentials (BAEPs) and sural nerve biopsy, was performed on 100 hereditary ataxia patients: 48 with Friedreich's ataxia (FA), 18 with Early Onset Cerebellar Ataxia (EOCA) and 34 with Autosomal Dominant Cerebellar Ataxia (ADCA). An early "peripheral" and "central" sensory impairment was observed in FA probably due to axonal loss and not related to disease severity or duration. On the contrary, BAEP and CS findings suggested a progressive involvement of the auditory and motor pathways. The presence of a non progressive sensory neuropathy allowed a distinction of EOCA patients in two groups: with and without peripheral neuropathy. The clinical and genetic heterogeneity was confirmed by the variability of evoked potential results. The ADCA patients showed the mildest degree of electrophysiologic abnormalities with an involvement of the peripheral pathways, both sensory and motor, more frequent than the central ones.

Production of cytokines at the operation site.

BACKGROUND AND AIM: Cytokines are part of a family of molecules involved in the initiation, control and termination of the events that occurs in wound healing process. Aim of this study was to evaluate the production of some cytokines [interleukin (IL)-6, IL-10, IL-1alpha, IL-1ra, interferon (IFN)-gamma] in the drainage wound fluid from patients undergoing incisional hernia repair. METHODS: Ten female patients with abdominal midline incisional hernia undergoing to surgical repair were included in this study. In all cases a closed suction drain was placed in the wound below the fascia and it was removed on the 4th postoperative day. Wound fluid was collected on the 1st, 2nd, 3rd and 4th day and its amount in each time was recorded. The production of IL-6, IL-10, IL-1alpha, IL-1ra and IFN-gamma were evaluated as quantity produced in 24 hour. RESULTS: In all patients the amount of drain fluid from surgical wound was highest on the 1st day after surgery, afterwards there is a significant reduction. The production of all cytokines evaluated was highest on the 1st day decreasing on the 2nd day except for IL-1alpha that not show any modification. The produciton of IL-1ra, IL-6, IL-1alpha and IL-10 was significantly reduced on the 3rd and 4th postoperative day in comparison with the respectively values recorded on the 1st day, whereas IFN-gamma levels were similar. CONCLUSIONS: The dosage of cytokines in the drain fluid led us to better evaluated the events that follow surgical wound and their analysis offers further information in the role of cytokines in healing process, with the goal to get supportive treatments to promote the best evolution.

[Inguinal hernioplasty with PHS: our experience].

Three hundred patients underwent inguinal hernia repair from January 2003 to December 2004; 40 patients, randomly selected, received the PHS (Prolene Hernia System) mesh. Surgery was performed under local anesthesia in 80% of cases and under spinal anesthesia in the remaining 20%. There were 29 men and 11 women; mean age was 58,5 years (range 39-78). Postoperative pain was light and morbidity was low; all patients had early return to normal activities. Particularly, there were differences between the self-employed and the employees: the former began work again 2-4 days after surgery in 85% of cases, while the latter began after 15-20 days. Immediate complications were rare and always minor: they included seroma (1%) and ecchymosis (1%). Follow-up examinations did not show any recurrences.

[Ultrasound-guided percutaneous injection of ethanol in Plummer's adenoma. Our experience with 8 patients].

The authors report their experience in the study of 8 patients showing symptoms of thyroid hyperfunction (Plummer's adenoma) and treated with US-guided percutaneous ethanol injection. The treatment consisted in injecting sterile ethanol in varying amounts (2 to 5 ml) according to nodule size, using a fine needle under US guidance. The patients underwent 3 to 6 injections, according to biochemical (T3-T4-TSH) and scintigraphic findings. Follow-up at 12 months showed regression of clinical symptoms, a trend of hormone levels toward normalization and recovery of previously suppressed parenchymal function. No significant complications were observed, except for a transient thyrotoxic crisis in the patient bearing the largest nodule. The treatment of Plummer's adenoma by means of percutaneous ethanol injection under US guidance appears to provide specific clinical and technical advantages over other conventional treatments.

Conventional electromyography in myotonic dystrophy.

In 31 subjects of 23 different families with unequivocal signs and symptoms of myotonic dystrophy EMG was performed with conventional technique on brachial biceps, extensor digitorum communis and tibialis anterior muscles. In 14 of these patients motor and orthodromic sensory conduction velocities were measured in median nerve as well. During full muscular effort, an interference pattern of low amplitude was seen in most of the explored muscles, while "after discharge" of different duration was present in 57-83% of the patients. At weak effort, mean potential duration was nearly always significantly reduced if polyphasic potentials were not included in the calculation; polyphasic potentials, however, exceeded the limits of normal incidence and their duration was longer than that of motor unit potentials of simple shape. At rest, spontaneous activity of short duration and positive sharp waves occurred in about 50% of the patients, but rarely in more than two places. Bursts of myotonic or pseudomyotonic discharges occurred either spontaneously or mechanically in all subjects. Conduction velocity along motor, or sensory, peripheral nerve fibres was constantly within normal values. Results were discussed in relation to the hypothesis of a primary neuronal pathogenesis of muscle wasting, and the conclusion was that nerve changes are to be considered as a collateral disorder dependent on the pleiotropic action of the gene.

Epidermoid cysts of the fourth ventricle.

The location of epidermoid cysts within the fourth ventricle is rare. The Authors present three cases operated on. The most frequent symptom was ataxia. Increased intracranial pressure and hydrocephalus were rare even when the tumors were large in size and filled up the fourth ventricle. Neuroradiological data are presented. Surgical removal was generally easy though the tumor invaded the cisterns and a true peritumoral capsule was found only in the lower part of the ventricular floor. The bursting of the cyst followed by continuous production of keratin with subsequent invasion of cisterns and foramina is suggested as a possible mechanism to explain the tumoral growing.

[Arterial hypertension: clinical significance of U wave inversion in electrocardiogram].

Negative U wave is usually observed in ischemic heart disease, left ventricular hypertrophy and in chronic cor pulmonale, sometimes in association with electrolytic imbalance or toxic effects of drugs. We assessed by ECG and echocardiography 559 patients with longstanding arterial hypertension. Negative U wave was significantly more frequent in patients with increased left ventricular mass and/or with congestive heart failure, particularly in the middle-age hypertension group. Negative U wave appears to be an ECG sign closely associated with the anatomical evolution toward hypertensive heart disease with hypertrophy and/or congestive heart failure.

[Pilot study of screening for the early diagnosis of tumors of the upper respiratory and digestive tracts in individuals at risk in the Friuli-Venezia Giulia region].

A pilot study on a screening program for tumours of the upper respiratory and digestive tracts was conducted from December 1988 to May 1989 in the Friuli-Venezia Giulia Region (Northern Italy), an area showing high mortality rates for cancers of these sites. Six hundred seventy-one patients at risk (i.e. habitual smokers and/or drinkers) were referred to an E.N.T. specialist by 16 general practitioners for free examination. Of these 436 (65%) actually underwent examination and, among them, 55 (12.6%) precancerosis and 8 (1.8%) epithelial tumors were detected. Although some low risk patients (mainly females) were eventually seen as well, two non-epithelial tumors being found, this pilot study would appear to have accomplished its purposes, namely to establish a closer relationship between GPs and ENT Specialists so as to discover cancerous and precancerous lesions early on and to address an educational message against smoking and heavy drinking to at risk subjects.

Fine needle aspiration cytology in a case of diffuse sclerosing carcinoma of the thyroid.

The fine needle aspiration (FNA) cytologic findings in a rare case of bilateral diffuse sclerosing carcinoma of the thyroid are described. Along with a generally papillary architecture, the FNA biopsy smears showed a number of features not usually seen in thyroid carcinomas: psammoma bodies, an abundance of squamous metaplasia and a dense inflammatory component. The FNA diagnosis was multifocal papillary carcinoma with squamous metaplasia; histopathologic examination made the final diagnosis.

Modalities of origin of the middle cerebral artery. Incidence on the arrangement of the perforating branches.

In a series of 100 anatomical dissections on the basal brain arteries a case of real duplication at the middle cerebral artery level has been found. The importance of such a finding is discussed in the light of literature data. The knowledge of similar anatomical patterns is fundamental in the study of the lenticulostriate arteries arising from the sphenoidal segment of the middle cerebral artery. In fact the neurosurgeon must be familiar with all the possible variations of these perforators, given that the opening of the Sylvian fissure is by now an obligatory manoeuvre for the transcisternal access to the circle of Willis.

A simple procedure for management of spontaneous cerebrospinal fluid rhinorrhea by injection of fibrin sealant. Technical note.

A simple and safe procedure is proposed to manage cases of "empty sella" complicated by rhinorrhea. It is performed under fluoroscopic control only by introducing a needle in the sphenoid sinus via a transnasal route and by injecting fibrin glue into the sinusal cavity. This procedure requires a short hospitalization and it allows to avoid the compliances of other techniques.

Hemangiopericytomas of the spinal canal. Report of four cases and review of literature.

The Authors report four cases of spinal hemangiopericytomas. The diagnosis has been ascertained by electron microscopy. Despite the malignant nature, the association of surgery and radiotherapy is interesting in that it prolongs the survival rate. Clinical, neuroradiological, microscopic and ultrastructural features of intraspinal hemangiopericytomas are evaluated in the light of the review of the relevant literature.
