Relationship between metabolic clearance rate of antipyrine and hepatic microsomal drug-oxidizing enzyme activities in humans without liver disease.

The aim of this study was to look for correlations between the metabolic clearance rate of antipyrine and (a) the concentration of cytochrome P-450, and (b) the NADPH cytochrome c reductase, aminopyrine demethylase, and aniline hydroxylase activities in the liver microsomes of 20 patients without liver diseases. The results confirmed interindividual differences in the values of in vivo and in vitro assessments of hepatic drug metabolism. No significant correlation appeared in this group of patients between the metabolic clearance rate of antipyrine and the microsomal amount of cytochrome P-450, whereas the metabolic clearance rate of antipyrine exhibited a significant relationship to the NADPH cytochrome c reductase (p < 0.05), aminopyrine demethylase (p < 0.05), and aniline hydroxylase (p < 0.02) activities. These results suggest that the metabolic clearance rate of antipyrine: (a) is not strictly related to the amount of cytochrome P-450 in hepatic microsomes; and (b) although significantly related to the activity of hepatic drug-metabolizing enzymes, does not have a predictive value to assess the enzyme equipment of liver microsomes in a given normal subject.

[Operative trauma of the common bile duct. Report of eight cases (author's transl)].

The authors report eight cases of operative trauma to the common bile duct; seven patients were sent up for reoperation often during the late stages following an attempt at immediate or secondary repair. Emphasis was placed on the interest of extensive resection of the stenosis and wide and high opening of the common bile duct for construction of the bilio-jejunal anastomosis, and furthermore the late risk of onset of portal hypertension requiring portacaval anastomosis, finally the possible degradation of the good initial results requiring difficult reoperations.

[Medullary carcinomas of the thyroid. General review apropos of 3 further cases].

The authors report of three cases of medullary carcinoma of the thyroid and, from a review of the literature, study the definition, frequency, embryological and etiological characteristics of this disease. The physiology of thyrocalcitonin, the concept of the APUD system the secretion of active substances by the tumour cells are considered. The macroscopic and microscopic characteristics and its mode of spread are described. The two main clinical forms, the sporadic form and the familial form are described together with the other endocrine involvements. Among the factors in positive diagnosis, accent is placed on per-operative histological examination and on biological tests of familial detection. Among the forms of treatment the essential place of surgery explains partly the relatively favourable course and prognosis of the malignant disease.

[Ultrasonic aspects of abdominal abscesses and hematomas].

Abdominal ultrasonography often enables to display the fluid collection of an abcess or haematoma. This type of diagnostic procedure is most useful in a post-operative period. The intestinal gases which stop the ultrasonic beam may be responsible for diagnostic failures. Positive results, which are the most frequent, are never the less precious to assess and decide a treatment policy.

[Large non-secreting islet cell adenomas. Review of the literature].

The authors report a case of benign non secretory islet cell tumour. Ten other cases of this type were found in the literature. These non-functional tumours évolve without symptoms and are rarely diagnosed in the presence of an abdominal mass. They are usually discovered at autopsy. Clinical examination and further investigation do not always demonstrate the pancreatic origin of the tumour observed. Treatment is surgical consisting of removal of the tumour. Pathological examination leads to the diagnosis of non-secretory islet cell tumour which is usually benign; only the course can confirm this.

[Congenital diverticuli of the common bile duct. Review of the literature].

Among congenital dilatations of the common bile duct, a congenital diverticulum of the common bile duct may remain clinically latent for a long period. One case is reported in a 77 year old subject. Precise diagnosis of the disease is often made at operation and on pathological examination. The diverticulum is cured in most cases after surgery which consists of simple removal, but which may sometimes require reconstruction of the common bile duct.
