Survival and locations of recurrence following abdomino-perineal resection for rectal cancer.

Records of 134 patients treated by abdomino-perineal resection (1971-1979) were reviewed. One hundred and thirteen had curative operations. Mortality was 2.6% and morbidity was 51%. Ninety-three were evaluable and were followed for 5 to 8 years; they were evaluated for survival and pattern of recurrence. Five-year survival for Dukes' A, B, C, and D lesions was 86, 62, 31, and 0%, respectively. Thirty-seven had recurrence: Four pelvic, nine pelvic and distant, and 24 only distant lesions. The overall incidence of failures was 47%, failure rates by stage were 11% for stage A, 27% for B, 48% for C, and 70% for D. Incidence of local recurrence was significantly higher in stage C compared to stage B. Irrespective of stage, after detection of local or distant recurrence, survival did not differ. Furthermore, radiotherapy for local recurrence and chemotherapy for distant lesions did not improve survival time.

Surgery of toxic goiter: indications and long-term results.

Treatment of hyperthyroidism is still debated, surgery in particular is conceived as the last step of a multidisciplinary approach. 130 patients were reviewed in a follow-up based on a double cell at fixed dates, after subtotal thyroidectomy for multinodular and diffuse toxic goiter. Clinical and laboratory tests (T3, T4, TSH) showed a rate of 9.1% of recurrent toxicity and 20.7% of hypothyroidism. These data confirm that surgery offers satisfactory results in the management of hyperthyroidism if indications are carefully followed. It is also shown that TSH has a predictive role in the screening of patients at risk of developing postoperative hypothyroidism.

Effects of CO2 laser on the healing of experimental colon anastomoses.

Anastomotic leakage after colonic resection still remains a troublesome complication of major clinical significance. Systemic (nutritional status, blood viscosity) or local factors (contamination by local germs, suture technique) seems to be implicated in the healing process of the suture line. The CO2 laser as a cutting procedure in colonic resection should provide a useful tool for its accuracy and the bactericidal properties. We have tested the strength of an end-to-end anastomosis of rat colon resected by CO2 laser, scalpel and diathermy, respectively, by measuring the bursting pressures at different postoperative days. The present study shows that the use of CO2 laser resulted in a significant increase of colonic bursting pressure at the anastomotic site.

Male sexual function after abdominoperineal resection for rectal cancer.

In a series of 26 male patients undergoing abdominoperineal resection of the rectum for malignant disease, a detailed history of sexual function was obtained, using a questionnaire before and 12 months after the operation. The overall incidence of sexual dysfunction was 61.5 per cent, total and partial erectile impotence being, respectively, both 27 per cent. Taking age into account, among men of the youngest age group (41-48 yrs), incidence of complete and partial erectile impotence was 14 per cent. In the middle age group (49-57 yrs), 22 per cent reported total and 33 per cent reported partial erectile impotence, whereas in patients of the oldest group (58-65 yrs), total erectile impotence was present in 40 per cent and partial in an additional 30 per cent. The extent of the disease (Dukes' stage) was found to be of no value as a prognostic index of postoperative sexual dysfunction. It is concluded that the age of the patients is the most important factor related to sexual activity after abdominoperineal resection for cancer.

Liver regeneration after atypical hepatectomy in the rat. A comparison of CO2 laser with scalpel and electrical diathermy.

Rats were subjected to partial hepatic lobectomy with removal of about two thirds of the liver either by scalpel, by CO2 laser, or by electrical diathermy. Macroscopic examinations of livers at various days after surgery indicated that CO2 laser surgery results in minor tissue damage, reduced complications from inflammation and infections, and faster healing compared to electrical diathermy, but it is less satisfactory than the scalpel with respect to all these parameters. Rat liver regeneration was followed at the molecular level by measuring the changes of protein biosynthesis activity in the liver, as monitored by modifications of ribosome organization. Activation of the protein biosynthesis process started 1 day after surgery when hepatectomy was performed by scalpel, while 2 days were required when CO2 laser or electrical diathermy were used instead.

Mucopoietic activity of zolimidine: is its effect through cellular turnover or by modification of the gastric mucosal blood flow?

Zolimidine 2-(methylsulphonyl-phenyl)-imidazo-(1,2-a)pyridine produces a high percentage of healing of gastro-duodenal ulcers. The action of the drug develops through an increase of protective factors on the gastric mucosal barrier. This study has the purpose of evaluating if the action of the drug is also mediated by microvascular factors. The analysis of the values of BAO, MAO and PAO confirmed that treatment with zolimidine does not modify gastric acid secretion; the values of the Neutral Red clearance at PAO, before and after the treatment, do not show a statistically significant variation of the gastric mucosal blood flow. According to the most recent literature, the increase of the gastric mucopoietic activity is carried out by a direct cellular action.

[Serum gastrin levels in patients with recurrent peptic ulcer].

Serum gastrin behaviour after stimulation with a protein meal was examined in 20 patients of both sexes suffering from x-graphically and endoscopically accertained recurring peptic ulcer, and a marked increase was found in the hormone in the majority of patients when observations were made 15 and 45 minutes after the meal. The genesis of this increase is therefore discussed, with consideration of the importance that extra-gastric sites of production of the hormone may have in such penomenon.

[In vitro thyroid diagnosis in surgery of the thyroid].

After a brief outline of the physiology of the thyroid hormones and the laboratory tests measuring thyroid function, the dosage of normalised T4 (T4N) in patients suffering from various thyroid diseases and subjected to surgical operation is discussed. As the fundamental presupposition of thyroid surgery is that the operation is made in conditions of euthyroidism, the use of a quick and reliable preoperative test giving an exact evaluation of the patient's thyrometabolic conditions is necessary. In conclusion it is stated, on the basis of experience acquired in 90 patients operated for various thyriod diseases, that the last thyrometabolic evaluation can be based on T4N dosage.
