High-resolution computed tomography (HRCT) has been described to be useful in assessing bronchiolitis obliterans (BO) syndrome in the transplanted lung. Currently, BO syndrome is diagnosed if lung function deterioration shows persistent or progressive irreversible airflow obstruction, with FEV1 of less than 80% of baseline values, without clinical evidence of infection. The aim of this study is to assess the value of HRCT in evaluating BO syndrome after allogenic bone marrow transplantation (BMT). Fourteen HRCT scans were performed in 6 women and 3 men with moderately severe irreversible airflow obstruction and a clinical diagnosis of BO syndrome after BMT. Two patients had normal HRCT scans. In the remaining 7 patients, 7 of 11 HRCT scans were abnormal, with non-specific findings of bronchial dilatation (1), consolidation (2), hypoattenuated areas (4) and vascular attenuation (4). In comparison with the consistency of positive HRCT findings of BO syndrome in transplanted lungs, HRCT surveillance for BO syndrome in post-BMT patients with clinically established disease is of limited value, unless other pulmonary complications such as opportunistic infections are to be excluded.
STUDY DESIGN: The computed tomography scans of two groups of patients of similar age and sex, with and without sacral insufficiency fractures, respectively, were assessed retrospectively. OBJECTIVES: To determine the incidence of sacroiliac joint vacuum phenomena in individuals with and without sacral insufficiency fractures and to evaluate a possible association between vacuum phenomena and sacral insufficiency fractures. SUMMARY OF BACKGROUND DATA: The occurrence of vacuum phenomena is well recognized in the vertebra, but not in the sacroiliac joint. Gas foci in sacral insufficiency fractures recently has been reported. It has been suggested that the presence of vacuum phenomena may aid in the diagnosis of these fractures. METHODS: The computed tomography scans of 28 female patients with sacral insufficiency fractures and of 60 age- and sex-matched control individuals were evaluated. RESULTS: In the group with sacral insufficiency fractures, vacuum phenomena were detected in 69.2% of patients and in 63.5% of sacroiliac joints, with sacroiliac joint-related osteophytes found in 19.2% of patients. Very similar incidences were demonstrated in the control group. There was no correlation between sacroiliac joint osteophytes and vacuum phenomena in either group. The vacuum phenomenon was detected within the sacral insufficiency fracture in only one patient, in whom the fracture communicated with the adjacent sacroiliac joint. CONCLUSION: Vacuum phenomena are frequently found in the sacroiliac joints of elderly women, and their presence is of no diagnostic significance when demonstrated in patients with sacral insufficiency fractures.
Ten Chinese patients with systemic lupus erythematosus (SLE) and with persistent respiratory symptoms were evaluated with high resolution computed tomography (HRCT), chest radiographs and lung function tests. Fourteen of 15 HRCT scans performed were abnormal. The predominant disease pattern, seen in 60% of patients, was one of chronic interstitial lung disease with honeycombing, architectural distortion, parenchymal bands, pleural irregularity, and a lower zone predominance. Three of 10 patients had histological evidence of either lung fibrosis or interstitial pneumonitis. Airways disease and pleural thickening were seen in 20% and 87% of scans, respectively. Pleural thickening and honeycombing were present in 53% and 20% of chest radiographs, respectively. All concurrent lung function tests were abnormal. Reduced diffusion capacity of carbon monoxide (DLCO/VA) was observed in 60% of lung function tests. There was no correlation between duration of disease and DLCO/VA. However, pathological reduction of DLCO/VA was seen in 71% of patients with honeycombing, and 88% of patients with ground glass opacity. Our study has documented a high incidence of HRCT features of chronic lung destruction and a lower zone predominance in SLE patients with persistent respiratory symptoms.
A 68-year-old woman presented with dyspnoea having undergone a short period of tracheal intubation during an episode of congestive cardiac failure 10 days earlier. On the fourth day of admission, she suddenly developed acute respiratory distress followed by a cardiac arrest and was found to have an enlarging neck mass. Following intubation and resuscitation, computerised tomography was carried out and surgical exploration revealed a retropharyngeal abscess and a pseudo-aneurysm of the common carotid artery resulting from pharyngeal rupture which most probably occurred as a complication of the original tracheal intubation.

The radiological appearances of 11 cystic renal cell carcinomas (RCC) presenting over a 13-year-period are reviewed and correlated with their clinicopathological features. The tumours occurred in seven males and four females with a mean age of 59.1 years (range 27-87 years). Their ultrasound features were categorized according to Bosniak [5]. There were six category 3 or 4 cysts (indeterminate or malignant); three category 2 cysts and two category 1 cysts. Computed tomography scans (CT) demonstrated malignant features in all seven patients scanned. Cyst aspiration, intravenous urograms and arteriograms were generally unhelpful. Histological growth patterns of nine tumours were determined and all were staged by Robson's classification: eight were Stage 1, one was Stage 3 and two were Stage 4. Three of four deaths were related to tumour bulk, and the fourth patient died from an unrelated cause. All seven survivors had Stage 1 disease. No correlation existed between different histological subtypes and tumour aggression. Overall, the behaviour of cystic RCCs was similar to those of solid RCCs apart from the high percentage (72.2%) of Stage 1 disease. We conclude that any deviation from a simple cyst on ultrasound should be assessed by CT and/or surgery. Indeterminate lesions on CT including multilocular lesions should be surgically explored.
A 62 year old Chinese woman presented 25 years after having both breasts augmented with paraffin injections. Development of paraffinomas and multiple episodes of paraffin-related mastitis eventually resulted in bilateral mastectomies. The unusual distribution of migrated calcified paraffinomas in the thoracic wall and its lymphatic system is documented on computed tomography.
The authors report a case of the superior mesenteric artery syndrome presenting acutely with a Boerhaave-type syndrome. Pneumomediastinum was seen on the chest X-ray. There were no known predisposing factors or previous history typical of this syndrome.

The acute presentation of trigeminal neuroma, due to sudden haemorrhage, in a 26-year-old Chinese man, is described. The clinical, CT and MRI features are reviewed together with the other four cases reported in the literature.
Malignant lymphoma of the central nervous system is usually confined within the cranium, with primary involvement of the spinal cord and nerve roots being very rare. The occurrence of primary lymphoma in the cauda equina in a 16-year-old Chinese male is reported. Contrast enhancement during magnetic resonance imaging is useful in accurately demonstrating disease extent prior to surgery.
A 64-year-old woman, being managed for carcinoma of the cervix, was incidentally found to have a solitary liver lesion on computed tomography (CT). The rounded lesion was well-defined and hypodense on unenhanced CT. Dynamic and delayed contrast CT demonstrated the typical enhancing pattern of cavernous haemangioma. The role of imaging in the diagnosis of hepatic haemangiomas is discussed.
Thromboembolism is a well described complication of transfemoral arteriography. Its origin and course is rarely demonstrated. We describe a case of thromboembolism with histopathological correlation demonstrating the origin and course during contrast injection.
It has been suggested that technetium 99m nanocolloid is as effective an inflammatory radiopharmaceutical as indium 111 leucocytes. This study compares the efficacy of 99Tcm nanocolloid and 111In leucocytes in the detection of orthopaedic infection in 19 patients with a high clinical suspicion of infection. The two scintigrams were performed within 24 h of each other. A scintigram was considered positive where there was an increase in tracer uptake at the site of interest. Concordance rate of 73% was achieved. The numbers of false positives with 111In leucocytes and 99Tcm nanocolloid were three and six respectively. The single false negative in both was of a patient with tuberculous spondylodiscitis. Sensitivity was 75% in both. Specificities were 79% and 60% for 111In leucocytes and 99Tcm nanocolloid respectively. Positive Predictive Value was only 33% with 99Tcm nanocolloid and 50% with 111In leucocytes. 99Tcm nanocolloid also proved less reliable in accurately detecting infected prostheses. We conclude that 99Tcm nanocolloid cannot replace 111In leucocytes in the diagnosis of orthopaedic infections.
An acute presentation of the superior mesenteric artery syndrome was diagnosed in a 13-year-old Chinese boy in whom no predisposing cause was found at surgery. Duodenogram combined with dynamic computed tomography, including 2-dimensional multiplanar reconstructions, was useful in confirming the diagnosis prior to definitive surgery.
