A prospective study of immune responsiveness in human melanoma. I. Assessment of initial pretreatment status with stage of disease.

The clinical findings in 262 patients with melanoma were correlated with an assessment of their cellular-mediated immunity by delayed-type hypersensitivity (DTH) skin tests, by lymphocyte blastogenesis, and by leukocyte enumerations. Patients with systemic metastases (stage IV) and those with evidence of tumor burden had fewer positive DTH reactions for recall antigens than did patients with localized disease (stage I), patients with no evidence of tumor burden, and 227 normal controls. However, no differences were found in the magnitude of response among patients with melanoma or when compared with controls. Patients with melanoma had fewer responses to dinitrochlorobenzene (40% than did controls (98%), but there were no differences by stage or tumor burden. Similarly, blastogenesis in the presence of PHA, Con A, and PWM was depressed when compared with controls, but there was no meaningful correlation with the clinical status. Leukocyte, T cell, or B cell counts revealed no alteration from normal for the patients with melanoma. The usefulness of these studies for routine clinical monitoring is questioned.

Thymus-dependent and nonthymus-dependent lymphocytes in patients with sarcomas.

Several measurements indicate that cellular and humoral immunity is depressed in patients with sarcoma; but whether such observations reflect diminished numbers of lymphocytes and their subpopulations is not known. We determined the numbers of peripheral leukocytes, lymphocytes, rosette-forming thymus-dependent cells (T cells), and nonthymus-dependent SmIg-bearing cells in 134 patients with various mesenchymal sarcoma and in 47 normal controls. In all of these determinations, patients and controls gave similar values. Furthermore, values for patients with or without tumor, with or without metastasis, and with bone or soft-tissue sarcomas were similar. All 134 patients were studied prospectively, and the 84 who remained free of disease after 6 months had virtually identical values to the 50 who showed disease progression.

Respirator cartridge service lives for exposure to vinyl chloride.

Respirator cartridge service lives (1 ppm breakthrough) were determined for low level (approximately 10 ppm) exposures to vinyl chloride using simple apparatus and an infrared spectrophotometer. Two classes of organic vapor cartridges were tested; six brands which received approval from MESA-NIOSH before July 15, 1975 ahd two brands which were not approved but were available through local safety supply outlets on that date. Cartidges were tested both as they came from the supplier and after being preconditioned for 6 hours. Effects of insult concentration, preconditioning, and absorbent weight were investigated. No cartridge service life exceeded 1 hour; many did not provide protection for even 30 minutes. As a result, the use of cartridge containing half mask respirators for vinyl chloride exposure should be questioned.

Transfer factor versus combination chemotherapy: a preliminary report of a randomized postsurgical adjuvant treatment study in osteogenic sarcoma.

Twenty-six patients with classic osteosarcoma were randomized to receive either transfer factor or combination chemotherapy. Eight of 14 patients who received transfer factor converted their skin test markers, evidence of activity of the transfer factor. Of these eight patients, all are alive; four are free of disease. Of the 18 patients who received combination chemotherapy, 14 are alive, 12 of whom are free of disease. The immunologic test procedures performed sequentially reveal that transfer factor appears to enhance cell-mediated immunity, but it is evident that in this study, a control (saline) arm in the protocol could not be included. It is of interest that the chemotherapy regimen used does not appear to suppress such activity permanently. The individual test results, however, are not very helpful for predicting response to treatment. The small numbers of patients and the short duration of this study, combined with the exclusion of parosteal osteogenic sarcomas and jaw tumors, do not permit a meaningful comparison with other published studies.

Postpartum and Postabortal Ovarian Vein Thrombophlebitis.

Six cases of ovarian vein thrombophlebitis are reported including one resulting in operative death, a case diagnosed by phlebography, a postabortal case, and a case followed by habitual abortion. An historic review of this disease is presented. It is suggested that the ovarian vein may be the most commonly involved vein in puerperal pelvic thrombophlebitis.

Seasonal periodicity of three species of caryophyllaeid cestodes in the creek chubsucker, Erimyzon oblongus (Mitchill), in North Carolina.

One hundred and seventy-two specimens of Erimyzon oblongus from Lake Raleigh, North Carolina, were examined for caryophyllaeid cestodes from December 1972 to July 1974. Monobothrium ulmeri, Biacetabulum meridianum, and an undescribed species of Penarchigetes were recovered from the "stomach," "pyloric bend," and the first loop of the small intestine of the suckers. Infections of M. ulmeri were limited to an 8-month period and were seasonally periodic in prevalence, mean intensity, maturation, and length distribution. A "temperature dependeant rejection response" is discussed as a possible factor involved in the periodicity of this species. The maturation of M. ulmeri corresponded closely to the host's reproductive cycle, and may have been affected by changes in the host's hormonal levels during the spring. Infections of R. meridianum were present throughout the year, and exhibited a seasonal periodicity in prevalence, maturation, and length distribution. The mean intensity of infection suggests that the establishment of this species is inhibited by heavy burdens of M. ulmeri. Penarchigetes sp. was nonseasonal in prevalence, mean intensity, and maturation. The continuous availability of infective procercoids and the lack of a sucker "rejection response" are discussed as possible explanations for this destribution. Unlike B. meridianum and Penarchigetes sp., the mean intensity of M. ulmeri showed a close relationship with host sex.
