Congenital diverticulum of male urethra.

Congenital diverticulum of the male urethra is an uncommon condition. Urinary tract infection, urethral obstruction, and disturbances in micturition are the most common symptoms. Six cases of congenital diverticulum of the male urethra were studied with regard to diagnosis and treatment. Four patients underwent endoscopic treatment of the diverticulum. The procedure is simple and rapid, with low incidence of complications and high rates of success.

Bacillus Calmette-Guerin immunotherapy of infiltrating bladder cancer.

Oral administration of bacillus Calmette-Guerin was used to treat 10 patients with muscle invasive transitional cell carcinoma of the bladder. The treatment induced tumor regression in 7 patients (70 per cent), including 1 who died without evidence of recurrent tumor. Skin test reactivity was correlated with response to treatment. Only 1 patient had marked skin test reactivity at the time of tumor recurrence. Our data demonstrated that patients with invasive bladder cancer may derive benefit from immunotherapy.

Varicocele: clinical or surgical treatment?

Varicocele, known as the most common cause of male infertility, has usually been treated surgically and, although successful results have been reported, its therapeutic efficiency has been questioned. Sixty-six infertile men with varicocele were under treatment within the period December, 1971-1981. Thirty-eight underwent surgical correction (varicocelectomy) and twenty-eight received only medical treatment. The spermograms of all the patients reported sperm counts, percentage of general motility, percentage of grade III motile sperm (rapid and directional movements) and percentage of oval forms, before and after the treatment. The results showed that there was an improvement in all the spermogram parameters in both groups. Except for one variable all the others failed to show a significant difference by statistical analysis between both treatments, however, the surgical treatment resulted in higher values when compared to the medical one. Pregnancy rates, while higher for the surgical group, was not statistically different in both groups. The medical treatment showed to be effective in elective cases. Nevertheless, the surgical treatment remains the better choice in the treatment of subfertility associated to varicocele.

The value of thermography and of the Doppler ultrasound in varicocele diagnosis.

The role of varicocele in male infertility is known. Among infertile patients that look for treatment, approximately 30% suffer from varicocele. The demonstration of the existence of subclinical varicoceles, that is, those that are not detectable through clinical examination, that are responsible for alterations in the spermogram, lead to the development of diagnostic methods of greater sensitivity. In the present study, the results obtained in eighty-eight (88) patients were analyzed with a view to confirming and comparing the sensitivity and efficiency of the Doppler ultrasound and of thermography in the detection of subclinical varicoceles. Thermography revealed itself as more sensitive, allowing the diagnosis of 40.9% of subclinical varicoceles.

A comparison of treatment methods for the prophylaxis of recurrent superficial bladder tumors.

We assigned 55 patients with superficial transitional cell carcinoma of the bladder by rotation to 1 of 3 treatment categories. Group 1 patients were treated by transurethral resection alone, group 2 by transurethral resection followed by topical thiotepa and group 3 by transurethral resection followed by orally administered bacillus Calmette-Guerin. Recurrent bladder tumors developed subsequently in 80 per cent of the patients in group 1, 42.9 per cent in group 2 and 6.2 per cent in group 3.

A comparative study of electrosurgical vs. cold urethrotomy in the treatment of urethral strictures.

Thirty-one patients with urethral stricture were subjected to internal urethrotomy under visual control. The patients were divided into two groups according to the surgical procedure: electrosurgical or cold resection. A modification of the Sachse knife was introduced in order to permit its use with the American Cystoscope Makers Inc. instruments. The success rate with cold resection was 81.81% and with the electrosurgical procedure 40%. Internal urethrotomy with cold resection is a simple and harmless operation, being a first choice in the treatment of urethral stricture.

Relation between the vaginal introital and perianal flora in recurrent cystitis in women.

15 patients with urinary infection were selected and analyzed in relation to infection in the vaginal and perianal areas. After treatment, 12 patients with urinary infection were cured, but bacterial organisms persisted in their vaginal and perianal areas. In spite of the persistence of the vaginal and perianal infection, those patients remained free of urinary infection. We can conclude that other factors, besides the persistency of bacterial organisms in the vaginal introitus and perianal regions, may be important in the development of ascending urinary infection.

Evaluation of the impairment of spermatogenesis in varicocele through semen analysis and testicular biopsy.

Twenty-four patients with varicocele and 26 normal men were analysed according to the seminal ejaculate and testicular biopsy. Patients were divided into two groups based on the criteria of sperm normality and abnormality, respectively. Hypospermatogenesis with both normal and abnormal seminal fluid occurred in the varicocele patients, but no statistical differences could be established when compared with each other. We concluded that testicular biopsy is not useful in the evaluation of patients with varicocele.

Varicocele: relation between anoxia and hypospermatogenesis.

Three groups of patients were analysed: varicocele patients with abnormal seminal fluid (17 cases), variococele and normal sperm (seven cases) and normal males (17 cases). Blood gas was secured in the internal spermatic vein, perpheric vein and femural artery, in an attempt to evaluate the possibility of altered gas content as a cause of spermatogenic depression. No statistical differences were found when we compared the three groups. We concluded that anoxia was not the cause of hypospermatogenesis in varicocele patients.

Varicocele: medical and surgical treatment.

Varicocele is the most frequent finding in male infertility. Thirty percent of infertile male patients requesting treatment present with varicocele. Between December 1971 and December 1984, 151 infertile patients presenting varicocele were treated in this study: 28 underwent medical treatment, 38 surgical treatment, and 23 both medical and surgical treatment. The spermograms of all the patients were studied before and after treatment. The following variables were considered: volume, sperm count, percentages of general motility and of grade III sperm, and of oval forms. There was not a significant statistical difference in the analysis of the spermogram parameters before and after treatment in the three groups. The pregnancy rate for the medical-surgical group was 60.9% and for the surgical group 47.4%--not a statistically significant difference. The purely medically treated group had a pregnancy rate of 25%, which is significantly lower than the other groups.
