Marrow autotransplantation accelerates haematological recovery in patients with malignant melanoma treated with high-dose melphalan.

In a Phase I study, melphalan 140 mg/m2 was administered to 8 patients with disseminated malignant melanoma. Marrow was removed from the patients immediately before melphalan administration and returned i.v. 8 h later. Studies on marrow culture and melphalan pharmacokinetics predicted that this was a safe time to administer non-cryopreserved marrow. Four patients received lower doses of i.v. melphalan without autologous marrow. In the group receiving autologous marrow the time for recovery of peripheral-blood granulocytes to 800/mm2 or greater was significantly less (P = 0.01) than in those not receiving marrow. In 7 patients the tumour showed evidence of response to the drug and there was 1 complete remission. This treatment deserves investigation in patients with tumours more sensitive to drugs than melanoma.

Naphthylamidase activity of Leptospira.

Extracts of 18 serotypes of the genus Leptospira were found to possess naphthylamidase activity, and differences in the pathogenic and saprophytic strains were noted. The former exhibited a preference for the leucyl naphthylamide substrate, whereas the latter demonstrated greater hydrolysis of alanyl naphthylamide. With the leucyl naphthylamide as substrate, pathogenic strains showed 10 to 20 times higher naphthylamidase activity than saprophytic strains. Optimal temperature and pH for enzymatic hydrolysis also differed between pathogenic and saprophytic strains. Maximal enzymatic activities for pathogenic and saprophytic naphthylamidases were 41 and 37 C, respectively, at pH 8.0 to 8.5. The pH and temperature optima suggested that the leptospiral enzyme activity was not leucine aminopeptidase.

Biosynthesis of the bufadienolide ring of scillirosid in Scilla maritima.

Administration of sodium [1,2-14C2]oxaloacetate and sodium [1-14C]acetate to Scilla maritima plants yielded labelled scillirosid. A systematic degradation of scillirosid indicated that the alpha-pyrone ring of the bufadienolide is formed by the condensation of a pregnane derivative (derived from mevalonic acid via squalene) and one molecule of oxaloacetic acid.

The future of materials management--the view from the for-profit sector. Interview by David Cassak.

David Cassak met Gene Burton, Vice President of Materials Management for HCA, the country's largest for-profit hospital company, at the recent Federation of American Hospitals meeting to talk with him about some of the things that he and his organization are trying to do in purchasing and materials management. With over 400 hospitals, either owned or managed, and a host of alternate site delivery programs, such as home healthcare and HMO and PPO plans, HCA is clearly a driving force for innovation in today's healthcare environment.
