[Serum concentrations of calcitonin and parathormone in the cord blood of premature infants].

Determinations of serum calcium (Ca), calcitonin (CT) and parathyroid hormone (PTH) were carried out in mixed cord blood of 23 preterm infants. Gestational age ranged between 25 and 37 weeks. 17 of theme were vaginally delivered while 6 were delivered by emergency Caesarean section. 4 neonates died because of respiratory distress syndrome. The serum was stored at -30 degrees C until the determinations. Serum Ca levels were determined by spectrophotometry while CT and PTH levels by RIA (Immuno Nuclear Co). In cord serum the mean (M +/- SE) Ca,CT and PTH concentrations of all neonates examined were respectively: 9,9 +/- 0,6 mg/dl; 176 +/- 44 pg/ml and 1100 +/- 446 pg/ml. Serum values of CT and PTH in preterm newborns delivered by emergency Caesarean section were significantly higher than in those neonates vaginally delivered (CT: 302 +/- 115 vs 94 +/- 9 pg/ml; p less than 0.005) (PTH:2655 +/- 1857 vs 466 +/- 59 pg/ml; p less than 0.05). No differences were observed between serum CT and PTH levels in preterm neonates of different gestational age. Both CT and PTH serum concentrations were higher in neonates who died. In conclusion, the preterm neonate is able to secrete both peptides and to maintain Ca homeostasis; the mode of delivery likely affects the CT and PTH secretion; unexplainable high CT and PTH serum levels were detected in poor outcome preterm infants.

Calcitonin and parathyroid hormone in newborn infants with fracture of the clavicle.

Determinations of serum calcium (Ca), phosphorus (P), calcitonin (CT), and parathyroid hormone (PTH) were carried out in 36 full-term newborn infants with fracture of the clavicle (CF) and in 46 normal neonates (N). At the 6th hour of life the CF neonates demonstrated lower serum Ca and higher serum CT in comparison with normal infants. In the hours following, no significant differences between the two groups for the Ca levels were found, whereas serum CT remained significantly higher in the CF newborns at the 24th, 48th, and 72nd hour of life. Significant differences between normal and CF infants in the PTH serum levels were detected only at the 48th hour, when PTH was lower in the CF newborns. The results of this investigation indicate that the fracture of the clavicle is a significant and peculiar factor in stimulating CT secretion. Serum Ca level appeared to be controlled by CT rather than auto-regulating the secretion of the hormone.

[Serum levels of parathormone, calcium, magnesium and phosphorus in the perinatal period].

Two groups of newborns on different diets were studied: group A was given artificial milk, while group B was breast fed. Blood samples from the cord artery and vein were taken at birth and further blood samples after 1, 6, 12, 24, 48, 72, 96 and 168 hours. No baby underwent more than three blood samples. Calcium (Ca), magnesium (Mg), phosphorus (P) and parathormone (PTH) were measured in all samples. There was a rapid increase in PTH levels in all subjects in the first hours after birth. At 48th the babies in group A had significantly higher serum levels of P than the group B subjects, and their serum Ca and Mg levels were significantly lower. At 72th PTH serum levels were significantly higher in group A, while Ca and Mg levels were significantly higher in group B. In conclusion, the subjects demonstrated a normal secretion of PTH at birth, and the type of diet notably influenced the homeostasis of calcium in the perinatal period.

[Kinetics of erythrocyte populations in newborn infants with hyperbilirubinemia].

Determinations of erythrocyte glucose-6-phosphate dehydrogenase, pyruvate kinase and glucose-phosphate isomerase activities have been carried out in jaundiced and non-jaundiced newborn infants at birth and after 5 days of life. Only hyperbilirubinemias of unknown etiology have been considered. Previous investigations have demonstrated that erythrocyte age dependent enzyme activities decrease in the healthy newborn during the first week of life. Following the results of the present paper, jaundiced newborns do not show such a decrease, since a statistically significant difference between paired data of glucose-6-phosphate dehydrogenase and pyruvate kinase in jaundiced and non-jaundiced newborns have been found. It is likely that the lack of decrease of enzyme activities in the jaundiced infants depend upon the loss of oldest red cells with low glucose-6-phosphate dehydrogenase and pyruvate kinase. Our observation suggests the occurrence of a slight hemolysis in newborn infants with hyperbilirubinemia of unknown etiology.

[Methemoglobinemia and acidosis during the first week of life].

Recently attention has been called on the possible role of acidosis in the increased methemoglobin formation in the erythrocyte of newborn infant. In the present paper the relations between acidosis and methemoglobin content in the red cells of newborns has been investigated. No significant differences between the percent of methemoglobin in the normal newborns and percent of methemoglobin in the newborns with acidosis has been found. In addition, no correlations between the base excess and percent of methemoglobin has been observed. On the contrary, two newborns with low glucose-6-phosphate dehydrogenase activity demonstrated a significantly increased methemoglobin content in their red cells. The results of our study do not confirm a key role of acidosis in the mechanism of methemoglobin formation in the neonate. It is likely than impairment of red cell metabolism should be the main factor in the formation of methemoglobin in the first days of life.

Moderately increased hemolysis in newborn infants with hyperbilirubinemia of unknown etiology.

Serial determinations of carboxyhemoglobin (COHb) levels were performed in full-term newborn infants during the first few days of life and their mothers. A close correlation was found between COHb in the mother and that determined in the cord blood. The correlation between COHb in the mother at delivery and that found in the neonate disappeared after 48 h of life. The determination of COHb after this period demonstrated significantly higher values of COHb levels in jaundiced neonates compared with normal infants. Determinations of erythrocyte age-dependent enzyme activities carried out at birth and after 5 days of life did not demonstrate any significant difference between the mean values in jaundiced and normal infants. However, the normal infants demonstrated a decrease of glucose-6-phosphate dehydrogenase and pyruvate-kinase activities from birth to the 5th day which is not appreciable in neonates with hyperbilirubinemia of unknown etiology. The results are discussed in relation to the role of hemolysis in neonatal hyperbilirubinemia.

[The effects of diet on the secretion of calcitonin in the newborn infant].

Determinations of serum calcium (Ca), phosphorus (P) and calcitonin (CT) were carried out in two groups of full term newborn infants: 80 on bottle feeding (group A) and 80 breast feeding (group B). Samples of blood were drawn at 1, 6, 12, 24, 48, 72, 96 and 168 hours of life. Each newborn was tested no more than twice. No differences between the two groups could be detected before the 72th hour. At that time, the Ca of group A was significantly lower than Ca of group B while the CT and P were significantly higher in the same group A. So that, the P serum level appears to be one of the most important factor regulating the homeostasis of Ca after the first 72 hours of life.

[Functional aspects of neutrophils in full-term infants].

The measure of superoxide anion (.O2-) generation was carried out in the neutrophils (PMNs) of newborns. Comparison between .O2- generation in the resting state and after stimulation with zymosan was considered. PMNs were incubated in their own plasma and in plasma from healthy adult subjects. The results demonstrate a decrease of .O2- generation (in comparison with adult PMNs) in the PMNs from newborn when they were incubated in their own plasma while the .O2- generation reached the values equal to adult's PMNs when the cells of newborns were incubated in the adult's plasma. However PMNs from some newborns demonstrated a low .O2- generation even though they were incubated in adult's plasma. The data are suggestive of a predominant, but not always exclusive, responsibility of plasma factors in the decrease .O2- generation by PMNs of full term babies.

[Separation of erythrocytes from full-term infants into different age populations].

Separation of erythrocytes into fractions of different age was carried out by the method of discontinuous gradient. Five fraction of various specific density were prepared and in each fraction age dependent enzyme activities PK and G-6-PD were assayed. The erythrocytes from cord blood and from newborn 4-5 days old were studied. The results demonstrate the correlation between density and erythrocyte age in both cord blood and blood from newborns. However, 4-5 days old demonstrate a shift of various populations toward fractions of higher density.

[Serum calcitonin in full-term infants].

Determination of calcium (Ca) calcitonin (CT) secretion in mother during labour and their newborns during the first hours of life was carried out. Mean serum levels of Ca decreased after the birth. Mean serum level of Ct was higher in the cord blood comparing with mothers blood. These levels increased after the 6th hour and reached the peak at the 12nd hour. After the 24th hour the CT levels decreased significantly although at the 48th hour they remained higher than those in the cord blood.

[Variations in erythrocyte enzymatic activity in the first days of life].

The erythrocyte activities of glucose 6-phosphate dehydrogenase, pyruvate kinase and glucose phosphate isomerase decrease during the first days of life. Furthermore the level of these enzymatic activities is higher in newborn infants with higher percentage of carboxyhemoglobin. It is likely that a premature and rapid fall of reticulocytes causes a decrease in enzymatic activities sensitive to aging and that the higher hemolysis can represent a disappearance of oldest erythrocytes which present lower enzymatic activities. These data therefore confirm the block in erythropoiesis in the first hours of life.
