The laminin receptor and basement membrane dissolution: role in tumour metastasis.

Tumour invasion and metastasis is a complex process involving multiple interactions of tumour cells with host cellular and extracellular elements. Metastasizing tumour cells traverse basement membranes at many stages in the metastatic cascade. Immunohistology studies demonstrate that the basement membranes are defective in all human malignant epithelial neoplasms studied to date. The basement membrane is absent in regions of microinvasion and adjacent to actively invading tumour cells. In contrast, benign neoplasms retain a continuous basement membrane. This distinction may have diagnostic value in surgical pathology. Tumour cells are hypothesized to traverse basement membranes by a three-step process: attachment, local degradation of the basement membrane by type IV collagenase and other proteases, and locomotion. The first step may be mediated in part by specific cell surface receptors which bind to laminin in the basement membrane. The laminin receptor has been purified from a series of different human carcinomas. The receptor has an Mr of 67 000 and a binding coefficient of 2 nM. The content of unoccupied receptors is markedly augmented in invasive human breast cancer compared to benign controls.

Multicentric Kaposi's sarcoma of the conjunctiva in a male homosexual with the acquired immunodeficiency syndrome.

A 38-year-old male homosexual with the new Acquired Immunodeficiency Syndrome (AIDS) had biopsy proven Kaposi's sarcoma of the right palpebral conjunctiva, extraocular mucocutaneous areas, and lymph nodes. Histologically, the palpebral tumor was characterized by atypical spindle cell proliferation and multiple slit-like vessels. Staining for Factor VIII-related antigen was positive in the cytoplasm of some tumor cells. Electron microscopy disclosed Weibel-Palade bodies in cells lining scattered slit-like vascular channels. The palpebral tumor mass was relatively nonadherent to its surrounding tissues and thus its simple excision was noteworthy; despite the prominent vascularity of the tumor, minimal bleeding was associated with its surgical resection. Any patient with AIDS should have all mucosal surfaces routinely examined for the presence of Kaposi's sarcoma lesions.

Amino acid content of guinea pig perilymph collected under conditions of quiet or sound stimulation.

With the cochlear aqueducts blocked, guinea pig cochleas were perfused with artificial perilymph. The collected perfusates were treated with 5-(dimethylamino)-1-naphthalenesulfonyl chloride (dansyl-Cl). The derivatized constituents were analyzed by high- performance liquid chromatography using fluorometric detection. The concentration of none of the compounds measured, including the putative transmitters glutamate and aspartate, was found to increase in the perfusate in response to sound stimulation. These results do not support the suggestion that one of these compounds might be primary afferent transmitter of audition.

Intrafollicular pressure within preovulatory follicles of the pig.

Antral pressure was measured within the follicles of unstimulated ovaries in prepubertal pigs and following an ovulatory stimulus with exogenous gonadotropins. No increase in intrafollicular pressure (IFP) was observed as the time of ovulation approached. A wide range of IFP was noted within follicles of the unstimulated ovary. In many follicles IFP was greater than 30 cm H2O, suggesting that the antral fluid was not in hydrostatic equilibrium with the surrounding thecal capillaries. IFP of unstimulated follicles could be increased to more than 400 cm H2O by antral injection of mineral oil without follicular rupture--a demonstration of the need for stigma formation in the release of the ovum from the follicle. Stimulated follicles were found to be more distensible than unstimulated follicles. The follicles also fell into two groups--those in which sustained versus transient elevation in IFP occurred following oil injection. It is postulated that the follicle wall develops the ability to undergo stress relaxation during follicular maturation and that this process plays a role in regulating IFP.

A simple, sensitive method for the quantitation of kanamycin in tissue and biological fluids.

A method is described for the quantitative determination of kanamycin in blood, urine, cochlear perilymph and kidney. The method is simple, involving only protein precipitation, chromatographic separation and fluorometric determination following reaction with fluorescamine. The method is sensitive, determining as little as 80--100 ng/ml of kanamycin. The method is reliable, recovering better than 90% of the kanamycin added to the fluids and tissues mentioned. It is also less cumbersome and less expensive than other methods currently used.

Metabolic requirements for entry of DNA in Branhamella catarrhalis.

Although requirements for transformation in Branhamella catarrhalis are quite complex, DNA synthesis does not appear to be one of these needs, as indicated by the inability of nalidixic acid to interefere with transformation. Exogenous sources of energy, such as cAMP and cGMP also failed to enhance frequency, suggesting cells may actively engage in energy production to achieve uptake of DNA, or lack a transport mechanism for these compounds.

Benefits to Down's syndrome children through training their mothers.

This study investigated the hypothesis that training of mothers with Down's syndrome children would be beneficial both to the child and parents. The mothers were taught behaviour modification techniques based on learning theory and were given group discussions on dealing with their family or personal problems. The subjects were 16 mothers with a Down's syndrome child, divided into two groups on the basis of their child's sex and chronological and mental ages. The Griffiths Scale was used for assessment. The mothers in the treatment group received 12 sessions of training and group counseling over a 6-month period, whereas the control mothers received no additional attention except the usual routine from the general practitioner and health visitor. The result show clear gains to both the child and mother in the treatment group. The child improved, especially in language development as well as in the other areas, and the mother-gained more confidence and competence in her daily management of the child.

Radiation therapy for nodal disease in malignant melanoma.

Radiation therapy has been widely used for palliative management of inoperable metastatic malignant melanoma. For patients with nodal disease, response rates of approximately 70% have been reported. There are limited data concerning the role of adjuvant irradiation following therapeutic lymph node dissection. In this review, 57 patients with isolated resectable and nonresectable nodal disease have been treated with radiation. The overall response rate is 84% for bulky disease. Large fractions are beneficial. The median disease-free survivals were 11 months after adjuvant treatment and 7 months for those with inoperable disease. The median overall survivals were 20 months and 18 months, respectively. Local control in long-term survivors was excellent. Sixty-five percent of patients developed distant metastases. There is a need for additional studies with the use of adjuvant radiation therapy following lymph node dissection.

A home advisory service for pre-school children with developmental delays.

This paper describes the development of a home advisory service for families of children with developmental difficulties. It demonstrates the transition from a research phase to routine service provision, and indicates that the expansion in numbers of families receiving the service has been achieved whilst maintaining or improving the actual quality of the service. Information is provided on the placement of children who have left the service, and on the plans for future research within the service.

Preventive health care for preschool children or health surveillance.

The debate about provision of health surveillance programmes for preschool children is described. Improvement in the health of the child population has caused a need for its continuance in its present form to be questioned. The aspects of child health and development that still cause concern and the current approaches to them are reviewed. The problem of disadvantaged children and their health needs are discussed. Changes in social policy are required as well as continued initiatives in the provision of preventive health care for vulnerable families.
