The repair of large incisional hernias.

An old technique for the repair of incisional hernias, but with the use of Marlex mesh instead of fascia lata, was used in eight cases, with good results for the limited time the patients have been observed. The operation involves the use of a multifingered sheet of Marlex mesh, a material with characteristics that make it ideal for repair of large hernias.

An immunohistochemical and clinicopathological study of granulocytic sarcoma ('chloroma').

A small series of granulocytic sarcomas (GS) or 'chloromas' has been studied by means of conventional histology and immunohistochemistry. The latter was found to be most useful in the diagnosis and characterization of these neoplasms, which are rare in Great Britain. Polytypic antisera to leukocyte elastase and cathepsin G have proved to be most useful, giving intense staining in all cases. These two markers are more specific than muramidase (lysozyme) and alpha 1-antitrypsin, since, unlike the latter two, they do not stain histiocytic cells. The results of staining with two monoclonal antisera, AGF 4.48 and AGF 4.36, are also described. In addition, the clinicopathological details of the seven cases are summarized and the literature is briefly surveyed.

Persistent growth of BALB/C mouse plasmacytoma and human myeloma cell lines in the presence of phorbol myristate acetate is associated with continued expression of Lap18 (stathmin).

Lap18 is a highly conserved cytosolic protein that is expressed in dividing cells. Data from a number of studies show that a range of cell lines and mitogen-stimulated normal cells cultured in PMA phosphorylate and subsequently down-regulate Lap18. This has been found to be associated with growth arrest, although it is not clear that these events are causally related. In the present study we confirm that the HL60 promyelocytic leukemia and K562 erythroleukemia cell lines, when cultured with PMA, behave in this manner. This was not the case for any of five mouse plasmacytoma cell lines and six lines derived from patients with multiple myeloma or plasma cell leukemia. All of these lines contain Lap18, although the level of this protein in the mouse but not the human plasmacytoma cell-line cells is relatively low. All the neoplastic plasma cell-line cells phosphorylate Lap18 on culture with PMA, but this does not induce growth arrest nor result in down-regulation of Lap18 expression. Further experiments are required to test whether there is a mechanistic relationship between the continued growth of plasmacytoma cell lines and their failure to down-regulate Lap18 on culture in PMA.

Understanding the effects of a myocardial infarction on sexual functioning: a basis for sexual counseling.

Sexual counseling is an integral component of the rehabilitative process for clients who have experienced a myocardial infarction (MI). This article describes myths and fears concerning the dangers of resuming sexual intercourse after a heart attack, positions for intercourse, and the effects of intercourse on arrhythmias, angina, and blood pressure. The purpose is to provide the results of empirical research to dispel myths and fears and to consider the causes of sexual dysfunction, the changes MI brings about in sexual activity for men and women, and the effects of age, exercise, and antihypertensive drugs on the resumption of sexual activity. Guidelines are provided for conducting a sexual/cardiac history as part of the counseling model with specific information for the post-MI client.

Positron emission tomography for tumour assessment.

As positron emission tomography (PET) is outside of the main theme of this conference, it is necessary to outline what it offers as a method for assessing tissue function in vivo. This is followed by illustrations of how PET has and is being used to study the physiology and biochemistry of tumours and pharmacokinetics and pharmacology of anticancer agents. Strategies are offered as to how PET may be used for the development of new chemotherapeutic agents.

Distribution of fucosylated N-acetyl lactosamine carbohydrate determinants during embryogenesis of the kidney in man.

The monoclonal antibodies AGF4.48 and AGF4.36 raised against the promyeloid cell line HL60 recognise a fucosylated N-acetyl lactosamine determinant. This oligosaccharide sequence has been shown to be present on a variety of tissues at different developmental stages. Using the immunoperoxidase technique and the AGF4.48 and AGF4.36 antibodies on formalin-fixed, paraffin-embedded tissues, the distribution of the corresponding antigenic determinants during human renal embryogenesis has been studied. Both antibodies bind to the surface of the cells of the ampullae of the ingrowing ureteric bud branches, but not to the remainder of the ureteric bud. Reactivity at this site persists until after fusion of the ureteric bud with the S-shaped tubule, but is then lost. The determinants are also found on different segments of the proximal convoluted tubule in the foetal and adult kidney. The determinants are thus found on the cells responsible for induction of renal tubulogenesis, and separately at specific stages and functionally distinct sites on the developing tubule.
