[Various rib anomalies].

Three patients are presented who were suffering from different anomalies of the ribs: There was one each plus and minus variant and a female patient with Gorlin-Goltz syndrome (basal cell nevus syndrome) demonstrating several changes in the ribs as an expression of a genetically determined segmentation disturbance. These presentations are followed by a detailed discussion on the types and causes of such anomalies.

[Prenatal sonography and computed tomography of fetal cerebral malformations].

In three pregnant women, sonography and amniocentesis suggested cranial abnormalities of the foetuses. In view of the far-reaching consequences of such a diagnosis, CT was carried out to confirm the diagnosis. It was possible to show the intra-uterine abnormalities and the type of malformation in considerable detail. In one case an encephalocele was demonstrated, in the two others, an anencephalic foetus was shown.

Contrast-enhanced, K-space-centered, breath-hold MR angiography of the renal arteries and the abdominal aorta.

The purpose of this study was to evaluate the capability of contrast-enhanced breath-hold fast imaging with steady-state precession (FISP) three-dimensional MR angiography (MRA) to detect stenotic lesions of the abdominal aorta, the renal arteries, and the iliac arteries by using a K-space-centered 20-ml gadolinium-diethylene pentaacetic acid (Gd-DTPA) bolus. Fifty patients were studied before conventional x-ray angiography. Contrast-enhanced breath-hold FISP three-dimensional MRA was applied in the coronal view, centered at the renal arteries. Twenty ml of Gd-DTPA was used in all subjects. A test bolus was applied to determine the injection time for the K-space-centered bolus injection. Of 300 segments, 284 segments were classified correctly, 11 were overestimated, and five were underestimated. Sensitivity was 98%, specificity was 96%, positive predictive value was 96%, negative predictive value was 98%, and accuracy was 97%. Of the 50 patients studied, 43 were staged correctly. No venous overlay was seen in 31 patients; partial overlay was seen in 16 patients, and venous structure overlay obscuring arterial anatomy was found in two patients. Six of nine accessory renal arteries could be identified by MRA. Intraobserver variability was .94. This study has shown the ability of contrast-enhanced breath-hold FISP three-dimensional MRA to detect and grade vascular lesions in the abdominal aorta and the renal arteries. The method may serve as a screening tool in the future.

A catalogue of isomerization transformations of fullerene polyhedra.

Representation of isomerization and carbon insertion or extrusion mechanisms as patch replacements on a fullerene surface allows construction of a catalogue of topologically distinct local transformations of fullerenes, classified by patch boundary and pentagon content. All isomerization patches and isomerization pairs containing up to five pentagons and with an upper limit for the boundary length depending on the number of pentagons are listed. Several infinite series of transformations are identified.

A catalogue of growth transformations of fullerene polyhedra.

Carbon insertion or extrusion mechanisms transforming one fullerene to another are presented as patch replacements on the fullerene surface. A systematic catalogue is constructed for the topologically distinct local insertion/extrusion transformations of fullerenes, classified by patch boundary and pentagon content. All pairs of patches with the same boundary but different numbers of atoms, i.e., growth patches, containing up to five pentagons, with an upper limit for the boundary length that depends on the number of pentagons, are listed. New transformations and infinite series of transformations are identified.

Lower extremity occlusive disease: diagnostic imaging with a combination of cardiac-gated 2D phase-contrast and cardiac-gated 2D time-of-flight MRA.

PURPOSE: The goal of this work was to test the ability of a combination of 2D phase-contrast MR angiography (2D-PC-MRA) and triggered 2D time-of-flight MRA (2D-TOF-MRA) in comparison to intraarterial digital subtraction angiography (DSA) to correctly diagnose the location and shape of occlusive lesions in the iliac and femoral arteries and to determine whether 2D-TOF-MRA is helpful to clarify questionable lesions demonstrated by 2D-PC-MRA. METHODS: In 50 patients with claudication, 2D-PC-MRA was performed in three consecutive coronal positions from the aortic bifurcation to below the trifurcation. Axial 2D-TOF-MRA was performed additionally at the site of detected lesions of >50% and lesions in doubt to obtain more precise information about the stenosis. Lesions were classified as follows: low grade occlusion, <50%; high grade occlusion, >50%. MRA was performed within 24 h of a DSA examination. RESULTS: In all patients, the arterial tree from the aortic bifurcation to the trifurcation could be visualized. One hundred twelve lesions were detected by MRA. Sensitivity was 96% and specificity was 92%. Sixty-two lesions were classified as high grade occlusion and reevaluated. In this category, sensitivity was 100% and specificity was 96%. CONCLUSION: The combination of 2D-PC-MRA with triggered 2D-TOF-MRA detects stenotic lesions in the lower extremity arterial system with high sensitivity and specificity.

Effect of a patellar realignment brace on patients with patellar subluxation and dislocation. Evaluation with kinematic magnetic resonance imaging.

The object of this study was to evaluate the effect of a patellar realignment brace on patients with patellar subluxation or dislocation. Twenty-one patients (24 patellofemoral joints) with clinical evidence of patellar subluxation (N = 16) or dislocation (N = 5) were examined with the joint inside a positioning device to allow active-motion, kinematic magnetic resonance imaging. To analyze the patellar tracking pattern, the same imaging parameters (patellar tilt angle, bisect offset, and lateral patellar displacement) and section locations were used before and after application of a patellar realignment brace. No statistically significant differences were found in any of the three parameters for the patellofemoral relationships before or after wearing the patellar brace. The results indicated no stabilizing effect of the tested brace in patients with patellar subluxation or dislocation during active joint motion.

[Oligoclonal bands of four IgG subfractions and IgM and IgA in the serum and cerebrospinal fluid of friends with multiple sclerosis].

Using the methods of isoelectric focusing and immunoblotting the CSF and serum immunoglobulins were analysed in 47 patients with multiple sclerosis and 57 patients with other neurological diseases. The oligoclonal bands were found in the IgG fraction in 42 multiple sclerosis (ms) patients and 13 controls. The percent proportion of oligoclonal bands in various IgG subfractions was similar in both groups and was: 66% and 61% respectively for IgG1, 22% and 22% for IgG3, 12% and 17% for IgG2. In no case was IgG4 found. Additionally sporadic oligoclonality of IgA and IgM in CSF was found. Positive results were obtained in 2 and 2 cases with ms, and in one control case (only IgA), among controls the oligoclonal bands were found most frequently in neuroinfections (38.5%) and polyneuropathy (20.5%).
