Six samples of ventricular cerebral spinal fluid from an external ventricular drain, and peripheral venous blood were taken during the course of parenteral diazepam administration in man. The diazepam level was measured by gas phase chromatography. The levels of diazepam in the CSF were low compared with the plasms concentrations and around the same level as the free circulating fraction. The drug appears in the CSF relatively late after intravenous administration. It would appear that the passage of the drug from the blood into the CSF is slow and not very marked. Therefore it is not the main cause for the pharmocodynamic effects which are the result of the drug passing the blood barrier in the more restricted sense.

An overdosage of alfathesin was accidentally administered to a 4 kg infant with multiple congenital anomalies during an operation for craniosynostosis. The overdose was a consequence of a mistake in adjusting the rate of administration of a constant flow infusion pump. The total dosage corresponded to eight times the necessary dose administered over a period of 30 minutes. The only notable effect was a delayed awakening, which necessitated assisted ventilation. The high therapeutic index of alfathesin is responsible for the absence of serious consequences.
On the basis of the analysis of fifteen cases of children weighing less than ten kg, in a department of neurosurgery, the authors describe the use of Alfatésine by infusion in association with a morphinomimetic, a technique compatible with the existence of raised intracranial pressure. In view of the weight of the children, availability of a venous line represents a handicap and a danger to such an extent that the authors suggest limited assistance in the form of sedation by a halogenated agent, with the application of strict rules concerning dose and surveillance, bearing in mind its effects on intracranial pressure. The quality of anesthesia and of recovery is emphasised and the doses described. The effects of Alfatesine and of Fentanyl on intracranial pressure, cerebral metabolism and blood flow are reviewed. These favourable effects justify the use of Alfatesine in this context. One case of obvious overdose, responsible for delayed recovery, is discussed.

A 17 years old female was first seen in a neurology department in December 75. She was running a low grade fever; her general status was not good. At the physical examination she presented:--an amyotrophy of both arms, mainly proximal,--a thermo analgesic dissociation which was complete from C1 to C5 and incomplete from C5 to D4, and--a spastic quadriparesis (with a posterior spinal syndrome). The first physical disabilities appeared 15 months before. The myelography revealed an enlarged spinal cord in the upper part of the cervical region and the contrast media could not go through the foramen magnum. At surgery, a solid mass, localized on the right side, inside the medulla oblongata and the spinal cordal, with a cyst at each extremity, reaching C4 in the lower part and the recessus lateralis in the upper part. During surgery the patient was breathing spontaneously. After the removal of the mass, she presented an apnea. The quadriplegia following surgery was very short. She recovered rapidly a normal motor activity in both legs and in the right arm. Respiratory paralysis remained complete for 2 months. The right diaphragm recovered completely in 4 months, the left one only in part. 4 months after surgery, she went back home. One year later for security reason she still has a tracheotomy. The distal paralysis of the left superior arm is still present.
The electrocardiogram (E.C.G.) was recorded in 100 cases of meningeal hemorrhages. The most frequent abnormalies encountered were bradycardia (32%) and inversion of the T wave (34%). Rhythm and conduction problems were less frequently found. Bradycardia was frequently and T wave inversion rarely found in cases without disturbances of consciousness and those with favourable prognosis. The contrary was found in comatous patients and cases with lethal outcome. The existance of an aneurysm and the location of it did not influence the type of electrocardiographic abnormalies. The influence of the parasympathetic and especially the sympathetic nervous systems is emphasized.

The authors reported 92 observations of anesthesia for gaseous encephalotomography interest the adult. The contrast produce is air. 49 under diazanalgesia and myoresolution. Diazepam, +Fentanyl, pancuronium bromide N2O to 60 p. 100. 25 under diazanalgesia and myoresolution. Diazepam, +Fentanyl, succinylcholine, N2O to 60 p. 100. 18 under narco-analgesia and myoresolution. +Fentyl, pancuronium bromide N2O to 60 p. 100. The conditions of the study are described in the first part. The results and their analysis permit the appreciation of: - the patient confort, the quality of the examination; -the respect of the hemodynamics for this examination, reputed to be "difficult"; -the immediatly noticeable diminution of side effects; -the absence of side effects; -the justification and interesting of the control ventilation; -the quality of waking up. In the conclusion the authors underline the interest of their different techniques and the possibility of using them in operations in sitting position in neurosurgery, and all important chirurgical intervention.
