[A controlled trial of a new material for coating urinary catheters].

Hydrogel is a highly biocompatible material which is used for coating Foley catheters. It has, in vitro and in vivo, demonstrated the lack of irritation to the surrounding urothelium. This controlled, randomized trial study the effectiveness of hydrogel-coated catheter (H catheter) compared with silicone-coated catheter (S catheter) in the prevention of nosocomial urinary tract infections (N.U.T.I.) for intensive care patients. To be included in this trial, subjects should have no urinary tract infection and no Foley catheters. Preliminary results concern 266 subjects (129 with H catheter, 137 with S catheter) Sex ratio M:F was 0.95; mean age was 50.7 years. The main criteria of judgement was the cumulative incidence of N.U.T.I. There was no significant difference between the two groups: 15.8% for H catheter versus 12.9% for S catheter. In the two groups mean duration of catheterization was longer for patients with N.U.T.I. Other evaluations seem to be necessary in order to confirm these results.

[Value of hypovolemia in subarachnoid hemorrhage].

Hypovolemia seems a commun state in patients with subarachnoid hemorrhage. It has been further suggested that hypovolemia could cause ischemia if vasospasm is present. Total blood volume and red blood cells volume determined in 84 consecutive patients volume of distribution (of 51Chromium labeled autologous red cells, shows an hypovolemia in 70% of these patients). Associated hypomotremia requires Na restitution with normovolemic hemodilution to facilitate cerebral blood flow.

[Study of an epidemic of Clostridium difficile-associated diarrhea].

Twenty three Clostridium difficile strains isolated from diarrheic faecal samples of 22 patients in a neurosurgical department of the Pellegrin Hospital in Bordeaux between January 1984 and May 1987 (15 isolated in 1984, 2 in 1985, 3 in 1986 and 3 in 1987) and 15 strains isolated from 15 patients in other departments of the same hospital in 1984 have been compared using their antimicrobial susceptibility, cytotoxin production and electrophoretic pattern. Twelve of the 15 strains isolated in the neurosurgical department and 4 of the 15 strains isolated in other departments in 1984 were phenotypically identical. These findings suggest that the origin of C. difficile associated-diarrheas can be nosocomial.

[Comparison of two skin antiseptics in blood transfusion. Comparison with the results of the bacteriologic control of labile blood products].

The comparison of two skin antiseptics (70 degrees alcohol and 0,5% alcoholic chlorhexidine solution) was carried out by the method of in vivo impressions. The study used 45 healthy volonteers from whom a bacteriological sample was taken from both bends of the elbow, without use of an antiseptic, and after the application of one according to usual sample taking methods. The subjects were divided into two sets according to the antiseptic being tested. The results of the cultures after 24 and 48 hours are expressed in the number of germs per cm2, which thus permits us to calculate a reduction in percentage or log 10 form, and to appreciate the efficiency of the antiseptic studied. Routine bacteriological inspection of labile blood products was carried out on products picked at random every week during 18 months of exclusive use of 70 degrees alcohol by the sample taking services. The results show a comparable in vivo effectiveness of the two antiseptics on the superficial aerobic flora of the bend of the elbow, and the ease of use of 70 degrees alcohol has led to its choice. This choice is confronted with the results of the inspection of blood products: out of 1 293 inspections, only one slight contamination (cocci gram+) was found on a unit of whole blood. These data as a whole can be compared with the work of various authors.
