[Descriptive anatomy of the posterior root of the trigeminal nerve (preliminary study)].

By 25 dissections, an angulation of 40 degrees at the emergency of the trigeminal nerve was observed. To the three types of fibers described by SICARD, a fourth type is added of V2 fibers wrapped by cranio-median and caudo-lateral fibres. Two groups form the motor root, a postero-superior group and an antero-inferior group. Anastomoses exist between the motor and sensitive roots as well as a few unusual sensitive rootlets.

[Technics and indications of general anesthesia for tonsillectomy and adenoidectomy. Apropos of an experience using anesthetic perfusion and cold loop technic].

The authors present the results of an enquiry carried out in University and regional hospitals in France and recall the interest of general anaesthesia with tracheal intubation during tonsillectomy. This makes possible, even in young children, the use of the technique of cold loop dissection. Its main advantage is in the absolute control of the airways which permits the operator to carry out careful hemostasis. Propanidide used in perfusion, allows these patients to wake up rapidly and the awakening is of good quality; this is essential in this type of operation.

[Problems presented to the surgeon, anesthetist and resuscitation personnel by fractures of the malar bone (study of 100 cases)].

The authors emphasize the difficulties encountered with fractures of the zygomatic bone, both from the technical operative points of view and conditions of anaesthesia and intensive care. They distinguish in both fields, the so-called simple zygomatic fractures from fractures associated with multiple injuries of the face or skull. After recalling the surgical techniques, they consider routine management, from diagnosis to recovery from the anaesthetic, emphasizing the problem of the time of the operation, of the method ensuring freedom of the airways and anaesthetic indications used together with problems of intensive care.

[Neurologic complications of pagetic basilar impressions and their surgical treatment].

Paget's disease of the skull is the main cause of basilar artery syndromes in the adult. They may cause various neurological symptoms, including signs of ischemia of the spinal cord or medulla, or involvement of the cranial nerves and brain stem and, also, distant symptoms due to hydrocephalus, with various mental disorders including transient, recurrent, coma. The authors discuss 30 cases found in the world literature and 6 unreported personal cases, study the clinical symptoms of these cases of basilar artery compression due to Paget's disease, and the methods of neuro-radiological investigation, and emphasize the interest, in severe forms, of early surgical decompression before the stage of severe neurological complications. Regular supervision of patients with Pagets disease is thus essential to detect at an early stage, decompensation of basilar artery insufficiency in Paget's disease. In late forms, calcitonin may be indicated.
