[Kaposi disease, limited to the skin, in ulcerated hemorrhagic rectocolitis].

We report the first case of limited cutaneous Kaposi's sarcoma during ulcerative colitis, in a HIV-negative 68 year-old man without any risk factors for this tumor. In our case, the steroid-therapy necessitated by the colitis is the most important aetiological factor of the sarcoma and its withdrawal leads to a complete remission of the tumor. The link between steroid-therapy and Kaposi's sarcoma is well known. A link between ulcerative colitis and Kaposi's sarcoma can be discussed but not proven. The possible occurrence of this cutaneous tumor during ulcerative colitis is of interest.

Radial artery compliance in young, obese, normotensive subjects.

Obesity is characterized by a number of cardiovascular alterations, and whether these alterations involve arterial compliance is unknown. In 12 young, obese, normotensive subjects (age, 23.9 +/- 1.3 years; mean +/- SEM) and 12 age- and sex-matched lean control subjects we measured blood pressure, radial artery diameter, and radial artery compliance continuously over the systodiastolic pressure range with a Finapres device and recently developed echo-tracking device. Measurements were obtained at baseline and after prolonged ischemia, that is, when diameter and compliance are increased. Blood pressure values were normal in both groups (obese subjects: 109.2 +/- 4.9/68.2 +/- 2.7 mm Hg; lean control subjects: 108.2 +/- 4.1/60.7 +/- 3.8 mm Hg), but in addition to a marked increase in body mass index (38.5 +/- 0.8 versus 23.1 +/- 0.9 kg/m2, P < .01), obese subjects showed a slight and nonsignificant increase in heart rate (71.1 +/- 3.2 versus 66.7 +/- 3.3 beats per minute, P = NS), increases in left ventricular wall thickness and left ventricular mass index (121.5 +/- 4.8 versus 103.4 +/- 3.3 kg/m2, P < .01), no changes in plasma renin activity and plasma norepinephrine (compared with normal values), and a marked reduction in total body glucose uptake (glucose clamp technique). Obese subjects showed radial artery diameter and compliance values that were greater than those seen in control subjects throughout the systodiastolic pressure range. The differences were 13% (P < .05) and 96% (P < .01), respectively, and both diameter and compliance remained higher in obese than lean subjects after forearm ischemia. In obese and lean subjects baseline radial artery diameter values correlated highly with body weight, body surface area, and body mass index.(ABSTRACT TRUNCATED AT 250 WORDS)

Clinical screening for narcolepsy/cataplexy in patients with fibromyalgia.

We report two cases of fibromyalgia occurring during the course of a long-standing narcolepsy/cataplexy. This association is probably not fortuitous and we propose an easy clinical procedure to search for narcolepsy/cataplexy in these patients. Narcolepsy/cataplexy is a treatable condition requiring drugs that in turn may improve in the long-term symptoms of fibromyalgia.

[Digital necrosis disclosing antisynthetase syndrome].

The antisynthetases syndrome associates lung disease, polymyositis, Raynaud's phenomenon and frequently polyarthritis. Anti-amioacyl-tRNA synthetase antibodies are present. The most common is anti-Jol. We report a case with severe arteritis of the finger with multiple zones of skin necrosis which responded favourably to corticosteroids given in bolus and cyclophosphamid. Finger arteritis, in addition to classical Raynaud's phenomenon, may be a new manifestation of the anti-synthetases syndrome.

Significance of isolated vasculitis of the vasa vasorum on temporal artery biopsy.

OBJECTIVE: To assess the significance of an isolated vasculitis of vasa vasorum on temporal artery biopsy. METHODS: A diagnostic procedure was performed with 28 patients presenting clinical and biological inflammatory process and an isolated vasculitis of vasa vasorum on temporal artery biopsies. RESULTS: Panarteritis nodosa was diagnosed in 5 cases: polymyalgia rheumatica was evidenced in 4 cases; temporal arteritis was strongly suspected in 2 cases; various types of underlying disease were found in 12 patients; a noncharacterized systemic disease was found in 5 cases. CONCLUSION: Isolated vasculitis of vasa vasorum on temporal artery is a meaningful lesion frequently indicating a systemic vasculitic disorder.

[Crescentic glomerulonephritis and primary Gougerot-Sjögren syndrome].

Glomerulonephritis complicating primary Gougerot-Sjögren's syndrome is extremely rare. We report the case of a 72-year old woman with primary Gougerot-Sjögren syndrome complicated by progressive renal failure. Kidney biopsy revealed a crescentic glomerulonephritis. A rapid improvement occurred with corticosteroids. This observation is, at our knowledge, the first case of crescentic glomerulonephritis described during this disease. A review of the literature concerning glomerulonephritis complicating primary Gougerot-Sjögren syndrome is reported.

Concurrent acute megaloblastic anaemia and pneumonitis: a severe side-effect of low-dose methotrexate therapy during rheumatoid arthritis.

In a patient suffering from rheumatoid arthritis, we report the first simultaneous occurrence of two side effects of low-dose methotrexate: an acute megaloblastic anaemia and a pneumonitis. A combination of methotrexate suspension, folinic acid and corticosteroids led to recovery. The correlation between the haematologic and pneumologic toxicity is discussed.
