[Percutaneous endoscopic gastrostomy (PEG)].

When gastric decompression or enteral feeding are needed for patients affected by neurological problems or neoplasias with a long life expectancy (months), PEG represents the method of choice because it has a lesser number of complications, needs a mild sedation, and has a lower cost compared to surgical techniques.

Hematotoxicity of 5-fluorouracil and alpha-interferon on the human normal hemopoietic progenitors.

The toxic effects of a combination of 5 fluorouracil (5-FU) and interferon alpha on normal human hemopoietic progenitors were assessed in vitro. CFU-GM growth was evaluated following 1 h incubation of bone marrow cell suspension with 5-FU 100 micrograms/ml or IFN-alpha 2a 1000 U/ml, or with both drugs, either simultaneously or sequentially added. IFN-alpha did not show toxic effects on normal human hemopoietic progenitors and the severe toxicity of 5-FU on the latter was not modified by IFN-alpha. These results suggest that the changes observed in vivo of 5-FU toxicity induced by IFN-alpha should be attributed to indirect effects of the latter.

[Pulmonary metabolism of beta-endorphin in asthmatic patients in asymptomatic periods and after bronchospasm induced by methacholine].

Blood concentration of endogenous beta-endorphines can change during the clinical evolution of chronic bronchopneumopathies. The authors assessed the beta-endorphine concentrations in the pulmonary arterial and systemic arterial blood in 8 asthmatic patients during a symptom-free period and after methacholine-induced bronchospasm. The beta-endorphine analysis was performed in duplicate dor each sample, by means of a RIA assay. There is not difference in the systemic arterial blood concentration of beta-endorphines between asthmatic patients and normal subjects. Furthermore, there is no change in the beta-endorphine blood concentration during the passage through the pulmonary tissue after methacoline-induced bronchospasm.

[Pneumatic dilatation in the treatment of esophageal achalasia].

The purpose of therapy in esophageal achalasia is to reduce the pressure at the level of the lower esophageal sphincter. In this study 26 patients (16 males and 10 females) between 30 and 50 years of age, affected by esophageal achalasia underwent esophageal dilatation with Rigiflex pneumatic dilators. A total of 40 dilatations were performed. A complete success using the Rigiflex pneumatic dilators, was achieved in 24 out of 26 patients (92.3%).

Feasibility and toxicity of combination chemotherapy with ifosfamide, vinorelbine, cisplatin versus ifosfamide, vinorelbine in patients with advanced non small cell lung cancer.

Vinorelbine (VNB) and Ifosfamide (IFO) have recently been proposed for treatment of non small cell lung cancer (NSCLC). The two drugs separately induce response rates in excess of 20% and, when combined, of 32.56%. Cisplatin (DDP) is considered a standard in chemotherapy of NSCLC affected patients. We report data on the feasibility and the toxicity of an IFO, VNB and DDP combination in comparison with IFO, VNB association. Results obtained show that the IFO, VNB, DDP combination has a more severe toxicity profile than the IFO, VNB combination although not to a degree precluding its feasibility. Responses, however, appear somewhat more favorable than in the group treated with the combination IFO, VNB. It is therefore necessary to ascertain if clinical advantages in survival and symptom palliation offered by IFO, VNB, DDP combination outweigh impairment in quality of life due to its significant toxicity.

Chronic treatment with azathioprine and cyclophosphamide in rats: structural and functional effects on small intestine and liver.

A report is presented of the effects upon the liver and enteric mucosa of rats treated chronically with azathioprine (AZA) and cyclophosphamide (CY) in equiactive doses three times greater. The groups treated either with equiactive doses of AZA and CY or with doses three times greater of AZA and CY presented limited and moderate morphological alteration in the liver and in the mucosa of the small intestine, while all rats treated with high dose of CY died in the first phase of the experiment.
