A novel, selective synthetic acetamide containing culture medium for isolating Pseudomonas aeruginosa from milk.

A selective synthetic medium has been developed both in liquid (Z-broth) and solid (Z-agar) forms for selective isolation of Pseudomonas aeruginosa from foods. The simple, easy to prepare peptone-free synthetic medium contained acetamide that is metabolized to ammonia and acetic acid providing nitrogen and carbon supply. The medium contained no inhibitors. Selectivity of the liquid medium was tested by inoculation of pure cultures of different bacteria belonging to the groups Bacillus, Pseudomonas, Enterobacteriaceae and Staphylococcus. It was found that the selectivity of the medium was complete for the examined range of bacteria. However, a similar result was obtained when nitrofurantoin broth was used. Applicability of the synthetic agar medium was also tested by a nation-wide inter-laboratory test using two milk samples containing 10(3)/ml (sample I) and 10(5)/ml (sample II) Pseudomonas aeruginosa. According to this test, no microbiologically relevant differences were found between the results obtained by Z-agar and cetrimide-agar a frequently used selective agar in case of sample II. However, a relevant and statistically significant difference was found in the results of sample I in favour of the Z-agar, that could indicate the presence of a low number of bacteria. Concerning repeatability and reproducibility, Z-agar proved to be superior to cetrimide agar.

Acetamides

Iron status in a healthy population of Hungarian secondary school boys and girls.

Iron status was assessed in a sample of 103 male and 282 female students aged from 15 to 18 years attending secondary schools in Budapest. Using a ferritin model, in boys the prevalence of iron depletion, iron-deficient erythropoiesis and iron-deficient anemia was 3.9, 2.9 and 1.0%, respectively. In girls these values were 9.6, 8.2 and 2.8%, respectively. Males had better eating habits than females. Data for nutrients intake connected with iron status are discussed. The authors recommend the improvement of iron status by dietary means and oppose the indiscriminate iron supplementation.

[Connection between nutrition and economy].

The economical conditions remarkably influence through food supply the nutrition although it is an a priori biological category. These determine the implementation of nutritional habits, conscious nutritional intentions, and limit the chances of individual decisions. The influence of economical conditions could be measured by use of "income elasticity" and "dietary efficiency". The latter serves as background for the elaboration of diets of different price but approaching the nutritional physiological optimum. The households have an important economical role because of value of time used for food purchasing and preparation of meals. In general the GNP/capita values determine the proportion of nutrients in diet and by this means the risk and frequency of diet-related diseases. Some of Central and Eastern European countries, among them Hungary as well, take place in groups higher by one-two categories, than appropriate to their GNP/capita. In Hungary the price of foodstuffs increased vigorously and the food consumption decreased by 3 per cent between 1994 and 1980 or 1992. Mainly the consumption of meat, milk and dairy products has diminished. The meat consumption has been shifted towards the consumption of poultry showing lower rise in prices. The consumption of vegetable oil and margarine has increased essentially because the lard became more expensive than the vegetable oil. The changes in prices touch more strongly the people of low income, their diet is combined chiefly from cheap foodstuffs and therefore it is monotonous, the qualitative starvation appears. Sociological surveys showed poverty in 17-31 percent of population in 1992. The inequality in income shows a strong association with the health status of population, furthermore with the lifestyle and the features of nutrition. Also in Hungary we should reckon with the unfavourable consequences of economical factors in the scope of health status and nutrition.

Cost-Benefit Analysis

Proximal bleeding control obtained by a balloon catheter in the surgical repair of a left supraclavicular traumatic arteriovenous fistula.

Endovascular procedures offer various methods for the treatment of posttraumatic arteriovenous fistulas. We report the treatment of a posttraumatic arteriovenous fistula between the left subclavian artery and internal jugular vein combined with a large subclavian false aneurysm. To avoid left thoracotomy we attained control of arterial bleeding by means of a transfemorally introduced balloon catheter advanced into the left subclavian artery. We found that the combination of endovascular procedures with classical surgery can reduce the risks in the treatment of arteriovenous fistulas.

Nutritional survey of pregnant women in Hungary.

A nutritional survey of a Hungarian group of pregnant women was carried out. One hundred and twenty nine women aged 25.9 years, entered the study, but only 70 completed all the protocol. Average body weight gain was 12.4 kg and the mean birth weight of the newborns was 3,386 g. Mean energy and nutrient intakes of pregnant women showed similar patterns as in Hungarian non-pregnant women of the same age. The mean energy intake was high (11 MJ), being 10% higher than for non-pregnant women. The mean protein and lipid intakes were also high, 91.9 g and 108.4 g, respectively, exceeding by 7% and 6% the intakes of the non-pregnant women. Dietary intakes of saturated fatty acids (SFAs) and monounsaturated fatty acids (MUFAs) were close to 12% of energy, and the intake of polyunsaturated fatty acids (PUFAs) was 7.6% of total energy. Palmitic acid (16:0), oleic acid (18:1 n-9) and linoleic acid (18:2 n-6) made the greatest contribution to the total peak area of SFAs, MUFAs, and PUFAs, respectively. The ratio of P/S (polyunsaturated/saturated fatty acids) was appropriate: 0.65; however, the ratio of linoleic acid (18:2 n-6), to linolenic acid (18:3 n-3) was high: 16. The cholesterol intake was somewhat lower in pregnant than in non-pregnant women. Complex carbohydrate intake was low (245 g) but it was still by 75% higher than in non-pregnant women. The excess sodium intake (6.3 g) was very similar to that of non-pregnant women. Mean values for retinol, tocopherol, ascorbic acid, cobalamin and copper intakes were higher than the Hungarian Recommended Dietary Allowances (RDA). Thiamine, riboflavin, pyridoxine, niacin, calcium, iron and zinc intakes were insufficient. Data showed an imbalance in the energy and nutrient intakes of Hungarian pregnant women, and this could be harmful for both the mother and pregnancy outcome.

Nutrition survey of the Hungarian population in a randomized trial between 1992-1994.

OBJECTIVE: Collection of data relating to nutrition, nutrient supply and nutritional status, and to certain risk factors of diet-related diseases in a group of Hungarian population. DESIGN: Collection of data on diets three times 24h dietary recall, anthropometric measurements to obtain information about the nutritional status, laboratory tests measurements of biochemical markers. SUBJECT: Examination of 2559 volunteer adult persons, selected in a quasi random way in Budapest and seven other counties. RESULTS: The diets of the persons examined contained more than necessary protein 14.6 en%, considerably more fat 38.0 en%, mainly fat of animal origin with low PUFA and including high cholesterol, little total carbohydrate 45.1 en%, in that much added sugar 15.4 en%. Vitamin supply was generally satisfactory, borderline deficiency may be observed in the case of tocopherol equivalent and in some age-groups, first of all in women, in the case of thiamine, riboflavin and pyridoxine. Out of the macro- and microelements, sodium intake is three to four times the acceptable level, potassium and calcium intakes in both genders, iron and zinc intakes in women show deficiency. The mean value for relative body mass, the distribution of BMI values prove a high incidence of overweight 34.3% and obesity 21.1%. Means for biochemical markers appear to indicate that nutrient supply was in the normal range of laboratory tests. The averages of total cholesterol and LDL-cholesterol 5.6 and 3.6 mmol/L, respectively unequivocally suggested the risk of cardiovascular diseases. CONCLUSIONS: The nutrition of the Hungarian population presents several risk factors of diet-related diseases: P/S ratio of 0.27-0.29, Na/K ratio of 2.6 g/g, intake of some micronutrients (vitamins, macro- and microelements) was low in certain groups of investigated persons.

Cardiovascular risk factors distribution in Hungarian adults.

Between 1992-1994 nutrition and nutritional status of 2259 persons (1173 men, 1386 women) were examined in Hungary. The average of dietary fat intake was 38.0 (5.7) in %, the median of P/S ratio 0.26-0.27, showing the dominance of saturated fatty acids. According to TC, 34.4% of the examined persons could be ranked to moderate and 27.4% to high risk groups. Regarding LDL-C levels, 27.2-27.2% belonged to both groups. TG indicated risk in 19.5% and 2.4%, respectively and HDL-C values were under the lower limit of normal range in 7.6% Sodium intake is three- to fourfold of the desirable quantity, that of potassium just reaches or remains under the recommended level. As a result, the value of Na/K ratio (mmol/mmol) is over 4. Borderline and/or define hypertension was found according to systolic pressure in 27.4 and 7.9% of men and in 18.1 and 5.2% in women. Diastolic pressure was in this domain in the case of men in 16.9 and 21.9%, in women in 12.7 and 16.1%. Hypertension is frequent both in middle-aged and old men and women. The high CVD mortality in Hungary may be in considerable part explained by the outlined risk factors.

Serum lipid pattern of adolescents in Hungary.

The authors have measured and compared the lipid parameters in groups of adolescents aged from 15 to 18 years with and without a history of myocardial infarction of parents. No essential differences were seen among the lipid variables of the two groups. Nonetheless in a considerable part of both girls and boys in either group serum total cholesterol and LDL-cholesterol was higher than the "acceptable" values. The authors propose the initiation of an immediate intervention program in the population serving preventive purposes.

Effect of sex hormones on copper, zinc, iron nutritional status and hepatic lipid peroxidation in rats.

Sex dependence of copper, zinc, iron nutritional status and hepatic lipid peroxidation was studied in gonadectomized, hormone substituted and sham-operated male and female rats. The experiment was performed on H-Wistar rats (106 +/- 10 g) fed conventional laboratory food ad libitum for 12 weeks. The estrogen dependence of copper status is fully confirmed by the results of this study. In hormonally active females the malondialdehyde production was depressed which is supposed to be connected with high copper and low triglyceride levels in the liver. The anaemia observed in estrogen substituted rats may be a result of lower lipid peroxidation rate. The levels of retinol and alkaline phosphatase activity in sera are probably influenced by estrogen action. It is suggested that testosterone leads to acceleration of lipid peroxidation.

Hepatic lipid peroxidation and trace elements--nutritional status in streptozotocin-induced diabetic rats.

The aim of this experiment was to study the interrelationships between nutritional status of chosen trace elements (Cu, Zn, Fe) and hepatic lipid peroxidation in streptozotocin-induced diabetic rats. Both copper accumulation and disruption of iron storage were observed in livers of diabetic rats. MDA 0' (baseline) and MDA 30' (produced) levels measured in the liver were negatively correlated with blood glucose levels. MDA 30' levels correlated positively with iron concentration in the liver. It is supposed that the hormonal lability during experimental diabetes caused changes in metabolism of trace elements, and subsequently influenced the rate of lipid peroxidation.

Ephaptic influence of the electrical activity of muscle on the neighboring nerve.

Experiments were performed on isolated nerve-muscle and leg preparations of frog in order to investigate the excitatory influence of the electrical activity of muscle on the contiguous nerve under various conditions. The results show that the active muscles are able to induce multiple transmission of excitation from muscle to nerve and to modify the excitation travelling in the nerve being in contact with the muscle. It is concluded that under some special circumstances the ephaptic influence of the electrical activity of muscle on the nerve in its neighborhood can play an occasional role in the processes related with the nerve and muscle excitation.

Action Potentials

[Treatment of an infected vascular prosthesis by irrigation-suction (lavage)].

The number of the infections of vascular prosthesises is more and more increasing in the past few years, in connection with the developing of vascular surgery. In our clinic, all septical complications amount to a 2-3% of the total number of reconstructive vascular interventions (8). In a period of 01. 01. 1980-06. 30. 1988 lavage procedure was performed in 19 cases. In 14 patients a primary wound healing was available, in 2 patients after moving away of the original prosthesis, an extra-anatomical bypass was necessary, and in three patients we could not avoid amputation. The above mentioned method is known in the literature since 1959, but in spite of this, there are few publications--with few cases--about this topic. The authors emphasize the relatively good results of this simple method.

Epidemiology of Salmonella derby strains isolated from swine, pork and pork products.

A total of 35 Salmonella derby strains, isolated from 6 types of samples of porcine origin from 9 different places in Hungary were examined for their characteristics. Thirty-two strains (91%) were of phage type 25, 2 (6%) of phage type 15 and 1 (3%) of phage type 58. Colicin production was observed in 3 (9%) strains. Five strains (14%) were found to be resistant to tetracycline (Tc). The strains harboured plasmids of 2.2, 2.4, 3.4, 4.2 and 72 Md. The 72 Md plasmid appears to be characteristic of S. derby and possibly encodes Tc resistance. The 72 Md plasmid belonged partly to incompatibility (Inc) group I1, while the other plasmid of the same size belonged to Inc. group B. The findings suggest that healthy salmonella carrier pigs carried the infection from the farm to the abattoir. Slaughtering of infected pigs may have led to contamination of the carcasses and, thereafter, that of the pork and pork products.
