[Epidemic due to group B Coxsackie viruses in a newborn infants' department (author's transl)].

Virological and clinical researches have been done on hospitalized newborn infants' with various infectious diseases (respiratory infections, aseptic meningitis, serious generalized disease). Initially from some newborn children Coxsackie B5 virus has been isolated and later from other patients Coxsackie B3 virus. The pathological findings, in those newborn who died as a result of serious generalized disease were dominated by hemorragic lesions in different organs.

Social class, prenatal care, maternal age and parity: a study of their interrelation in six Italian centres.

"Multiple Correspondence Analysis was used to describe the complex structure formed by those sociodemographic variables, whose association with the occurrence of prenatal and neonatal deaths and diseases has been most frequently stressed in literature: social class, prenatal care, maternal age and parity. The study regards 41,537 women included in a multicentre survey of perinatal preventive medicine, which was carried out, between 1973 and 1979, in six Italian centres...." It is found that "in all centres there are distinct groups of women characterized by a set of unfavourable factors closely interrelated: low social class implies lower prenatal care, higher occurrence of precocious or belated childbearing and higher number of pregnancies, often unintended." Cigarette smoking in pregnancy: relationship to perinatal outcomes in six Italian centres.

In this study the relationship of maternal smoking in pregnancy to low birth weight, fetal growth retardation, preterm delivery, and perinatal deaths has been investigated in a sample of 36,544 women with their single newborns, from 6 Italian centers (Triese, Milan, Parma, Rome, Naples, Bari), where a multicenter survey of perinatal preventive medicine (MPPI) was carried out, between 1973 and 1979, with the financial support of the Consiglio Nazionale dell Ricerche. The proportions of babies weighing 2500 g or less, and of babies with birth weight, head circumference or crown-heel length below the 10th centile were similar among nonsmokers and "stopped" smoker mothers, while they increased with increasing smoking levels. As regards head circumference and crown-heel length below the 10th centile, odds ratios are essentially the same before and after adjustment for age, parity, and socioeconomic class (about 1.5 for "1-10CGT" and up to 2.5 for "11+CGT"). Only slight increases have been observed in the odds ratios concerning low birthweight below the 10th centile (about 1.5 for "1-10CGT" and up to 3.5 for "11+CGT") after adjusting for the socioeconomic demographic variables (i.e. mainly removing the favourable effect of high socioeconomic class or low parity, which more frequently characterize smoker mothers). Owing to the lower smokers' frequencies, cigarette smoking estimated attributable risks concerning low birth weight and "small for dates" babies in the MPPI centres (between 4% and 19%) are considerably lower than those estimated in studies carried out in the US or Canada. Data analyzed in the present study do not indicate any relationship between maternal smoking and preterm delivery or perinatal deaths.

Smoking habit in pregnancy and sociodemographic background in six Italian centres.

Studies carried out in foreign countries (US and UK, mainly) indicate that maternal characteristics, such as age, parity, social class, and prenatal care, are related to child's growth, mortality, and morbidity, as well as to cigarette smoking. These characteristics may act as confounding variables in the analysis of the effects of maternal smoking on babies in fetal and neonatal periods. Until now there has been a lack of information on the subject, because even the most recent available data concern women over age 14 regardless of obstetric history. This paper deals with smoking habits, before and during pregnancy, of 37,664 women included in a multicenter survey of perinatal preventive medicine (MPPI), which was performed in 6 Italian centres (Trieste, Milan, Parma, Rome, Naples, Bari) between 1973 and 1979, with the financial support of the Italian National Research Council. The results of the MPPI and other surveys are compared and the association between maternal smoking habits and sociodemographic background is investigated by multiple correspondence analysis. As to Italy, unlike UK and US, in the 1970s women of high social status show the highest prevalence of the smoking habit. Moreover, in pregnancy, the large majority gives up smoking, or at least reduces it, mainly in high socioeconomic levels, so that the proportion of pregnant women who keep on smoking over 10 cigaretts per day is very low (0.5-3.8%) and poorly related to sociodemographic factors. Therefore, it seems unlikely that these may exert serious confounding effects on the relationships between smoking in pregnancy and perinatal outcome.

Decreasing expression of NM23 gene in metastatic murine mammary tumors of viral etiology (MMTV).

Murine mammary tumors induced by the Murine Mammary Tumor Virus (MMTV) were chosen to study the expression of the NM23 gene during the metastatic process because of their viral etiology, different from that of the previously reported experimental tumor systems. NM23 mRNA levels are higher in non metastatic tumors than in metastatic ones. Moreover, the NM23 expression is higher in tumors induced by the C3H variant of the MMTV than in tumors induced by the RIII variant. These data are a further support to the hypothesis of a basic role of the NM23 gene in the down-regulation of tumor progression.

Efficacy of intravenous immunoglobulin therapy in a case of autoimmune-mediated chronic active hepatitis.

In a patient suffering from histologically-documented immuno-mediated chronic active hepatitis, intravenous immunoglobulin (IVIG) treatment was initiated as an obligatory alternative therapy because of femur head aseptic necrosis which had followed long-term therapy with steroids. The first cycle of IVIG was followed by remission of most symptoms, normalization of liver enzymes, disappearance of circulating immunecomplexes (CIC), and a negative lupus band test. Improvement in the patient's condition was demonstrated, in liver histology, by the disappearance of peri-portal mononuclear cells infiltrates, while immunohistochemistry showed the disappearance of the intracellular IgG deposits.

[Guidelines for health surveillance of health care workers exposed to biological risks set up by the Italian Society for Occupational Health and Industrial Hygiene: application in health facilities of the Regional Health Administration--District No. 7, Ancona].

BACKGROUND: Evidence Based Medicine and the need to achieve better management of health budgets call for verification and, if necessary, revision of the criteria used in Occupational Health, in order to ensure appropriate measures as regards protection of health at the workplace. In December 2003, the Marche Region initiated a process of reform of the regional health service, which will be completed in two years, and will provide a new regional health organization that will improve the quality and appropriateness of health services available to the population. The reform also involves the Occupational Health Services responsible for prevention activities for 20,000 health care workers employed in regional public health facilities. The need was strongly felt to set up a network that would meet the health needs of health care workers, by adopting a common language among occupational health physicians, by eliminating reported criticism, which is due not only to lack of communication between different structures and profiles, but also to the different methods of approach, evaluation and management of occupational risks. OBJECTIVES AND METHODS: From a historical point of view, the health sector has the biggest as regards prevention of biological risk. Therefore, with a view to harmonizing actions and approach among occupational health physicians in the evaluation of this risk, the publication by the Italian Society for Occupational of Health and Industrial Hygiene of Guidelines for health surveillance of health care workers exposed to biological risks, produced by the working group under the leadership of Prof. Lorenzo Alessio, was considered to offer an interesting opportunity to verify the reproducibility of methods and the quality of results, as applied to health facilities under the Regional Health Administration in Ancona (District No. 7). RESULTS AND CONCLUSION: The Guidelines fully confirmed the need to plan activities, starting from analysis of epidemiological and occupational data, thus assuring good results both in terms of efficacy and efficiency of the health surveillance programme used. This method also assures a high level of appropriateness of medical measures as regards the "safety" target, at the same time avoiding waste and poor management of human and economic resources, which were till now caused by differences in methods used in assessment of biological risk.

[Assessment of the presence of mutations of the epidermal growth factor receptors in tumors of various histologies].

Recent reports from US and Japan have established that mutations in the tyrosine kinase (TK) domain of the epidermal growth factor receptor (Egfr) occur in a subset of patients with lung cancer that respond to therapy with gefitinib, a TK inhibitor. To gain further insights into the role of Egfr in carcinogenesis of lung and tumors of diverse histology, that are currently under investigation with drugs of the same class, we have taken in examination a panel of tumors consisting in 110 pulmonary adenocarcinomas, 40 pulmonary squamous carcinomas, 40 gastric adenocarcinomas and 40 colorectal adenocarcinomas. The sequence analysis of exon 19 and 21 of the Egfr has allowed the identification of 10 cases exhibiting specific deletions in exon 19 and 1 case with point mutation in a conserved residue in exon 21. All Egfr mutations occur specifically in lung adenocarcinomas while tumors of different histology result unaffected. The rate of mutation affecting these other tumors is either very rare, involves different domains of the receptor or other tyrosine kinases. The molecular analysis of the Egfr gene can help identify patients that will benefit from gefitinib therapy.

[Perinatal risk factors for infection in the newborn. Multicenter clinico-epidemiologic investigation].

UNLABELLED: The neonate, in particular if preterm, has a specific immunological system that makes him/her more susceptible to infections which are still a major cause of mortality and morbidity. The early onset infective patterns are often vertically transmitted from the mother to the fetus in the perinatal period. Some mother conditions like genitourinary infections, pre-delivery fever, PROM, pPROM, preterm delivery, abortions, fetal demise are important neonatal risk factors. The role of these factors as causes of early onset neonatal infections was evaluated by an Italian multicentric epidemiologic investigation supported by the MIUR. Mothers admitted to obstetrician hospitals for parturition were studied in a case control retrospective analysis. Mothers presenting with the selected risk factors were the cohort of cases while the admissions without risk factors next to each case represented the controls. The following risk factors were considered for analysis: 2nd trimester abortion, PROM, pPROM, preterm delivery and fetal demise. Eight hospitals entered the multicentric research and 29610 patients have been analyzed: 2466 PROM, 478 pPROM, 946 preterm delivery, 244 abortions, 133 fetal demise. Every woman received a microbiological screening by cervico-vaginal and rectal swab and/or urine culture. As much as 3892 cases from the University Hospitals of Parma and Torino concern, the cervicovaginal swab was positive in 76.5% pPROM, in 50.6% PROM and 50.5% preterm deliveries. The positivity of the swabs showed statistical relevance in cases compared to controls. In cases presenting with abortion the frequency of positive cultures (44.1%) was higher than the controls, but it did not reach statistical relevance. Conversely, positivity of cultures was lower in cases with fetal demise than control group but without statistical difference. In the cohort of 675 women selected from our Institute (University of Parma) the overall rate of positivity of cervico-vaginal swabs was about 44% and only one germ was isolated in 94.6% cases: 49,8% Gram positive and negative, 14.1% Streptococcus hemolyticus B group, 34.7% Candida. The rate of cultural tests performed before delivery was statistically higher in cases than controls (74.3% vs 23.2%, p < 0.001). Furthermore 14.3% newborns from mothers in the case group and 2.3% newborns from mothers in the control group were admitted to the neonatal care unit. Among them 8.4% and 1.2% respectively needed antibiotics while 0.4% and 0.2% respectively presented with early onset infection. Ear swabs were performed in 48,8% of infants born to mothers from the case group and in 26.9% of infants born to mothers from the control group, in 6.5% and in 2.9% respectively were the skin swabs performed as well. No statistical difference was met. The choice to perform cultural examinations was statistically higher in cases than controls, but the positivity rate was similar in both groups, perhaps because of a more specific choice in the control group. CONCLUSION: our data confirm the relationship between infections and preterm delivery, PROM, pPROM and support the hypothesis that infections might be the cause of these conditions and not simply an association. A prompt maternal microbiological control during pregnancy and delivery, especially in women presenting with known risk factors, gives the possibility to do early diagnostic-therapeutic interventions and to minimize the frequency and severity of early sepsis in newborns.

[Histological and ultrastructural aspects of oto-mastoid cholesteatoma in children].

Cholesteatomas were intraoperatively removed from subjects under 14 years of age presenting widespread cholesteatoma with bone erosion. The samples were studied under light and transmission electron microscopy in order to consider the features of inflammation and bone erosion. The results showed that the perimatrix of cholesteatoma in children is rich in mononuclear inflammatory elements and generally presents the features of chronic, as well as acute, inflammation. The perimatrix infiltrates and erodes the surrounding bone. Next to the resorption areas many areas were observed having new bone the surface of which is lined with osteoblasts. The new bone formation activity is marked in these cases of childhood cholesteatoma and appears to be an attempt at spontaneous repair which is thwarted by the persistence of inflammation.

[Mitochondrial oculopharyngeal myopathy: description of a case].

We describe a 55 years old man affected by eyelid ptosis, mild ophthalmoplegia externa and severe dysphagia owing to pharyngoesophageal dyskinesia. Skeletal and eyelid constrictor muscles EMG showed myogenic alterations. Muscle histological findings of ragged red fibers and oxidative histochemical alterations, together with ultrastructural investigation clue for mitochondrial myopathy diagnosis. This, considering the unusual clinical aspects of the case, confirms as mitochondrial disease can widely vary in their phenotypical expression.

[A case of aneurysm of the celiac trunk ruptured into the duodenum].

A case of repeated, exsanguinating gastrointestinal haemorrhage in a 36-yr-old man, due to rupture in the duoedenum of an arteriosclerotic aneurysm of the coeliac artery, is described. Selective arteriography enabled the cause of bleeding to be established. The pictures for the coeliac and superior mesenteric arteries showed good hepatic arterial vascularisation via the vessels issuing from the right hand side of the superior mesenteric, immediately after its point of origin. Successful aneurysmectomy was not performed until the fifth operation, when the cause of bleeding had thus been made clear.

[Neurologic pathology in the neonatal period: relation to birth weight and gestational age].

Six Italian university centers have taken part in the Perinatal Preventive Medicine Project of the National Research Council since 1973. In this report the preliminary data on neonatal neurological disorders of 38775 single not malformed infants are presented. Neurological abnormality has been defined by the presence of at least one of the following symptoms: seizures, hypertonia, hypotonia, apneic spells. The relative frequencies of seizures vary from a minimum of 0.28% to a maximum of 0.75% in the six centers. The frequencies of the other symptoms demonstrate a greater variability among centers. Males are more affected than females. The relative frequency of neurological abnormalities is higher among babies with low birthweight, short gestational age or retarded intrauterine growth. Also, the frequency of seizures is higher among babies with low birthweight or retarded intrauterine growth while the relationship between seizures and gestational age is not clear.
