Aneurysmal cysts of long bones.

The authors reviewed 121 cases of aneurysmal cysts of the long bones treated at the Tumour Centre of the Rizzoli Institute of Orthopaedics of which 103 had been followed up for more than one year. A classification is proposed based on 5 morphological groups and 3 grades of aggression. All except 4 of these lesions were treated by curettage or resection (2 were treated by radiotherapy alone, and 2 only had biopsies). The overall results showed a recurrence rate of 13%, none of which were in the inactive group. There was no prognostically significant difference in the recurrence rate between the active and aggressive groups. In the morphological groups there were significant prognostic differences. The authors therefore advise a programme of treatment based on an assessment of the morphological type and aggressiveness of these tumours.

Prognostic significance of serum lactic acid dehydrogenase in Ewing's tumor of bone.

Pretreatment serum lactic acid dehydrogenase (LDH) levels were examined in 82 patients with Ewing's tumor of bone (23 metastatic and 59 localized). The percentage of patients with increased serum LDH levels was significantly higher in the metastatic group than in the group of patients with localized disease (87 vs. 45%). After treatment with combined therapy, in this latter group the recurrence rate was significantly higher in patients with elevated serum LDH levels at admission than in those with normal serum LDH levels (50 vs. 9%). Of the 16 patients with localized disease who relapsed, 13 (81%) had elevated serum LDH levels at the time of recurrence. These results indicate that in Ewing's tumor serum LDH levels may serve as useful prognostic markers and could be valuable in monitoring the effectiveness of combined therapy.

The Gamma nail, sliding-compression plate. A comparison between the long-term results obtained in two similar series.

The authors report the results they obtained in a retrospective comparative study conducted on two systems of osteosynthesis used to treat trochanteric fractures: the sliding-compression screw-plate (CHS), and the Gamma nail. Two series were included in the study, each of which was made up of 50 cases; these were comparable in terms of fracture type, age of the patients, associated pathologies, and minimum follow-ups of 6 months. The parameters compared were: surgical trauma, average amount of time before a standing position could be resumed, resumption of motor activity previous to trauma, mechanical behavior of the instrumentation. The results show that both of the means of synthesis are capable of effectively stabilizing pertrochanteric fractures, avoiding the mechanical complications instead observed when more dated instrumentation, such as the screw-plate or nail-plate, are used, systems which are not characterized by a sliding cervico-cephalic screw. The amount of time before walking is resumed is quicker when the Gamma nail is used. There was no significant difference in terms of resumption of motor activity. Surgical trauma was the same in both series when the instrumentation was applied after reduction of the fracture in closed surgery, while blood loss was greater when fracture reduction required by application of the plate was performed in open surgery.

The Lefevre ulnar nail.

The Lefevre nail applies the principles of locked intramedullary nailing in the treatment of fractures of the ulna. It is made up of two segments which slide telescopically, so that assembly may be in compression, distraction or neutral. Fixation at the two ends of the ulna is guaranteed by two transverse screws. It may be used in pseudarthrosis and osteotomies for lengthening and shortening, as well as in simple or complex fractures. The cases presented provide an overview of the use of the nail at the Brest Center, where the nail was invented and developed, and at the Rizzoli Institute. The study includes 22 fractures, 1 pseudarthrosis and 1 osteotomy. Consolidation was obtained in all of the cases after an average of 2.5 months. Immobilization in plaster was required only in cases with associated lesion of radius. The functional results obtained were satisfactory.

[The adverse effects induced by ornipressin during oral and maxillofacial surgical interventions under general anesthesia].

There are various methods used in maxillofacial surgery for the control of superficial hemorrhage. The purpose is to achieve a better identification of the anatomical structures and reduce operating times without side effects. Ornipressin has no arrhythmogenic effects in presence of inhalational anesthetics and should therefore be considered safer then adrenalin. The authors examined the cardiocirculatory effects of this drug by means of an invasive hemodynamic study on five patients. The results showed that ornipressin causes worrying increases in blood pressure and peripheral vascular resistances, with reduction in the cardiac output.

The Gamma nail. A preliminary note.

The Gamma nail is a new device used in the treatment of pertrochanteric fractures in the elderly in which the principles of locked intramedullary nailing are applied. It is constituted by an intramedullary nail crossed in the proximal segment by a cervico-cephalic screw, by which compression of the fracture segments may be obtained. It is furthermore possible to obtain distal locking by means of two screws, such as for Grosse-Kempf nailing. It constitutes an alternative to Ender nailing, screw-plate, and nail-plate. Our experience with 31 cases did not reveal any differences as compared to what occurs with Ender nailing with regard to surgical stress. Instead, a shorter amount of time seems to be required for functional recovery, as the stability of the assembly allows for early weight-bearing.

Myelin basic protein in the CSF of patients with multiple sclerosis, optic neuritis and various neurological diseases.

We tested 104 patients for myelin basic protein (MBP) content in the CSF. Of these subjects 14 were selected as control group, 36 were affected by multiple sclerosis (MS), 14 by optic neuritis (ON) and 42 presented other not primarily demyelinating neurological diseases (ND ND). CSF MBP level was significantly higher in the MS group than in the other groups of patients, while no statistical difference was found between the MS patients with acute exacerbation and those in remission.

[Isolated systolic hypertension in the elderly].

Isolated systolic hypertension (ISH) is a very common condition in the elderly, even more than diastolic hypertension. Several studies indicate that ISH is a considerable cardiovascular risk factor in the elderly but the benefit of antihypertensive treatment has not been proved. Data in the literature on prevalence, pathophysiology, prognostic meaning and treatment of ISH are critically reviewed.
