[Diagnostic problems in a case of pyostomatitis vegetans].

After a clinical and histo-pathological introduction, the paper presents a case of Piostomatitis Vegetans, a rare stomatitis coexisting with chronic inflammatory disease of the intestinal tract, that was treated with local corticosteroids.

[Traumatic pseudocyst of the lung].

A rare complication of closed chest injury is the traumatic pulmonary pseudocyst. A well documented case of a 15-year-old boy with this definite, acutely formed, primary structural manifestation within the lung is reported with a short review of nomenclature, pathogenesis, diagnosis and treatment.

DSM-III diagnoses associated with dyspepsia of unknown cause.

The frequencies of DSM-III diagnoses in 30 patients with dyspepsia of unknown origin and 20 patients with organic dyspepsia were compared. Dyspepsia of unknown origin was associated with a higher prevalence of psychiatric diagnoses (86.7% versus 25.0%), particularly anxiety disturbances (66.7%).

Seminal plasma peptides inhibiting RNA synthesis.

A peptide fraction, called seminal plasma inhibitor (SPI), present in mammalian semen, was shown to block DNA transcription in vitro (Lugaro et al., 1984). Peptides responsible for this effect were partially purified from bovine seminal plasma. This report outlines the preparative procedure for the isolation of SPI and provides preliminary information on the action mechanism of the RNA synthesis inhibition.

Association of ulcerative colitis, sclerosing cholangitis and cholangiocarcinoma in a patient with IgA deficiency.

A case of an unusual association of ulcerative colitis, sclerosing cholangitis, cholangiocarcinoma and IgA deficiency in a 22-year-old man is reported. The patient was admitted to the hospital with jaundice. Ulcerative colitis had been diagnosed 4 years before. The association of sclerosing cholangitis and cholangiocarcinoma was demonstrated. In addition IgA deficiency was discovered. Possible pathogenetic implications are discussed.

Adenoma, Bile Duct↗

Bovine seminal plasma contains a low-molecular-weight factor that inhibits RNA synthesis.

A low-MW factor (800-1000 daltons) extracted from bovine seminal plasma (bSP) and partially purified by a five-step fractionation is very active in inhibiting RNA synthesis by E. coli RNA polymerase with calf thymus DNA as template (70% inhibition at factor:DNA ratio of about 1:100). The same factor also inhibits RNA synthesis in isolated liver nuclei but to a lesser extent. The bSP factor probably exerts its inhibitory activity on initiation rather than on the elongation processes. DNA heat denaturation experiments indicate that the factor stabilizes double-stranded DNA. The activity of bSP factor is almost destroyed by protease (pronase) digestion. Trypsin digestion is ineffective. Consequently, peptide integrity seems to be important for the biological activity. The factor is heat stable and does not contain nucleic acid components. Preliminary analysis indicates the presence of acidic amino acids with no basic or aromatic ones and that the active factor is not a product of histone or protamine degradation. When injected i.p. into 25-day-old female rats, the bSP factor has an inhibinlike activity.

Radial artery catheterization by surgical exposure in infants.

56 radial arteries were cannulated by cutdown technique in 54 critically ill infants (age (mean +/- SD) 12.2 +/- 25.9 days, weight 2710 +/- 910 g) for continuous pressure monitoring and repeated blood sampling. 30 patients survived their illness, 24 died. The mean catheterization time in the survivors was 8.1 +/- 4.1 in the non-survivors 6.9 +/- 5.6 days. Most cannulae were removed electively. 27 of the surviving infants were followed up after 10.5 +/- 5.1 months. 25 (86%) vessels were patent, 4 (14%) occluded. No trophic changes of hands or fingers were noted. We concluded that radial artery cannulation by cutdown in infants is a quick and safe technique and is suitable in patients with severe peripheral vasoconstriction.

[Blood folates in chronic uremic patients in dialysis treatment].

The authors, after having reviewed folic acid and folates chemical and biochemical characteristics and their absorption and excretion modalities, determine serum folate levels by radioassay method on 30 patients with renal failure in chronic dialysis and on 24 normal controls. Low serum folate levels concerned 14 uremic patients (46.6% of th cases): 10 in hemodialysis and 4 in peritoneal dialysis. The mean serum folate values was 3.31 ng/ml (+/- 1.93) for the uremic patients' group and 4.29 ng/ml (+/- 1.21) for the control group. The difference between the mean level of these groups was statistically significant (p less than 0.05). No significant difference was observed among the mean serum folate levels of the uremic patients in peritoneal dialysis and of those in hemodialysis. Significantly low serum folate levels were finally found for the uremic HBsAg-positive patients, but in the same subjects the dialytic treatment period had been very prolonged. The authors conclude emphasizing the usefulness of folic acid treatment in uremic patients on dialysis, also without having an evident hematologic picture of megaloblastic anemia.
