[Intramural gravidity - case report and account of literature (author's transl)].

Reference is made to a report on intramural pregnancy, the twelfth case so far described in accessible literature, and the authors' own views on the possible origin of this extremely rare location of pregnancy are added to other assumptions so far made on the pathogenesis. The hypothetical proposition made by the author's is based on the assumption of transperitoneal penetration of the uterine wall by the spermovium via a focus of endometriosis. Described are case histories, symptoms, tentative diagnoses, and surgical approach in the cases so far observed.

[Abdominal reposition of puerperally inverted uterus (author's transl)].

Inversion of uterus in puerperium is an extremely rare case and always accompanied by severe shock. To the obstetrician such event is almost always unexpected and calls for immediate decision and action. Dry manual reposition usually is impossible for tough inversion pressure, while severe disadvantages are implied in vaginal surgery with incision of the anterior and posterior walls of the uterus. Therefore, an abdominal method is recommended by the author, leaning on Huntington's technique. First, the shock is brought under control. Then the inverted uterus is levered out of the inversion funnel to complete reversion, step by step, with a pair of bullet forceps inserted after each step, four altogether. The uterus of two cases was completely reversed by means of that simple method in 30 minutes only. Even the most careful and vigilant obstetrician may find himself trapped in such rare situation of spontaneous uterus inversion due to the constitution of a particular patient. The procedure suggested in this paper, therefore, may be an approach practicable even in smaller and less equipped hospitals, when it comes to the need for immediate remedial action to cope with that most dangerous complication.

[Rifamycin chromopertubation under laparoscopic control (author's transl)].

There is an inherent danger of infection or promoting the exacerbation of an as yet incompletely healed inflammatory process when retrograde perfusion of the Fallopian tubes is performed, as in the diagnosis of sterility by laparoscopy and chromopertubation. Hence, the intense-red tuberculostatic drug, rifamycin SV (Rifocin) was instilled by means of the Schultz Instillator, either alone or in combination with indigo carmine, in place of the conventional indicator solutions, to test tubal patency in all patients under investigation for sterility, whether there was a history of acute, subacute or chronic salpingitis or not. This procedure was not followed by exacerbation of preceeding infection in a single case, which is particularly noteworthy in two patients shown to have extensive tubal and peritoneal tuberculosis.

[Possibilities to influence the duration of labor through decompression treatment during the third trimester].

Comparative examinations of labour duration in primipara and multipara of the age groups: before 20, between 20 and 25, 25 and 30, and over 30 (so called "old primipara") were performed. Equal numbers of women with and without abdominal decompression treatment during the III. trimenon were analysed, altogether 1312 patients. In the groups before 20 and between 20 and 25 years of age only an insignificant decrease in duration in treated cases against the control group was found objectively, despite the almost subjectively reported easing and shortening of labour.--On the other hand in 30 year old patients a clear and in over 30 year old women (so called "old primipara") a highly significant decrease was recognizable, which demonstrates the value of prenatal preparations by abdominal decompression treatment in these age group.

Immunoglobulin administration to fetuses with anemia due to alloimmunization to D.

BACKGROUND: The purpose of this study was to examine fetal tolerance of high-dose intravenous immunoglobulin (IVIG), given directly at the time of intravascular transfusion, and its effects on fetal hemolysis and pregnancy outcome in the setting of alloimmunization to D. STUDY DESIGN AND METHODS: Thirteen consecutive D+ fetuses requiring transfusion for maternal alloimmunization received high-dose IVIG (1.0 g/kg) and red cell transfusions. Twenty-four previous, consecutive fetuses with maternal anti-D served as controls. The schedules for subsequent transfusions were the same in the two groups. RESULTS: High-dose IVIG was well tolerated by all fetuses. In the IVIG group, daily decreases in hematocrit were smaller than those in controls after the second administration of IVIG (mean hematocrit decrease, 0.72 percent/day vs. 1.45 percent/day; p = 0.007). No significant difference was found in the total number of fetal transfusions, the gestational age at delivery, the duration of neonatal intensive care, the number of neonates requiring postnatal transfusion therapy, and perinatal mortality. CONCLUSION: In this small pilot study, direct administration to fetuses of IVIG with red cell transfusions was well tolerated and appeared to have a beneficial effect on fetal hemolysis.

Twenty-four cordocenteses in one woman.

We report on 2 consecutive pregnancies in a woman with a history of neonatal death secondary to Rhesus alloimmunization. Her first subsequent pregnancy was complicated by fetal hydrops at 20 weeks of gestation. The fetus received a total of 11 intrauterine transfusions, and was delivered at 38 weeks. In the patient's next pregnancy, the fetus developed hydrops at 18 weeks of gestation. Thirteen intrauterine transfusions were given to correct fetal anemia, and a healthy baby was delivered at 38 weeks of gestational age. Continuation of intravascular transfusion therapy may represent a reasonable alternative to selective premature delivery even in cases with highly aggressive maternal Rhesus alloimmunization.

[Vaginal sonographic flow measurements in the uterine artery in pregnancy--advantages of the pulsed Doppler method].

Uterine artery velocimetry was performed transvaginally in 42 healthy pregnant women by means of a newly developed "panorama" sector scanner which provides a real time image of the vessel in combination with the recording of a pulsed and gated Doppler signal. Measurements were made on both uterine arteries and averaged. A/B ratios and pulsatility indices declined as pregnancy advanced. A/B ratios I. Trimenon vs. III. Trimenon = 5.44 vs. 2.20, PI = 2.59 vs. 1.32. These changes may be secondary to the conversion of spiral arteries into dilated tortuous vessels by the trophoblast invasion. Flow velocity measurements of the proximal uterine arteries provide a summery of the blood supply to all the arcuate arteries. This method will enhance our understanding of adequate placentation and may provide a new tool in the diagnosis and monitoring of complicated pregnancies.

[Fetal megacystis--prenatal diagnosis and attempt at therapy].

Dilatations of the fetal urinary tract are suspect for obstructions. An extremely dilated bladder may also point to an innervation disorder. In the here presented case a very rare disease is the cause for the dilatation of the fetal bladder, the Megacystis-Microcolon-Hypoperistalsis-Syndrome. No mature ganglionic cells are detectable in the bladder wall histologically. Up to now eight cases have been reported, all female neonates died within the first days after birth. Our presented child - also a female - is now more than one year old, but artificial feeding is still necessary.

[Prenatal diagnosis of the sequelae of supravesical stenoses--hydronephrosis and hydroureter].

In the introduction, the extrarenal and intrarenal pelvis are distinguished and morphologically recognizable reasons for the dilatation of the renal pelvis and the ureter are shown. With examples from twelve corresponding cases from an a.p. diameter of 10 mm upwards, contributions to the following problems are offered: Time of earliest possible prenatal diagnosis, relation between dilatation of the pyelon and thickness of the parenchyma, on the one hand, and the mean value of the abdominal diameter, on the other hand. In addition to the degree of the narrowing of the lumen the importance of the secretal function of the kidneys which begins in the course of the development is discussed.

[Prenatal differential diagnosis of abdominal and umbilical cord hernias].

At the beginning a systematic presentation of congenital herniations is provided. Corresponding to it, typical sonographic evidences from the number of patients examined at the clinic are shown and discussed. Consequences from prenatal diagnosis of omphaloceles and paromphaloceles--namely in regard to an indication for abortion--result from the existence of associated malformations on the one hand, for the choice of the mode of delivery, on the other, with regard to a postnatal operative correction.

[Epiphyseal centers in the knee area - a sonographic and radiologic comparison].

In 26 cases we checked the possibility of representation and measurability of the lower femur- and upper tibia-epiphysis by ultrasound and compared it to corresponding X-ray investigations. No remarkable differences were seen between the results achieved by ultrasound or X-ray investigations concerning the size of the femur- and tibia-epiphysis on term. In the contrary we found differences in size of the epiphysis measured by ultrasound or X-ray before the 38th week of gestation. In some cases it was only possible to represent them by ultrasound alone. The reason for this phenomenon will be argued in the discussion.

[Management of breech delivery in primiparas - on the incidence of cerebral lesions. (author's transl)].

The investigation comprises a group of infants of primiparae with breech presentation after caesarean section and a group of infants delivered by means of manual aid. The school age children were examined on the incidence of brain injury. The investigation consisted of four parts; anamnesis, neurological status, EEG, psychological examination. Between these two groups there is not statistically-significant difference in the incidence of brain injury. The result of the investigation advocates the maintenance of selective sectioindication in case of primiparae and points to a possibly existing brain injury of infants with breech presentation before delivery.
