[Stress measurements with an instrumented internal spinal fixator].

Little is known about the loads acting on internal spinal fixation devices. The effective forces and moments associated with different external loads have now been measured using instrumented implants. Measurements were obtained in intact cadaver spines and following vertebrectomy. The influence of the implant itself on the distribution of the loads was also investigated. It was found that the implants are not loaded symmetrically. Following vertebrectomy, the forces acting on the fixation device are significantly higher than in the case of an intact spine.

[Theory and model in psychosomatic medicine--a historical sketch].

Starting with "personal medicine" as it was formed by Ludolf von Krehl in the frame of a medical clinic at a university, psychosomatic medicine, in its narrow sense, is described in its historical development. Differences to the conception of behavioral medicine are noted. Some theoretical models following the conversion model of Freud like stress concept, life-event or coping are historically explained. Actual research such as change of paradigm towards a systemic approach describes the actual situation, the conception of the "Gestaltkreis" completes this. Practice of today's psychosomatic medicine can be outlined by the concepts of simultaneous diagnostic and simultaneous therapy.

Four-channel telemetry system for in vivo measurement of hip joint forces.

The long-term loosening of artificial hip joints remains a serious clinical problem. Optimization of implant design and material will improve the fixation, but it requires a detailed knowledge of the forces which act on the implant. A four-channel telemetric transmitter was developed and arranged completely inside a hermetically closed artificial hip joint. This permits long term in vivo measurements of the three-dimensional forces without endangering the patient. The external telemetry system consists of an inductive power supply, an RF receiver, a microcomputer with hardware extension and a VHS video system. The personal computer offers real-time data processing of three orthogonal force components as well as slow motion analysis of recorded measurements. After several years of animal tests, two instrumented prostheses were implanted in the first patient (male) in May and August of 1988. In March 1990 a third prosthesis was implanted in a second patient (female). Joint force measurements have regularly been performed from the first post-operative day until now for several kinds of activity.

The quantitative spectrum of inositol phosphate metabolites in avian erythrocytes, analysed by proton n.m.r. and h.p.l.c. with direct isomer detection.

The spectrum of inositol phosphate isomers present in avian erythrocytes was investigated in qualitative and quantitative terms. Inositol phosphates were isolated in micromolar quantities from turkey blood by anion-exchange chromatography on Q-Sepharose and subjected to proton n.m.r. and h.p.l.c. analysis. We employed a h.p.l.c. technique with a novel, recently described complexometric post-column detection system, called 'metal-dye detection' [Mayr (1988) Biochem. J. 254, 585-591], which enabled us to identify non-radioactively labelled inositol phosphate isomers and to determine their masses. The results indicate that avian erythrocytes contain the same inositol phosphate isomers as mammalian cells. Denoted by the 'lowest-locant rule' [NC-IUB Recommendations (1988) Biochem. J. 258, 1-2] irrespective of true enantiomerism, these are Ins(1,4)P2, Ins(1,6)P2, Ins(1,3,4)P3, Ins(1,4,5)P3, Ins(1,3,4,5)P4, Ins(1,3,4,6)P4, Ins(1,4,5,6)P4, Ins(1,3,4,5,6)P5, and InsP6. Furthermore, we identified two inositol trisphosphate isomers hitherto not described for mammalian cells, namely Ins(1,5,6)P3 and Ins(2,4,5)P3. The possible position of these two isomers in inositol phosphate metabolism and implications resulting from absolute abundances of inositol phosphates are discussed.

[Anxiety, the heart patient and the organization of psychosomatic medicine].

Using a simplified biopsychosocial model, it is possible to outline several aspects of the partly real, partly unreal anxieties of the patient with heart disease, also in connection with defense strategies. Questions regarding the formal introduction of psychosomatic medicine are raised exemplary findings in the area of heart transplants, resuscitation and denial aspects are mentioned. The medical-care system and the patient complement each other in perception and strategic handling of anxiety, partly in stabilized, partly in destabilized ways. The development of a future organisation of psychosomatic medicine must differentiate between consultant services, liaison services, working groups and independent departments. As in the analytical-therapeutical situation, the understanding of the function of defense strategies is the prerequisite to overcome the symptom, which - experienced or not experienced, expressed or not expressed - means anxiety both for patient as well as for those who work in institutions.
