[The importance of risk factors in cerebrovascular processes while taking oral contraceptives (author's transl)].

21 young female and 15 young male patients with cerebrovascular insults were examined for risk factors. 14 of the 15 male patients showed clear cut risk factors: obesity, diabetes, hyperlipidemia, arterial hypertension, smoking, thromboses, vitium cordis. 20 of the 21 female patients took oral contraceptives. 60% of the female patients with angiographically confirmed stenoses and occlusion did not show any other risk factor. These results support the hypothesis that oral contraceptives are in themselves a risk factor.

[Silent pyloroduodenal gastrinomas. Apropos of 3 cases].

The authors report three anatomoclinical studies concerning apparently moderately aggressive endocrine tumors developped in the sub-mucosa of the duodenal bulb (2 cases) and of the pyloro-bulbar region (1 case), not connected with the pancreas, and occuring in the absence of a parietal ectopic pancreas. Two of these tumors were ulcerous but the associated syndromes (pains and hemorrhage) did not evoke, clinically, a Zollinger-Ellison syndrome. However, examination with immunofluorescence showed the presence of immunoreactive gastrine in a large number of cells. Having found some similar cases in the literature, the authors precise the features of these silent pyloro-duodenal gastrinomas: it is a variety of carcinoids or of "carcinoid-islet cell tumors". The cells contain argyrophile granules (Grimelius stain) which correspond in electron microscopy to neuro-secretory granules that may be quite different from G. granules. In the absence of significant clinical signs and of a radio-immunologic blood test, the presence of immunoreactive gastrin in the cells is the main feature for the diagnostic of these tumors. These tumors seem to arise from the gastrin cells of the mucosa or from their precursors as is suggested by the transitional forms with the fundus of the glands. Whatever the reason of the peculiar functional behavior, these neoplasms can be easily isolated from the anonymous group of duodenal carcinoids.

[Unilateral aplasia and hypoplasia of pulmonary artery in childhood (author's transl)].

In the light of 8 observations, made by the authors, vasoanomalies underlying the syndrome of the unilateral hyperlucent lung are discussed in their correlation to homolateral bronchial lesions. There were 3 cases with unilateral aplasia of the pulmonary artery in which the unilateral hyperlucency was found always on the healthy side. In these cases there was a normal bronchogram on both sides. The vascularization by the bronchial arteries causes characteristic changes both in the X-ray-picture and endoscopy. Hypoplasia of the pulmonary artery occurring simultaneously with hyperlucency of the affected side of the lung was found in five cases, 4 times in connection with homolateral, severe, diffuse deforming bronchitis and bronchiectases and 1 case with congenital bronchus stenosis. There is the opinion that besides a congenital form of the hypoplasia of the pulmonary artery a (virus?) --pneumonia, acquired in early childhood, can lead to a pulmonary hypoplasia and a bronchopulmonary dysplasia. The diagnosis can be made only after several special examinations performed in a multi-step system. The significance of the lung scintigraphy within the scope of this complex diagnostics is especially emphasized. Indication for surgery should be made very cautiously. Antibiotic long-term therapy and dsipensary care are considered to be essential.

Nicotine-11C: synthesis and distribution kinetics in animals.

In order to study "in vivo" the distribution kinetics of (-)-nicotine in animals, this molecule was labeled with carbon-11. Two synthesis methods are described. Immediately after intravenous administration of (-)-nicotine-methyl-11C in rabbits the gamma-camera shows a strong radioactivity build-up in the brain and kidneys. The biological interest of this carbon-11 labeled molecule is discussed.

[Identical psychosis of a monocygous twin set--a case report (author's transl)].

This is a report about a pair of identical twins with identical symptoms of a similar to schizophrenic psychosis. The course of the disease with special reference to the influence by each other and a similar and a dissimilar treatment is described. This disease is subgrouped to the symbiontic psychoses. Nosology, aetiology and psychodynamic aspects are discussed.

[Pathogenesis of osteopathy caused by anticonvulsives (author's transl)].

After a short survey of the recent suggestions of the pathogenesis of osteopathy caused by therapy with anticonvulsives it is demonstrated in the light of 3 case reports that 1. A vitamin D-sensible rickets, which is only due to abnormal living conditions, can develop in severely cerebral-damaged persons if there is no specific vitamin D administration. 2. An "antiepileptic rickets" is spontaneously reversible if the anticonvulsive therapy is stopped. 3. An enzymatic induction due to anticonvulsives is also detectable in the light of morphologic hepatic changes (biopsy, electron microscopy). Hence it follows that there are demands for a specific control of all patients treated with anticonvulsives. It is recommended to carry out a rickets prevention for a selected patient group.
