In exceptional cases, acromegaly develops as the clinical expression of an ectopic secretion of Growth Hormone (GH) or Growth Hormone-Releasing Factor (GRF), tumorous in origin. In the present report, we describe an instance of acromegaly caused by the secretion of GRF from a voluminous pancreatic tumor. The resection of this tumor resulted in a temporary disappearance of the biological and clinical symptoms of acromegaly, which then reappeared in conjunction with a rise in plasma GRF. From this pancreatic tumor, substances displaying a potent GRF activity were isolated and characterized. Amino acid analyses revealed that they were related to 3 peptides containing respectively 44, 40 and 37 aminoacids. The largest (hp GRF (1-44)-NH2) referred as hp GRF or somatocrinin is considered to be the primary molecule. The pancreatic tumor was multisecreting as proved by high plasma levels of somatostatin, pancreatic polypeptide and glucagon, normalized after the tumor removal, taken together with the immunocytochemical demonstration of the presence of these peptides in the tissue and with the isolation of somatostatin. In contrast hypercalcemia associated with an elevated plasma level of IR-PTH was unmodified by tumor removal. Diagnosis of acromegaly as ectopic endocrine syndrome will probably be facilitated by plasma GRF radioimmunoassay, as a result of production of anti synthetic GRF antibodies.

The fact that myringoplasty in children is relatively less satisfactory than in adults, motivated us to perform a retrospective study, gathering material on 53 children aged 4-12 years who underwent 68 operations to determine if any pre-operative or intra-operative parameters might predict the outcome of surgery. Results show that the overall success rate of Type I tympanoplasty was 78%, and that the majority of failures occurred later in the follow-up course. Furthermore, age and technique of operations played the most important roles in the results. The status of the non-operated ear did not influence the surgical outcome. Finally, revision myringoplasty was equally as successful as the primary operations.

The authors report two cases of mucoargyrophilic carcinomas of the breast, occurring in 73 and 81 year old women. Both tumors consist of a central mucoid carcinoma surrounded by solid areas with lobular or endocrinoid features. Most cells showed an abundant eosinophilic granular cytoplasm or a mucinous appearance. The histochemical reactions (Grimelius-Alcian Blue, Grimelius-Mucicarmine) and the ultrastructural study of the peripheral areas disclosed several cell types: poorly differentiated or mucinous cells were present in both cases; but most of the cells were argyrophilic and contained in their hyaloplasm numerous dense core "neurosecretory" granules; such cells revealed many microfilaments arranged in bundles or in whorls. Amphicrine cells were easily demonstrated by histochemical staining and their cytoplasm showed both endocrine granules and mucinous vesicles. These features suggest a possible relationship to other argyrophilic tumors of the breast. The histogenesis of these tumors is still little known.
We present our experience with the use of estradiol - bovine serum albumin (BSA) - fluorescein isothiocyanate (FITC) employed by LEE to localize estradiol receptors on frozen sections of breast carcinomas. 41 cases were studied on parallel sections with both conventional staining and a histochemical procedure. Heterogeneity of the cell population was usually found; however the histochemical result was influenced by morphological parameters. As far as histopathological types were concerned, invasive lobular carcinomas (3 cases) manifested a bright cytoplasmic fluorescence, while medullary carcinomas (3 cases) were poorly or non stained. Among infiltrating ductal carcinomas, well differentiated cases were composed of strongly fluorescent tubules, while poorly differentiated ones classified in BLOOM'S grade III were poorly stained. In most of the cases, the amount of stroma, mucus and carcinomatous or hyperplastic ductal components has to be taken in account to evaluate the fluorescence pattern of each microscopic field.

Chemical and structural surface changes at enosseous bioglass-ceramic-implants (Ap 40) were investigated by means of electron microprobe after an implantation period of 4 to 52 weeks. An intimate bonding between regenerated bone and bioglass-ceramic surface was evident at points already after 4 weeks. Initial degradation processes at implant surface came to a standstill after 12th week. A calcium and phosphorus rich layer of great importance for implant stability and biocompatibility was formed immediately contacting the bone. This surface layer had a thickness of nearly 25 micrometers -- 30 micrometers at 12 weeks and remains unchanged up to 52 weeks. The registered calcium and phosphorus impulse rate (cps) is equal to that of neighbouring bone. Chemical similarity of implant surface makes bioglass-ceramic Ap 40 a suitable biomaterial for durable bone replacement.

The brain uptake of 11C-methionine was studied in 26 children with classical phenylketonuria; one adult was used as a control. Labelled methionine uptake in brain was first measured during a low phenylalanine diet and again one week later after a load of phenylalanine. Ten children aged 1 to 30 months were studied twice at intervals of several months. In children having a phenylalaninemia less than or equal to 0.3 mumoles . ml-1, a decrease in methionine brain uptake was observed with increasing age, with the largest change occurring during the first year of life. After the phenylalanine load, a mean increase in phenylalaninemia by a factor of ten was accompanied by a mean decrease in brain methionine uptake by a factor of two while blood methionine remained unchanged. Brain activity curves increased with time for children younger than one year and having phenylalaninemia less than 0.6 mumoles . ml-1. After the age of 2 most patients had a decreasing curve regardless of the blood phenylalanine level. This study indicates that 11C-methionine brain uptake may be taken as an index of blood barrier permeability to essential amino acids, and of brain maturation. The results obtained suggest that an increase in phenylalaninemia to levels greater than 0.6 mumole . ml-1 induces a modification in brain uptake of amino acids, primarily during the first two years of life.
