Failure of endogenous prolactin to alter renal salt and water excretion and adrenal function in man.

Prolactin is a salt-retaining hormone in fish, and a similar role is frequently postulated in man, althrough clincial observation does not generally support this. The effect of TRH induced acute elevation of endogenous serum prolactin on renal water, sodium, potassium and total solute excretion was investigated during metabolic balance conditions and during escape form mineralocorticoid excess in 8 normal volunteers (6 males, 2 females)...

Lack of in vivo transformation of human growth hormone to its "activated" isohormones in peripheral tissues of the rhesus monkey.

Human growth hormone (hGH) B can be transformed to its biologically more active isohormones C, D and E by limited proteolysis with plasmin in vitro, but the physiological significance of this observation is not known. The possible occurrence of such an "activation process" in peripheral tissues was investigated in the rhesus monkey. Radioiodinated hGH (B, C) or isolated hGH-B were injected intravenously into 4 rhesus monkeys and blood samples were drawn at frequent intervals. Analysis of circulating hGH was carried out by precipitation of hGH with trichloroacetic acid (TCA) and excess anti-hGH antibody, and by polyacrylamide gel electrophoresis followed by autoradiography. The metabolic clearance rate, based on TCA-precipitable radioactivity, was 2.36 +/- 1.17 ml/min/kg (mean +/- SD), similar to that reported for man. No evidence for the presence of circulating hGH-D or E was found, although progressive accumulation of smaller radioactive fragments and free iodide was observed. It is concluded that peripheral tissues or plasma enzymes probably do not convert significant amounts of hGH-B or C to their more active charge isomers, at least in the monkey. Therefore, if enzymatic activation of hGH does occur in vivo, the pituitary gland would seem the most likely site for such a process.

[Infusion- and microcatheterization technics at an internal medicine intensive care unit].

It is reported on the infusion, stimulation and microcatheter technique in an internal intensive therapy ward. From July 1st 1970 to November 11th 1974 were altogether laid 1,642 superior cavacatheters, 1,057 times by puncture of the subclavian vein and 585 times through the brachial veins. Compared with the usual infusion techniques the advantages of the cava-catheter technique consist in the fact that a constant venous approach is existing and that solutions of a high percentage and medicaments may be infused relatively undangerously. Apart from this the cava-catheter gives the possibility to carry out continuous measurements of the central venous pressure and guarantees the taking of blood samples at any time. The phlebitides or thromboses, respectively, which are regularly appearing in flexules lying longer than 40 hours are avoided in most cases. Taking into consideration the contraindications and the exact performance of the various methods especially early complications such as puncture of the arteries, pneumothorax, detachment of the catheter, extended haematomas and wrong positions of the catheter may be reduced to a minimum. As late complications distinct infections in the place of puncture, unclear, partly septic temperatures and clinically manifest thromboses were observed. In these cases the cava-catheters should be removed. The temporary electrostimulation through electrode catheters plays an important part in the emergency medicine and the mastering of the various methods of the cava-catherization is taken for granted. During the period of report altogether 423 stimulation catheters were laid. The cardiac microcatheterization should be reserved for more special cases.

Pregnancy performance of patients under fifteen years of age.

Pregnancy performance of 471 patients under age 15 who delivered between 1968 and 1972 was compared with that of the same number of control patients between 19 and 25 years of age. The control group was selected by the paired method technic, whereby each study patient was paired with a control patients utilizing identical race and contribution to hospital cost as the economic determinant. The results of the study showed that young patients differed significantly from the control group in having an earlier menarche and a greater number of recurring pregnancies within 18 months of the initial one. Pregnancy-induced hypertension and pelvic inlet contraction were pregnancy complications occurring more frequently among study patients than control patients.

Free cortisol index: a rapid and simple estimation of free cortisol in human plasma.

A rapid and simple method for the estimation of free (nonprotein-bound) cortisol in human plasma is described. Total plasma cortisol (F) was determined by a competitive protein binding assay. An estimate of the saturation of Corticosteroid Binding Globulin was obtained by the use of "charcoal cortisol uptake" (CFU). "Charcoal cortisol uptake ratio" (CFUR) was defined as the ratio of the CFU of the unknown plasma to that of a reference serum pool. Free cortisol index (FFI) was calculated as (total plasma F) times (CFUR). Percent free cortisol and free cortisol concentration (FF) was determined by equilibrium dialysis at 37 C. 169 plasma samples with an F range of 0.8-61.2 mug/100 ml from 18 normal, 10 ACTH-stimulated, 17 estrogen-treated and 12 pregnant subjects were analyzed. A highly significant correlation between FFI and FF was found (r = 0.952). Inter-assay coefficients of variation were 7% for total plasma F and 3.5% for CFU. The method allows the rapid processing of large numbers of samples and can be performed on 0.2 ml of plasma.

Serum thyroxine and thyroxine-binding proteins in chronic renal failure without nephrosis.

Total serum thyroxine (T4), free thyroxine index (FTI), thyroxine binding globulin (TBG) binding capacity, serum albumin, alpha-globulins and urinary protein excretion were measured in 50 patients with chronic renal failure, but without nephrotic syndrome. 25 patients were undergoing chronic hemodialysis. T4 was within the normal range in most patients. There was a tendency to lower T4 values as compared to an age and sex-matched control group, but this did not reach statistical significance. TBG was normal in most patients. 4 patients showed elevated TBG concomitant with elevation of other alpha-globulins. Serum albumin was significantly decreased. No correlation existed between daily protein excretion and TBG or alpha-globulins, but the correlation between serum albumin and proteinuria was highly significant. T4 and proteinuria correlated with borderline significance. A highly significant correlation between T4 and TBG-albumin values was found. No correlation existed between FTI and TBG-albumin levels. The data suggest that T4 and TBG are normal in most patients with renal failure, even in the presence of significant proteinuria. Low T4 values, when found in renal insufficiency, may be secondary to low serum albumin and possibly prealbumin.

[Experimental infection of guinea pigs with Chlamydia. 1. Pathomorphological studies].

The changes that occurred following nasal instillation of a bovine strain of Chlamydia psittaci were characterized by hypertrophy and proliferation of epithelial cells and exudates containing polymorphonuclear leukocytes, with acute bronchopneumonia and less intense inflammatory reactions in nasal and tracheal mucosa, spleen and pulmonary lymph nodes. The intestines, liver, kidney and brain were scarcely affected. There was little prospect of complete recovery from the clinically mild, or even subclinical, pneumonia within the period of observation since the causal agent was present continuously from the 12th hour to the 91st day after infection.

[Experimental infection of guinea pigs with Clamydia. 2. Fluorescent antibody examinations].

The direct fluorescent antibody technique was applied to blood smears and organ impression smears from guinea-pigs killed at various times during 13 weeks following nasal infection with a bovine strain of Chlamydia. Antigen inclusions were demonstrated one hour after infection in blood granulocytes, after 3 hours in most of the organs examined and after 6-12 hours in all the organs. Antigen was present throughout the period of observation in blood smears and samples from nasal mucosa, trachea, lung, small intestine, spleen and liver. It seemed that polymorphonuclear granulocytes played an important part in the pathogenesis of generalized chlamydial infection. In the case of intranasal inoculation, the nasal mucosa was evidently the site of the initial infection and proliferation of the agent, and the origin of primary chlamydaemia. For the initiation and maintenance of the secondary period of blood infection, the lungs and trachea were the principal site of multiplication of the agent; spleen, intestinal mucosa and CNS were also involved to a lesser extent. The antigen content of lung, trachea, nasal mucosa and spleen at each stage of infection was generally correlated with the extent and intensity of the inflammatory reaction.
