Cancer of the nose and paranasal sinuses in the metal industry: a case-control study.

The association between nasal cancer and work in the metal industry was investigated in a case-control study located in the province of Brescia, north eastern Italy. Thirty five cases of malignant epithelial neoplasms of the nasal cavity and paranasal sinuses who were resident in the province of Brescia and diagnosed or treated by the ear, nose, and throat department and the radiotherapy unit (Centro Alte Energie) of the Brescia Hospital in the years 1980-9 were included in the study. Controls (102) were patients affected by benign and malignant neoplasms of the head and neck who were resident in the Brescia Province and matched the cases by age and sex. All the subjects were interviewed by telephone. Metal workers showed an increased risk of nasal cancer (odds ratio (OR) 3.1; 90% confidence interval (90% CI) 0.48-20); a higher risk was associated with work in foundries (OR 5.9; 90 CI 0.77-46). Work in wood, leather, and textile industries was also associated with increased risk of nasal cancer.

[Sentinel events in occupational medicine: the example of Local Health Unit No. 1 of the Tuscany region].

In order to apply the "sentinel health event" methodology we reviewed the certificates of 4541 deaths occurred in the period 1/01/83-31/12/88 in the USL 1 (Lunigiana) in the Tuscany Region. The histories of 6 work accidents, 6 cases of pleural mesothelioma, and 2 cases of sinunasal cancers have been collected by means of personal interviews and investigation at various workplaces. It should be noted that a case of sinunasal cancer was discovered in a farmer who used lead arsenate as an insecticidal. Furthermore the wife of a shipyard worker died of pleural mesothelioma. The death of a shipyard worker caused by pleural mesothelioma, provoked the critical review of the other workers of the same company who were also exposed to asbestos. Moreover, 73 deaths were recorded as due to silicosis; among them, 29 occurred in quartzite quarrymen. In addition, among 153 total cases of lung cancer, 9 were found to be associated with silicosis.

Mortality among pyrite miners with low-level exposure to radon daughters.

Mortality among pyrite miners with low-level exposure to radon daughters. Scand J Work Environ Health 14 (1988) 280-285. A cohort mortality study was conducted with regard to a pyrite mine located in central Italy. Exposure to radon ranged from 0.12 to 0.36 working levels (WL) in the work areas; most measurements were around 0.2 WL. The concentration of free silica in the dust was less than 2%. The cohort was determined from company files and included 1,899 subjects. Mortality was studied for the years 1965-1983. The loss to follow-up was less than 2%. The standardized mortality ratio for all causes and all neoplasms was 97 and 107, respectively. That for lung cancer and for nonmalignant respiratory diseases was 131 (95% confidence interval 97-175) and 173 (95% confidence interval 135-231), respectively. It was estimated that the extra cases of lung cancer attributable to radon daughters numbered 13 per 10(6) person-years and working level month in the whole cohort and 21.3 per 10(6) person-years in the subcohort with 10-25 years of exposure.

A case-referent study on nasal cancer and exposure to wood dust in the province of Siena, Italy.

The aim of the present study was to assess the risk of nasal cancer associated with exposure to dust in the wood and furniture industry in the province of Siena, Italy. Four to seven percent of the active male population is employed in this sector. A case-referent approach was used. The cases included male subjects seen at the Ear, Nose and Throat Clinic and the Radiotherapy Unit of Siena and diagnosed as having cancer of the nasal cavities or paranasal sinuses between 1963 and 1981. The referents were male patients admitted to the Medical Clinic of Siena for all causes except nasal neoplasia; they were matched 5:1 to the cancer patients for age and time of admission. Information was gathered (by postal questionnaires) on the occupational histories of all 36 of the cancer patients and 164 of the 180 referents. The odds ratio associated with exposure to wood dust was 5.4 (1.7-17.2) for all carcinomas, and 87.7 (19.8-407.3) for mucinous adenocarcinoma. The woods used by the exposed cancer patients were mainly oak, chestnut, poplar, and fir. The median duration of exposure was 40 years; no exposed cancer patient reported the presence of exhaust systems in their work environment.

Familial lead poisoning from contaminated wine.

Three members of a family were found be suffering from lead poisoning of nonindustrial origin, the causative agent being the home-produced wine left to ferment in a glazed earthenware vat. The clinical and neurophysiological features are discussed in the light of similar cases in the literature. Correct diagnosis and early treatment of nonindustrial lead polyneuropathy are often extremely difficult because of the elusive nature of the exposure.

[A cohort study of art glass workers in the Empoli area].

The investigation aimed at studying cause-specific mortality of art glass workers employed in 17 industrial facilities in Tuscany, Italy. A cohort of 3390 workers employed for at least 1 year was obtained from company payrolls. Follow-up was between the year each factory started operations, mostly in the mid-fifties, and the end of 1993. The cause specific expected mortality was computed relative to Tuscany rates, specified for gender, 5-year age groups and calendar year. Separate analyses were carried out for the job titles of makers, batch mixers and grinders. For males, 3180 individuals, the observed mortality for cancer causes was above the expected for the lung [standardized mortality ratio (SMR) 123, 10 observed (Obs)], larynx (SMR 166, 10 Obs), stomach (SMR 105, 30 Obs) and brain (SMR 150, 7 Obs). For non-cancer causes observed mortality was above expected for hypertensive diseases (SMR 178, 10 Obs) and diseases of the genitourinary system (SMR 169, 11 Obs). Increases for the above listed causes were shown also among makers. Mortality for larynx and lung cancer increased with time since first exposure and significantly increased SMRs were observed for 21 or more years since first exposure: this pattern was still present with smoking adjustment. The results showed consistently increased mortality for lung and larynx cancer in the overall cohort and among makers. Stomach cancer, brain cancer, hypertensive diseases and diseases of the genitourinary system were also increased in the overall cohort and among makers.

[Role of emphysema in the etiology of functional impairment in patients with severe chronic obstructive pulmonary disease. Study with high resolution computerized tomography].

PURPOSE: To investigate whether high-resolution CT (HRCT) can detect the subjects with massive emphysematous destruction in a group of patients with severe chronic obstructive pulmonary disease (COPD) and therefore be of help in selecting the candidates to surgical lung volume reduction. MATERIAL AND METHODS: We examined 40 former smokers with severe COPD (FEV1, < or = 40% of the predicted value, with no major improvement after inhalation of bronchodilators). Clinico-functional assessment included: a flow/volume loop (mean FEV1 = 28.6% of predicted), arterial blood gas analysis at rest breathing room air (mean values: PaO2 = 65.2 mmHg, PaCO2 = 47.4 mmHg), hematocrit value (mean: 45.2%) and the body mass index (mean value: 23.8). The patients were divided into two groups, namely bronchitic (21) and dyspneic (19) subjects, according to onset symptoms. All the patients underwent HRCT with evaluation of emphysema presence, type, site and extent: centrilobular emphysema was seen in 11 cases, panlobular emphysema in 3 and mixed emphysema in 26 cases; the site was superior in 75%, inferior in 7.5% and diffuse in 17.5% of cases; the mean visual score was 40.8%. The presence, type, site and severity of airways disease were also studied, as well as the pathologic dilatation of the pulmonary artery. The patients were divided into three groups (mild, moderate, severe) according to emphysema extent and they were considered to have bronchial disease in the presence of at least 2 of the 6 signs of bronchial involvement. RESULTS: The emphysema extent score was significantly correlated with the hematocrit value and Tiffeneau index (p < .2) in all the 40 patients. The severity of bronchial obstruction was the same in bronchitic and dyspneic patients. The subjects with chronic bronchitis had milder emphysema (mean extent 35% versus 47% in the dyspneic subjects) and a higher frequency of bronchial involvement. A decrease in FEV1 was significantly correlated with emphysema extent (p < .1) in dyspneic, but not in bronchitic, patients. Moreover, the former had better arterial blood gas and lower hematocrit values. CONCLUSIONS: HRCT is a useful tool in diagnosing the presence of emphysema in vivo and in assessing its extent in COPD patients because it permits to divide the patients into two groups which roughly correspond to the clinical patterns of types A (dyspneic) and B (bronchitic). Severe emphysema patients are the best candidates to surgical lung volume reduction. Airways involvement might play a major role in causing bronchial obstruction in the subjects with mild emphysema. In our series HRCT did assess the severity of emphysema in COPD subjects, but our lung function screening failed to predict emphysema extent.

[Automatic evaluation of total lung capacity and of emphysema involvement with spiral computerized tomography (CT) in obstructive pneumonia].

OBJECTIVE: To evaluate, in patients with chronic obstructive lung disease or chronic bronchitis, inspiratory helical CT with 3D postprocessing, to measure lung volumes and the amount of emphysema and to compare these measurements with lung function tests. MATERIAL AND METHODS: Seventeen patients with chronic obstructive lung disease disease or chronic bronchitis underwent pulmonary function tests and helical CT after a full inspiration with 3D postprocessing (lower threshold -1024 HU, upper thresholds -200, -300 and -400 HU). Lung inspiratory volumes (TLC-CT) were determined for each model; the amount of emphysema was evaluated by means of an automatic score and a visual score with HRCT. RESULTS: There is a good correlation between automatic and visual scores (p < 0.001); the automatic score had a good correlation with lung function tests, above all with total lung capacity (r = -0.56; p = 0.01) but the visual score had a much closer correlation with DLCO (r = -0.70; p < 0.001). TLC-CT had a significant correlation with pletismographic TLC (TLC-P); the upper threshold -200 HU was more correct (TLC-P = 8011 cc.; TLC-CT 200 = 7138 cc.; r = 0.83; p < 0.001). The volume change of 3D model was about 230 cc. per 100 HU (p < 0.001) modifying upper thresholds, but no change was observed in the volume occupied by emphysema and the percentage of emphysema presented minimal, clinically non significant modifications. DISCUSSION AND CONCLUSIONS: In emphysematous patients, the helical CT with 3D model construction is a good technique to evaluate lung volumes and to quantify emphysema with automatic score; this one, however, probably underscores the extent of pathology; therefore, the addition of a visual score with HRCT is probably worthwhile.

Effect of a dentifrice containing triclosan and a copolymer on plaque formation and gingivitis.

A total of one hundred twenty-one (121) adult male and female subjects completed a 6-month, double-blind clinical study comparing the effect on supragingival plaque formation and gingivitis of a dentifrice containing 0.3% Triclosan and 2.0% of a copolymer of methoxyethylene and maleic acid to a placebo dentifrice. Both the Triclosan/copolymer and placebo dentifrices contained 0.243% sodium fluoride in a silica base. Subjects were stratified into two balanced groups according to baseline plaque and gingivitis scores. They then received an oral prophylaxis and were assigned to use either the Triclosan/copolymer dentrifrice or the placebo dentifrice for the next 6 months. Subjects were evaluated for supragingival plaque formation and gingivitis after 3 and 6 months' use of the dentifrices. After 6 months, the Triclosan/copolymer dentifrice was shown to provide a 32.32% statistically significant reduction (99% level of confidence) in supragingival plaque formation, as compared to the placebo dentifrice. Also, after 6 months, the Triclosan/copolymer dentifrice was shown to provide a 25.64% statistically significant reduction (99% level of confidence) in gingivitis, as compared to the placebo dentifrice. It thus can be concluded from this clinical study that the twice daily use of the dentifrice containing 0.3% Triclosan and 2.0% of a copolymer in a 0.243% sodium fluoride/silica base provides statistically significant reductions in both supragingival plaque formation and gingivitis.

[Transrectal ultrasonography in the study of recurrences in patients surgically treated for rectal neoplasms].

The value of transrectal US is known in the preoperative staging of rectal cancer but remains debated in the follow-up of the patients submitted to anterior resection or local therapy. The authors report their experience with the postoperative follow-up of 80 patients submitted to 125 transrectal US exams to study method reliability. The results were 9 true positive, 2 false positive, 113 true negative and 1 false negative cases, with 90% sensitivity, 98.3% specificity and 97.6% accuracy rates. Positive predictive value was 81.8% and negative predictive values was 99.1%. Twelve patients were submitted also to MRI which correctly diagnosed one false negative result of transrectal US. Twenty-one patients were examined also with transrectal Doppler and color-Doppler US: in rectal cancer recurrences the peak velocity of hemorrhoid vascular flow was higher than in non-recurrent patients. On the basis of our results, transrectal US deserves to be included in the postoperative follow-up of the patients submitted to anterior resection or to local therapy for rectal cancer. Moreover, according to our preliminary findings, Doppler and color-Doppler US can improve transrectal US reliability in detecting local recurrences.
