[A single-stage 2-layer suture in cesarean section--the effect of the surgical technic on postoperative febrile conditions].

The aim of the authors is to study the influence of the figure-of-eight suture of the uterine wall on the postoperative febrile complications. 1018 women, delivered with Caesarean were followed. 937 of them with conventional two layer suture of the uterine wall and the rest 81 with a figure-of-eight suture. Postoperative febrile complications were 15.58% in those with two layer suture against 3.5% in the rest 81. Using dispersion analysis the authors point that the on the type of uterine wall suture depends the rate of the postoperative febrile complications. The rate of influence of the figure-of-eight suture is up to 10%. Having in mind that this factor "suture", should be considered as 99% influential with a risk for mistake no more than 1%--F (8.06) > FT (6.64). The results presented show that the figure-of-eight suture is to be preferred to the conventional two layer suture of the uterine wall.

[Further comment on the problem of first-time delivery at an age over 30].

The authors concentrate on operative delivery--caesarean section, vacuum extraction and forceps of primiparous women over 30 years of age. Patients are divided in three groups by age--up to 29 years, 30-34 years and over 35. The data from 1992 is compared to already known results concerning the period 1983-1989. A rise in the percentage of caesarean section in the third group--over 35 up to 47.60% is found. The authors point that this is a result of usually unnecessary assurance in connection with age. There is no change in the operative delivery rate in the other two groups. The authors propose further discussion on the problem.

[Extrauterine pregnancy--the clinical experience of using a new differential-diagnostic immunological test].

The authors report results of immunologic test, that has been used to help the decision in some difficult for differential diagnosis cases. Two immunologic parameters were investigated--the amount of circulating immune complexes (CIC) and the lymphocytic nuclear coefficient (LNC). The results are put on a specially designed nomogram. The combined test used in the Department of "Obstetrics and Gynaecology"--Pleven, gives excellent results. The advantages of the test are discussed and a clinical case is presented as an illustration.

[The role of cardiotocographic study in older primiparae].

The authors aimed to study the significance of cardiotocographic examination (ECTE) for determination of fetal risk in elderly primiparas (EP). Data on NST, one the one hand, and the state of the newborn and perinatal outcome on the other, were, compared for its realization. 207 primiparas were investigated, 73, of whom were EP (35 years). The fetal reactivity and variability of fetal heart frequency (FHF) were evaluated during the performed NST. Semiquantitative evaluation of FHF was also made, in order to improve the quality of NST. FHF score was examined as well. The following inferences were made as a result of the examination: 1. Lacking reactivity of the fetus, variability 5 min beats per min, FHF score--6 points and even curve of NST as a signal for fetal distress were found mainly in EP. 2. These cardiotocographic criteria were encountered in EP with accompanying and worsening the pregnancy diseases and should pray a leading role in determining obstetric management. 3. The performance of ECTE is compulsory in EP.

[The morphological findings in the myometrial arteries and in the placenta with reference to the age of primaparae].

The authors investigate primiparous women at various ages, searching for eventual changes in uterine blood vessels and in the placenta, which eventually could have unfavourable effect on the fetus. The study established progressive thickening due to fibrosis of the muscular layer of the myometrial arteries (in reduction of smooth muscular elements) as well as delay of maturation and differentiation of placental villi with advancement of age of the pregnant woman. These phenomena independently could be the cause for more frequent unfavourable outcome of pregnancy in elderly pregnant women.
