Juxtacortical osteogenic sarcoma of the jaws. Review of the literature and report of a case.

The juxtacortical osteogenic sarcoma is a rate tumor, especially in the jaws. Until now, only six cases have been reported. Before describing an additional case in the mandible, it was deemed appropriate to review the literature on juxtacortical osteogenic sarcoma in extracraniofacial locations as well. The data on juxtacortical osteogenic sarcomas of the long bones and those of the jaws are discussed.

Squamous cell carcinoma of the buccal mucosa. A review of 85 cases.

Of 85 cases of squamous cell carcinoma of the buccal mucosa, 61 were treated primarily and 24 secondarily. Sixty-one percent (39) were T3 lesions, and 59% (38) had regional metastases. Nearly half of the patients were more than 70 years of age. Thirty-one percent (19) of the primary patients were treated by surgery alone, 25% (15) by irradiation alone, and 36% (22) by various combinations. The absolute five-year cure rate was 45% for the whole group, and 54% for the primarily and 40% for the secondarily treated patients. For each individual T and N category, the results of surgery were superior over those of radiotherapy. Intra-arterial chemotherapy followed by radiotherapy is to be considered in advanced cases, where composite resection on general grounds is not contemplated. There was a 14% incidence of second primary cancers; these were mainly located in the oral cavity.

Experiences of patients who undergo laryngectomy.

Fifty patients undergoing laryngectomy are described. The sociological and psychological consequences of the operation for the patient and his relatives are described, and the factors which govern the development of oesophageal speech.

Dose response in radiotherapy for glottic carcinoma.

All consecutive patients with glottic squamous cell carcinoma treated primarily by radiotherapy from 1965 through 1974 have been analyzed to reveal response of the primary tumor in relation to the dose. This study appears to indicate presence of dose response for all stages of the primary lesions (T) of the glottic carcinoma except for the smallest ones namely TIS, T1a and T1b. The total number of advanced tumors is small and selected. But they tend to indicate presence of dose response. Out of 15 selected patients with T3 and T4 glottic tumors, 5 (33%) had attained local control by primary radiotherapy; two had distant metastasis at onset of therapy, while 5 (33%) needed salvage surgery for control of the residual or recurrent disease. The ultimate success rate is 66% (10 out of 15) for the patients with advanced tumors treated with a conservative approach. An ongoing study indicates similar, if not better preliminary results with high dose radiotherapy.

Squamous carcinoma of the oropharynx.

One hundred and ten patients with an oropharyngeal carcinoma seen in the period 1961--1971 at the Antoni van Leeuwenhoek Hospital in Amsterdam are analysed. Approximately 75% of the patients were older than 60 years. About 60% of the patients had a cervical lymph node metastasis at the time of admission. Virtually all patients were treated exclusively by radiotherapy. Of the total group, the absolute survival was 26%, for tumours of the tonsil 35%, and for the base of the tongue 17%. There was no significant correlation between the histological degree of differentiation and the survival. The possibility of improving the results of selected patients is discussed.

Reconstruction after mandibulectomy for cancer.

Reconstruction after mandbulectomy for cancer presents a major problem in head and neck surgery. This study reports the experiences with a planned staged method of reconstruction that aims at both cosmetic and functional rehabilitation and that is based on a close cooperation between surgeon and prosthodontist. Stabilization of the remaining mandibular fragments by means of intermaxillary fixation at the time the tumor is resected, delayed bone grafting, and preprosthetic surgery to allow the use of a functional denture are the high points of the method. The results in 58 patients are discussed.
