An autopsy case of adult T cell leukemia associated with hypercalcemia and hyperplastic parathyroid glands is reported. A 51-year-old man complained of systemic lymphadenopathy and hepato-splenomegaly; a diagnosis of adult T cell leukemia was made based on clinico-pathological findings. The pathogenesis of hypercalcemia in adult T cell leukemia is discussed. However, cases associated with hyperplasia of the parathyroid gland are very rare. We emphasize the role of the parathyroid gland in the pathogenesis of hypercalcemia in adult T cell leukemia.

Alkaline phosphatase in the brush border of areas of intestinal metaplasia of human stomach was studied cytochemically. All absorptive cells in the upper part of the villi of the duodenum had strong alkaline phosphatase activity but, in areas of intestinal metaplasia, the metaplastic glands consisted of alkaline phosphatase-positive and negative absorptive cells. Alkaline phosphatase activity was found in tall dense microvilli of absorptive cells in areas of intestinal metaplasia and in the duodenum. However, in some areas of metaplastic epithelium the activity was very weak in some tall dense microvilli of absorptive cells but strong in those of neighbouring absorptive cells. No alkaline phosphatase activity was found in short sparse microvilli of absorptive cells in areas of intestinal metaplasia. The difference in alkaline phosphatase activity in microvilli of different cells in areas of intestinal metaplasia, which is not seen in the duodenum, indicates abnormal morphological and enzymatic differentiation intestinal metaplasia.

The enzymatic activity and distribution of peroxisomes (microbodies) in rat and guinea pig hearts were studied cytochemically, by means of oxidation of 3-3'-diaminobenzidine (DAB) and by using B-glycerophosphate and cytidine-5'-monophosphate as substrates. Peroxisomes were localized in proximity to mitochondria and sarcoplasmic reticulum and measured from 0.2 micrometers to 0.5 micrometers in diameter in both animal species. DAB positive bodies were seen both at pH 9.0 and pH 5.0 in rat myocardial cells. However, in guinea pig myocardial cells the reaction was observed only at pH 9.0, or very faintly at pH 5.0. Acid and alkaline phosphatases were not demonstrated in the peroxisomes. Lipid droplets were surrounded by a ring of dense granular reaction product for enzymes, such as acid and alkaline phosphatase, and lipofuscin granules were limited by acid phosphatase or DAB reaction products. The pathophysiological function of peroxisomes is discussed.

The authors have studied the endothelial changes in the aorta and the arteries of heart, kidney and other organs in cholesterol fed rabbits and endotoxin administrated rats through electron histochemical and vascular clearance methods. The vascular changes are variable presumably due to the functional and morphological difference of vessels. The administration of cholesterol and endotoxin may result in increased vascular permeability which is directly related to the qualitative increase and decrease of hydrolytic enzymes in the vascular wall. The permeability of high molecular substances was enhanced with the widening of the intercellular space in contraction of endothelium and the changing of molecular structure of plasma membrane accompanied with plasma lipid, platelets and hemodynamic stress. This fact was confirmed by use of horseradish peroxidase and dextran fesin as tracers of different molecular sizes. The authors emphasize that direct injury and subsequent abnormal contraction of the endothelial cell may initiate increased permeability of plasma protein and lipid in the medial layer and eventually may develop and enhance arteriosclerosis.
