Storytelling: a holistic foundation for genetic nursing.

The article discusses the need for a paradigm shift in how genetic nurses think about genetic information and genetic testing. It conceptualizes a theoretical model to depict the relationships among storytelling, holism, and genetic nursing practice. Four themes are discussed to explain why storytelling is necessary for clinical genetics: knowing the patient as a whole person, honoring the preciousness of personhood and the essence of humanness, uncovering power imbalances that inhere in therapeutic relationships, and listening to people's values and beliefs that affect decision making. The benefit of storytelling for developing and disseminating knowledge in genetic nursing is examined.

A multiplicity of roles for genetic nursing: building toward holistic practice.

Nursing's unique contribution to human genetics is holism. There are many roles for basic and advanced practice nurses involved in genetics. Describing these roles makes it easy for nurses to learn more about genetic nursing and helps promote collaboration among professionals. Education for nurses about genetics helps foster greater nursing participation in human genetics. The goal is for nurses to be informed about genetics so that they can better understand people as whole persons whose growth and development, human response patterns, and biopsychosocial processes are affected by and have an effect on human DNA.

Serotonin transporter-blocking properties of nefazodone assessed by measurement of platelet serotonin.

This study aims to determine whether the newly introduced serotonin type 2 (5-HT2) antagonist nefazodone has serotonin (5-HT) transporter-blocking properties at clinical doses in depressed patients. Change in platelet 5-HT was used as an index of the cumulative peripheral 5-HT uptake blockade produced by nefazodone and selective serotonin reuptake inhibitors (SSRIs). Platelet 5-HT was measured before and during treatment with nefazodone (N = 6) or an SSRI (N = 10) in patients with major depression. Corresponding Hamilton Rating Scale for Depression (HAM-D) scores were also obtained over the course of the study. The results of the study demonstrated that the decrease from mean baseline platelet 5-HT after SSRI treatment was significantly greater than the change after nefazodone treatment (-88% vs. -3%; Mann-Whitney U-test, p = 0.0002). Pretreatment platelet 5-HT level, posttreatment platelet 5-HT level, nor the percent decrease in platelet 5-HT correlated with the percent change in HAM-D scores in either treatment group. In conclusion, therapeutic doses of nefazodone do not cause sustained 5-HT uptake inhibition at the platelet 5-HT transporter.

Ischemic preconditioning versus intermittent reperfusion to improve blood flow to a vascular isolated skeletal muscle flap of rats.

OBJECTIVES: We investigated whether intermittent restoration of blood flow just before reperfusion after ischemia could create beneficial effects similar to ischemic preconditioning, which involves intermittent stoppage of blood flow just before ischemia. METHODS: Male Sprague-Dawley rats were prepared with vascular isolated cremaster muscles, then subjected to 4 hours of ischemia and 60 minutes of reperfusion. Arteriole diameters and capillary perfusion were measured by using intravital microscopy. Four groups were used: (1) untreated, (2) ischemic preconditioning (IP), (3) intermittent reperfusion (IR), and (4) ischemic preconditioning plus intermittent reperfusion (IP+IR). RESULTS: Our results showed that IP significantly attenuated both ischemia/reperfusion-induced vasospasm and capillary noreflow. IR was effective in attenuating vasospasm in terminal arterioles. However, IR alone was unable to significantly attenuate capillary no-reflow. Combining both IP and IR achieved the best results. CONCLUSION: Our results suggest both ischemic preconditioning and intermittent reperfusion are useful techniques for attenuating ischemia/reperfusion injury.

When courts review medical appropriateness.

OBJECTIVES: The authors examined how the courts have responded to public and private insurers' use of medical appropriateness criteria to establish coverage and payment policies. METHODS: A structured review of all federal and state court health insurance cases decided between 1960 and June 1994 that involved a dispute involving medical appropriateness was performed. A total of 3,215 published court decisions were analyzed, of which 203 met the criteria of relevance and 124 explicitly mentioned medical appropriateness criteria. The main outcome variable was whether the court ordered the insurer to provide coverage. RESULTS: In 185 cases, a definitive decision was rendered, and the insurer was required to pay in 57% of the decisions. Whether the insurer relied on an assessment or not, whether the assessment process was formal or informal, and who conducted the assessment did not appear to influence courts' decisions, nor did the specificity of the coverage exclusion. Significant predictors of courts ordering coverage were court jurisdiction, contract language assigning discretion to the insurer, severity of patient's condition, and whether the treatment appeared to work for the particular patient. CONCLUSIONS: For practice guidelines to be accepted by the courts, it is more important to focus on how insurance contracts are written than on how medical assessments are performed.

A short echo 1H spectroscopy and volumetric MRI study of the corpus striatum in patients with obsessive-compulsive disorder and comparison subjects.

OBJECTIVE: It is likely that the corpus striatum is involved in obsessive-compulsive disorder (OCD). Prior studies have inconsistently found alterations in caudate volumes in patients with OCD. This study was undertaken in the hope that N-acetylaspartate and volumetric measures together would elucidate the presence and nature of corpus striatum volumetric abnormalities in OCD. METHOD: Thirteen patients meeting the DSM-IV criteria for OCD, who had been medication free for a minimum of 6 weeks, and 13 psychiatrically normal matched comparison subjects participated in the study. Short echo 1H magnetic resonance spectroscopy (1H-MRS) was used to measure levels of N-acetylaspartate and several other cerebral metabolites from a 4.5-cm3 volume in the left corpus striatum of all 26 subjects. Metabolite levels were estimated by fitting the time domain spectroscopy data with a noninteractive computer program. Volumes of the left and right head of the caudate nucleus in each subject were determined by semiautomatic segmentation of the volumetric images. RESULTS: N-Acetylaspartate levels from the left corpus striatum were significantly lower in the patients with OCD than in the comparison subjects. There were no differences in either left or right caudate volume between the two groups. CONCLUSIONS: Despite the lack of differences in caudate volumes between the OCD patients and the comparison subjects, the lower level of N-acetylaspartate in the left corpus striatum of the patients suggests reduced neuronal density in this region. Inconsistent volumetric findings among prior studies may reflect a poorer sensitivity of magnetic resonance imaging morphometry for detecting neuronal loss compared with 1H-MRS measurement of N-acetylaspartate.

Analysis of water in Autonomous Biological Systems (ABS) samples.

Several soluble components, peptidase and amino acids, and carbon isotopic ratio in the water retrieved from flight experiments of Autonomous Biological Systems (ABS) as well as ground control samples are analyzed to interpret the condition, dynamics, material balance of the ABS ecosystems. Organic carbons in flight samples were found to be more abundant compared with the control ones, which suggested the uniform ecosystems in low gravity might easily dissolve more soluble components. The Mir-1997 flight sample showed higher C/N ratio probably because of the dissolution of carbon-rich plant materials.

Microorganisms and plant of Autonomous Biological Systems (ABS) samples.

Distribution of microorganisms and cellular structure of an Autonomous Biological Systems (ABS) were studied with a special attention to the effect of space environments. Viable cell densities measured by the direct fluorescence microscopic method were in the order of 10(5) cells/ml for fractions 1 (upper suspension) and 2 (lower suspension), and 10(6) cells/ml for fraction 3 (sediments). These values were 10 to 100 times larger than the values obtained by the classical colony forming unit method. No difference between flight and ground samples was observed in the vertical distribution of viable microorganisms when fractionation and analysis were carried out after recovery. Intracellular distribution of chloroplasts in higher green plants, Ceratophyllum demersum, of flight samples was disturbed after 10 days of flight (24hrs/day light on). After 4 months of flight (Mir/STS-79/81) with 24 hrs light on, Ceratophyllum demersum was completely disintegrated. On the other hand, in the second 4-months-flight experiment with 16 hrs/day light on, Ceratophyllum demersum was only slightly deteriorated.

Behavior and reproduction of invertebrate animals during and after a long-term microgravity: space experiments using an Autonomous Biological System (ABS).

Aquatic invertebrate animals such as Amphipods, Gastropods (pond snails), Ostracods and Daphnia (water flea) were placed in water-filled cylindrical vessels together with water plant (hornwort). The vessels were sealed completely and illuminated with a fluorescent lamp to activate the photosynthesis of the plant for providing oxygen within the vessels. Such ecosystem vessels, specially termed as Autonomous Biological System or ABS units, were exposed to microgravity conditions, and the behavior of the animals and their reproduction capacity were studied. Three space experiments were carried out. The first experiment used a Space shuttle only and it was a 10-day flight. The other two space experiments were carried out in the Space station Mir (Shuttle/Mir mission), and the flight units had been kept in microgravity for 4 months. Daphnia produced their offspring during a 10-day Shuttle flight. In the first Mir experiment, no Daphnia were detected when recovered to the ground. However, they were alive in the second Mir experiment. Daphnia were the most fragile species among the invertebrate animals employed in the present experiments. All the animals, i.e., Amphipods, pond snails, Ostracods and Daphnia had survived for 4 months in space, i.e., they had produced their offspring or repeated their life-cycles under microgravity. For the two Mir experiments, in both the flight and ground control ecosystem units, an inverse relationship was noted between the number of Amphipods and pond snails in each unit. Amphipods at 10 hours after the recovery to the ground frequently exhibited a movement of dropping straight-downward to the bottom of the units. Several Amphipods had their legs bent abnormally, which probably resulted from some physiological alterations during their embryonic development under microgravity. From the analysis of the video tape recorded in space, for Ostracods and Daphnia, a half of their population were looping under microgravity. Such looping animals could be observed still at the end of the 4 month stay in space. No looping behavior was noted for Amphipods and pond snails.

Performance of a digital video camcorder for the Autonomous Biological System experiment onboard Space Station Mir.

A video imaging and recording system was utilized in the Autonomous Biological System experiment onboard the space station Mir. Video image of the mini-ecological system was successfully recorded. The whole system was retrieved to the ground after its operation in orbit for four months. Performance of the video system is summarized here together with technical problems encountered. Defects of pixel had been developed in the imaging device. Cause of these defects could be attributed to its exposure against space radiation. Auto white balance was another function of the camcorder that was deviated from normal range of its performance once in orbit but recovered to normal after a while. Possible use of imaging devices for dosimetry is proposed to record space radiation environment at the site close to the experiment.

Concluding remarks of Autonomous Biological Systems (ABS) experiments.

Team efforts for analysis on the Autonomous Biological Systems (ABS) space experiments are summarized here to conclude scientific findings, and to scope the extended studies in future. From the three experiments on Space Shuttle and Space Station Mir, a closed ecological system modeled by the ABS was verified to be capable of sustaining its members of animals and plants under space environment for a period of several months. The animals successfully completed their life cycle in space during the course of these experiments, this was the first time that the life cycle of higher organisms had been completed in space and ecological system. Importance of gravity for ecology was proven at the same time. Gravity is a dominant factor for ecology by formulating spatial patterns and distribution of members of ecological system. Under microgravity, the fate of ecological system was found highly sensitive against the variation of environmental factor, such as light illumination cycle.

Hydroxychloroquine reverses thrombogenic properties of antiphospholipid antibodies in mice.

BACKGROUND: Previous studies have demonstrated that human monoclonal and polyclonal anticardiolipin antibodies have thrombogenic properties in vivo. Using such a model in which these antibodies have been shown to increase both the size of an induced thrombus and the duration of time in which such a clot lasts, we investigated whether hydroxychloroquine alters the dynamics of such thrombus formation. METHODS AND RESULTS: Three groups of nine mice were injected with purified immunoglobulin G (IgG) from a patient with the antiphospholipid syndrome (IgG-APS) and then fed with hydroxychloroquine at various doses (100, 6, and 3 mg/kg body wt). Three control groups of mice were also studied, including mice injected with IgG-APS and then fed with placebo, as well as two other groups injected with IgG from normal human serum and fed either hydroxychloroquine or placebo. A standardized thrombogenic injury was subsequently induced in the femoral vein of each mouse and the area (size) of thrombus measured as well as the total period of time that thrombus was present. Mice treated with hydroxychloroquine and IgG-APS showed significantly smaller thrombi that persisted for a shorter period of time compared with animals treated with IgG-APS and placebo. CONCLUSIONS: Hydroxychloroquine significantly diminished both thrombus size and total time of thrombus formation in mice previously injected with IgG-APS.

A population-based study of reoperations after back surgery.

STUDY DESIGN: Longitudinal follow-up study of back surgery reoperations using an administrative database. OBJECTIVES: To identify population-based rates and factors that determine the need for reoperation after back surgery. SUMMARY OF BACKGROUND DATA: Reoperation after lumbar surgery has poorer results than the initial surgery, yet the population-based incidence and determinants of reoperation are not known. Reported rates of reoperation are derived from retrospective case series and range from 4% to 15%. There are conflicting data on the rate of reoperation after different types of initial surgery. METHODS: All patients who had back surgery in the Province of Ontario (population 10,000,000) between April 1990 and March 1991 were identified using hospital discharge abstracts and an ICD-9 code algorithm. Patients who had undergone prior surgery were excluded. Patients were observed from the index operation to subsequent readmission and reoperation with a maximal time to follow-up examination of 4 years. Basic demographic information and information regarding diagnoses, surgery performed, complications, comorbid factors, reoperation diagnosis, and surgery type were obtained. Patients were divided into surgical treatment groups, and their subsequent reoperations were identified. Multivariate analysis using proportional hazards modeling was conducted. RESULTS: The index surgery group consisted of 4,722 patients, of whom 449 (9.5%) underwent reoperations in the follow-up period. Complications from surgery were significantly higher in the fusion and fusion with decompression groups. The reoperation rate was not significantly different among individual surgery groups. Diagnosis, operation performed, complications after the index surgery, comorbid conditions, and sex did not predict the need for spine reoperation. Younger age was predictive of the likelihood of reoperation (P = 0.04) CONCLUSION: The incidence of reoperation after back surgery is independent of diagnosis and type of surgery performed. Despite different anatomic reasons for surgical intervention, the success of different types of surgery are not influenced by the factors identified in this study. More extensive surgery does not prevent nor predispose a patient to the need for further surgery.

Genomic DNA-based hMSH2 and hMLH1 mutation screening in 32 Eastern United States hereditary nonpolyposis colorectal cancer pedigrees.

Hereditary nonpolyposis colorectal cancer (HNPCC) is an autosomal dominant cancer syndrome characterized by early age of onset colorectal cancer (mean 45 years) as well as endometrial, urinary tract, and upper gastrointestinal adenocarcinomas. The HNPCC phenotype has been shown to segregate with germline mutations in the human homologues of the DNA mismatch repair genes MSH2, MLH1, PMS1, and PMS2. However, the majority of published DNA mismatch repair gene mutation surveys associated with HNPCC kindreds report multiple levels of preselection, including 2p and 3p chromosomal linkage analysis and the evaluation of microsatellite instability of proband colorectal cancers prior to mutation analysis. For this reason, the concise contribution of each of the known DNA mismatch repair genes to the HNPCC phenotype remains unknown. We report the results of a genomic DNA-based analysis of hMSH2 and hMLH1 germline mutations in 32 unrelated Eastern United States HNPCC kindreds. These families were selected for study on the basis of phenotype only. We identified three hMSH2 and six hMLH1 mutations in eight families, for a positive mutation rate of 25%. Two mutations were identified in one of the families. Four of the mutations detected have not been reported in the literature previously. One of the mutation-positive families is African American; the others were of European-American ancestry. These results provide a clarification of the contribution of hMSH2 and hMLH1 to the HNPCC phenotype and suggest that in the majority of Eastern United States HNPCC kindreds selected by phenotype alone, the molecular genetic basis for the disease remains unknown.
