Chlorzoxazone 6-hydroxylase and p-nitrophenol hydroxylase as the most suitable activities for assaying cytochrome P450 2E1 in cynomolgus monkey liver.

Western blot analyses of liver microsomes from 13 male and 12 female monkeys demonstrated that in each sample a variable amount of a cytochrome P450 (P450) protein, likely monkey P450 2E1, cross-reacted with anti-rat P450 2E1 antibodies. Therefore, the involvement of monkey 2E1 in the oxidation of typical substrates for 2E1 from other species, such as dimethylnitrosamine (DMN), p-nitrophenol (pNP), chlorzoxazone (CLZ), and aniline, was investigated. Kinetic studies using microsomes from five male and five female monkeys showed that CLZ and pNP hydroxylations were monophasic, with apparent KM values of 77 and 14 microgramsM, respectively, whereas aniline hydroxylation and DMN demethylation were multiphasic, suggesting that P450s other than 2E1 were involved in catalyzing the latter two reactions. When correlation analyses were performed using several monooxygenase activities determined in male and female monkey liver specimens, it was found that immunodetectable 2E1 contents were highly correlated (r >/= 0.75) with CLZ and pNP hydroxylations, weakly correlated (r = 0.6) with aniline hydroxylation, and not correlated with DMN demethylation or other monooxygenase activities; CLZ hydroxylation was strongly correlated with pNP hydroxylation, weakly correlated with aniline hydroxylation, and not correlated with DMN demethylation. Inhibition experiments showed that CLZ and pNP hydroxylations were immunoinhibited by 60-80% by anti-rat P450 2E1 and were inhibited by the prototypical 2E1 inhibitor 4-methylpyrazole with IC50 values of 1.5 and 13 microgramsM, respectively. In conclusion, the findings provide evidence that P450 2E1 is constitutively and equally expressed in male and female monkey liver and it exerts a major role only in hydroxylation of CLZ and pNP.

[Hereditary hemorrhagic telangiectasia (Osler-Rendu-Weber disease) Management of epistaxis and oral hemorrhage by Nd-Yag laser].

BACKGROUND AND AIMS: The paper reports the use of Nd-Yag laser treatment for some major otolaryngoiatric symptoms in hereditary hemorrhagic teleangectasia (Rendu-Osler-Weber disease), focusing on the importance of epistaxis and oral hemorrhage as a cause of continuous hemoreintegrative treatment, without forgetting the constant psychophysical stress undergone by these patients. MATERIALS AND METHODS: Photocoagulation of teleangectasia in the ENT district has been used by the authors since January 1994. A total of 8 patients, 5 males and 3 females, have been treated aged between 31 and 65 years old. During the first year specialist outpatient controls were performed every two months, after which the follow-up became four-monthly. RESULTS: The authors provide a detailed illustration of the methods used, the choice of power, and the advantages and results obtained, emphasising the outpatient nature of treatment, excellent patient tolerability and the low cost to the health service. CONCLUSIONS: In the light of these results, the Authors underline the unquestionable usefulness of Nd-Yag laser in the treatment of a few of the undoubtedly more stressing symptoms of Rendu-Osler-Weber disease.

Purification and characterization of three constitutive cytochrome P-450 isoforms from bovine olfactory epithelium.

Three constitutive forms of cytochrome P-450 (P-450s) were isolated from olfactory microsomes of cattle. The purified P-450s, designated P-450bov1, P-450bov2 and P-450bov3, were electrophoretically nearly homogeneous by SDS/PAGE and their apparent relative molecular masses were estimated to be 50000, 53000 and 51000 respectively. As indicated by several criteria including the N-terminal sequence and absorption spectra, the three olfactory forms of P-450 were distinct from each other and from all the other P-450s currently known in cattle. P-450bov1 and P-450bov2 were purified in the low-spin state, whereas P-450bov3 was in the high-spin state. Studies to evaluate, by Western blot analysis, the reactivity of these purified P-450s with antibodies raised against rat hepatic P-450 2E1, 2B, 1A and 3A and rabbit olfactory P-450NMa and P-450NMb showed that P-450bov3 strongly cross-reacted with anti-P-450NMb IgG, and P-450bov1 moderately with anti-P-450NMa IgG. As determined by immunoblots, P-450bov1 and P-450bov3 represented a great portion of the total olfactory P-450. In a reconstituted system with NADPH:cytochrome P-450 reductase and phospholipids, P-450bov1 was more active in the metabolism of xenobiotic compounds (i.e. O-de-ethylation of ethoxycoumarin and N-demethylation of hexamethylphosphoramide) than towards endogenous substrates (testosterone and progesterone). Conversely, P-450bov3 metabolized the xenobiotics at lower rates but exhibited total oxidation rates of the above sex hormones higher than those of P-450bov1. From the comparison of the catalytic, immunochemical and structural properties, it was inferred that P-450bov1 and P-450bov3 are the bovine orthologues of P-450NMa (2A) and P-450NMb (2G1) respectively, the only two olfactory P-450s previously purified from rabbit. P-450bov2, which showed low activity toward some exogenous and endogenous compounds, represents a novel purified olfactory hemoprotein possibly belonging to the 3A subfamily. These results are consistent with a specific presence of catalytically and structurally similar P-450s, at least for the major ones, in the olfactory mucosa of mammals.

A cladistic analysis of mitochondrial ribosomal DNA from the Bovidae.

There is a huge data base of genetic information for the domestic artiodactyl species Bos taurus (cow), Ovis aries (sheep), and Capra hircus (goat). However, the phylogenetic relationships of these economically critical taxa and their close relatives, family Bovidae, remain for the most part unresolved. In this report, we aligned new mitochondrial (mt) 12S and 16S ribosomal (r) DNA sequences from 26 bovid taxa with published sequences. Phylogenetic analyses of the more than 64 kilobases of mt rDNA from 57 taxa support a basal division in the Bovidae that separates Bos and its close relatives from Capra, Ovis, and their kin. As suggested by previous molecular and morphological studies, "antelopes" are a paraphyletic assemblage. Caprinae (sheep, goats, goat antelopes, and musk oxen) groups consistently with hippotragine and alcelaphine antelopes, while Bovini (cattle and buffaloes) clusters with tragelaphine and boselaphine antelopes. The traditional tribal subdivisions of Bovidae are supported in most cases, but there are exceptions within Caprinae and Antilopinae (gazelles and close relatives). The rDNA data consistently place the enigmatic genera Pelea, Pantholops, and Saiga, but the origin of Aepyceros, the impala, remains obscure. Combined phylogenetic analyses of the rDNA data with the skeletal characters of Gentry (1992) were used to assess the stability of the molecular results.

[Lipid pneumonia. Favorable outcome after treatment with intravenous immunoglobulins, steroids, cephalosporins].

Lipoid pneumonia. Recovery after treatment with intravenous immunoglobulins, steroids, cephalosporins. A case of lipoid pneumonia is described, due to accidental inhalation of cosmetic oil occurred in an infant 11 months old, which caused severe clinical and radiological features. The patient was treated with antibiotics, corticosteroids and intravenous immunoglobulins. Clinical and radiological recovery was achieved within 5 months and persisted at 9 and 20 months follow-up.

[Photocoagulation of the hypertrophic lower turbinates using ND:YAG laser: functional results].

The difficult nasal respiration due to hyperplastic hypertrophy of the turbinates is a broad topic of ongoing interest in otorhinolaryngology. The authors describe a study of 71 patients suffering from hyperplastic hypertrophy of the lower turbinates treated since 1994 by photocoagulation using a ND:YAG laser. The laser treatment was performed on an outpatient basis, under local anesthesia and assisted by the use of an Endo-Video Closed-Circuit Video-Camera. The authors explain the details of the procedure used, emphasize surgical precision and the numerous advantages, the scarcity of intra- and post-operative side effects, the lack of post-operative nasal plugging. In addition, they present the satisfactory functional results achieved. The patients treated were followed up for 6 months prior to surgery and 30 months after laser photocoagulation. After surgery the following tests were performed: fibroscopy, objective symptom evaluation, rhinomanometry, rhinometry, muco-ciliary transport time (MCT), IgAS assay and turbinate biopsy. The low cost of performing this procedure is certainly a plus. The results obtained show a significant and permanent improvement in nasal ventilation and this was both referred by the patients themselves and could be objectively revealed by the ongoing controls. In conclusion, photocoagulation using a ND:YAG laser is well accepted by both physician and patient. It can improve nasal ventilation while respecting the anatomic and functional integrity of the nasal fossae.

[Diagnosis in pediatric "allergic" patients].

BACKGROUND: A clinician who deals with allergic patients needs the help of in vivo and in vitro allergometric tests in order to decide difficult therapeutic choices. METHODS: In the section of Clinical Immunology of the Institute of Paediatrics (Palermo University) two allergologic tests, prick and MAST-CLA, have been studied retrospectively, they were performed on patients observed for allergic disorders over 5 years. The aim of this study was to evaluate the diagnostic capacity of these tests "on the field". The study population is made of 459 patients (age range from 6 months to 16 years) affected with asthma, rhinitis, presumably allergic dermatitis, association of asthma and dermatitis, urticaria, selected on the basis of anamnesis and clinical examination. Eosynophils, basophils, total IgE, specific IgE with DHS-MAST-CLA and prick tests (Bayropharm) for the allergens most frequently responsible of allergic disease in our country, have been evaluated. A double controlled assay of 1040 pairs of allergens with MAST-CLA has been performed. RESULTS: The agreement between prick and MAST appeared to be satisfactory for inhaled allergens. Eosynophils count and total IgE levels showed a significative difference between patients negative versus patients positive for at least one allergen. CONCLUSIONS: The pattern of the allergens most frequently positive changes with age and patients positive for pollens showed positivity for vegetal food allergens with a frequency higher than expected.

Recombinant human growth hormone treatment at low doses does not significantly change thyroid function in growth hormone deficient adults.

Pituitary-thyroid changes have been reported during recombinant human growth hormone (rhGH) therapy at the dose commonly used in prepuberty. We have previously demonstrated that low doses of rhGH were able to normalize body composition and both cardiac structure and function in growth hormone deficient adults (GHDA), without causing any of the side effects described when the GHDA were treated with doses commonly employed in the GHD children. The aim of this study was to evaluate the behaviour of pituitary-thyroid parameters in GHDA undergoing such a low dose of rhGH treatment. We studied 9 (2 females and 7 males, 25-34 yr) GHDA, 7 congenital and 2 acquired GH deficiency, before, during and after a 12-month rhGH treatment of dose of 10 micrograms/kg/day (= 70 micrograms/kg/week) divided in 3 injections, administered sc at 20:00 h on Monday, Wednesday and Friday, respectively. Thyroid deficiency and other hormonal deficiencies, when present, had been adequately corrected with replacement therapy. Serum IGF1, T3, T4, free-T3, free-T4, TSH, reverse-T3, T3/T4 and FT3/FT4 ratios were studied basally, every 3 months during the 12-month rhGH treatment and every 3 months for a period of 12 months off therapy. Analysis of variance (ANOVA) was performed as statistical method. All parameters (except IGF1) did not show any variation during and after rhGH treatment at low doses. The alterations of T3 and T4 metabolism, in the sense of a T3 increase and a T4 reduction, caused sometimes by rhGH treatment, could be due to the higher doses used and therefore should be considered another side effect, like artrhalgia, fluid retention, carpal tunnel syndrome, etc.

[Nd-YAG laser in otorhinolaryngology: our experience].

The Authors describe Nd-YAG Laser treatment in dealing with some of the most common hypertrophic-hyperplastic pathologies of the upper airways. Laser operations are performed under local anesthesia employing an Endo-Video-Telecamera in order to treat soft surgical targets not reachable otherwise. The Authors stress the numerous advantages of this techniques: high surgical accuracy, poor collateral intro- and post-operative effects, lack of post-operative intranasal packing, quick recovery time, absence of relapse, sparing of nasal physiology. Furthermore patient of the treatment acceptance and low sanitary expenses, are additional favorable factors. The Authors explain the details of the methods, emphasize the tolerability of the treatment and report the results obtained during two years of experience.

Low dose recombinant human growth hormone normalizes bone metabolism and cortical bone density and improves trabecular bone density in growth hormone deficient adults without causing adverse effects.

OBJECTIVES: Prolonged GH deficiency induces alterations in bone metabolism and structure. Trials in GH deficient adults (GHDA) employing high dose GH replacement therapy produced conflicting results, and caused several adverse effects. This prompted us to study the effects of rhGH treatment on bone metabolism and structure at lowest doses so far used. DESIGN: Nine GHDA (7 males and 2 females, aged 25-34 years) were studied before, after 12 months of rhGH treatment (70 micrograms/kg/week, divided into 3 injections, administered s.c. at 2000 h on Monday, Wednesday and Friday, respectively) and after 12 months off therapy. MEASUREMENTS: Serum IGF-I, IGFBP-3 bone-gla-protein (BGP), procollagen-III (PIIINP), PTH and vitamin D, and bone mineral density (BMD) at proximal (Prox) and ultradistal (Dist) sites of the radius were measured. RESULTS: Before treatment, IGF-I, IGFBP-3, BGP and PIIINP levels, as well as both Prox and Dist BMD, were significantly lower than in controls. GH therapy normalized all these parameters, except for the Dist value, which nonetheless increased. No significant changes in PTH and vitamin D variation were seen. After 12 months off therapy all parameters returned to pretreatment levels. CONCLUSIONS: Our results suggest that 12 months of rhGH treatment at the lowest doses so far used normalizes bone metabolism and cortical bone density, and improves trabecular bone density without causing adverse events.

Modifications of head turning and circling movement following sulpiride microinjections into nucleus accumbens in the rat.

The aim of this study was to determine whether a relationship exists between nucleus accumbens D2 receptors, circling behavior, and its first stage, the head turning. Rats were unilaterally lesioned in the substantia nigra with 6-hydroxydopamine and afterward treated with d-amphetamine IP following bilateral intraaccumbens microinjections (1, 5, 10 micrograms/0.5 microliters) of sulpiride, a D2 receptor antagonist. Computer-assisted video analysis allowed the study of some parameters (number of turns, type of turn, head turning duration, degree and speed) characterizing rotatory activity. Sulpiride microinfusion resulted in a dose-dependent decrease of the number of turns and head rotation speed and in a dose-dependent increase of head-turning duration. Two turn types were observed in relation to the animal's position: a large head-to-tail position with a short-diameter turn type following sulpiride microinjection, and a close head-to-tail position in relation to a wide diameter turn type in the control condition (saline). The results show a relationship between head turning parameters, circling behavior, and D2 receptors in nucleus accumbens, which may be also involved in the regulation of some mechanisms related to sensory-motor integration in the rat.

A paradoxical inhibition of androgenic hyperproduction by a Sertoli-Leydig cell tumour ovary.

A 17 year old woman was evaluated for amenorrhoea and severe hirsutism (Ferriman-Gallway index = 31). Pelvic ultrasound demonstrated a right unilateral ovarian mass (6 x 5 cm), whereas the computed tomography for the adrenal gland was normal. Endocrinological findings revealed normal concentrations of oestradiol, progesterone, dihydroepiandrosterone sulphate, cortisol, prolactin, follicle-stimulating hormone, luteinizing hormone and adrenocorticotrophic hormone (ACTH). Total testosterone, free testosterone, androstenedione and 17-hydroxy-progesterone concentrations, already elevated at basal conditions, did not increase after an ACTH test, whereas they decreased significantly after dexamethasone administration and increased after a human chorionic gonadotrophin test. Of all the tumour markers investigated, tissue polypeptide antigen and alpha 1-fetoprotein showed an increase in concentration. Selective venous ovarian catheterization indicated the presence of an androgen-producing tumour in the right ovary. The histopathological diagnosis was consistent with a Sertoli-Leydig cell tumour ranking between an intermediate and a poor grade of differentiation, with heterologous elements characterized by mucinous epithelium of the gastro-intestinal type. An endocrine evaluation performed postoperatively showed a normalization of all serum pathological hormones and tumour markers studied. Some particular aspects were focused on and discussed.

Longitudinal study of antibodies against thyroid in patients undergoing interferon-alpha therapy for HCV chronic hepatitis.

Seventy-five patients (50 M, 25 F), affected by chronic hepatitis caused by hepatitis C virus (HCV), without clinically overt thyroid disease, underwent r-interferon (IFN)-alpha-2a treatment (3-6 MU, 3 times/week) for 12 months. They were tested for thyroid function and for thyroid autoantibodies before (A), 6 (B) and 12 (C) months after the beginning of treatment and after 6 (D) months off therapy. Antithyroglobulin antibodies (Tg-Ab) and TSH were measured by IRMA, antiperoxidase antibodies (TPO-Ab), free T3 (FT3) and free T4 (FT4) by RIA, thyrotropin receptor antibodies (TR-Ab) by RRA. None of the patients showed TR-Ab positivity throughout the study. The number of the patients with one or both antithyroid antibodies progressively increased during treatment (A 10.7%; B 26.7%; C 45.3%) and decreased when off therapy (D 22.7%) with none of them positive for Tg-Ab alone (TPO-Ab 6.7%; Tg-Ab+TPO-Ab 16%). Tg-Ab increased during rIFN (median: A 29.0; B 35.0; C 73.0 U/ml) but decreased when off therapy (D 29.0 U/ml). Instead, TPO-Ab significantly increased throughout the study (A 1.0; B 3.0; C 6.0; D 7.0 U/ml). However, some patients showed for the first time an appearance of antibodies when off therapy. Five patients showed both antibodies and thyroid dysfunction: 2 at B, 2 at C, and 1 at D. Only 1 developed mild transient hyperthyroidism while the other 4 developed hypothyroidism, persistent however only in 1 case. Our study confirms that rIFN-alpha-2a frequently induces thyroid autoimmunity. TPO-Ab seems more useful than Tg-Ab in monitoring the immunological response.(ABSTRACT TRUNCATED AT 250 WORDS)

Accumbens microinjection of LY 171555 and sulpiride: effects on circling behaviour, in the rat.

In rats with unilateral 6-hydroxydopamine lesions of the substantia nigra (pars compacta) and amphetamine i.p. some parameters of the rotational activity, following intra-accumbens microinjection of D2 agonist (LY 171555) and antagonist (sulpiride), have been analyzed. An increase in the number of turns was observed with LY 171555, whereas sulpiride determined a decrease. The type of circling was also analyzed: D2 agonist induced a very close turn around the central axis of the animal and D2 antagonist a close turn in the centre of the cage. Data were compared with control condition, which exhibited a large turn around the walls of the cage. The results show that pharmacological manipulation of nucleus accumbens influences rotational behaviour with a specific role for D2 receptors.

PCR assays of variable nucleotide sites for identification of conservation units.

A number of authors have recently suggested that the best approach for identifying units of conservation is to follow a systematics model of character analysis (Amato, 1991; Cracraft, 1991; Vogler and DeSalle, 1994). This approach necessitates the use of an operational, typological, evolutionary species concept. The use of the phylogenetic species concept has the utility and philosophical logic appropriate for this task. Additionally, there is a large body of literature that uses this framework, along with a parsimony based character analysis to identify patterns of phylogeny (Cracraft, 1983; Nelson and Platnick, 1981; Nixon and Wheeler, 1990). While we advocate this approach, we recognize that one of its limiting factors is sample size. We propose that by selective direct sequencing plus rapid sampling of variable target characters by polymerase chain reaction (PCR) assays of specific sites, sufficiently large numbers of individuals can be accurately, inexpensively, and quickly surveyed for diagnostic characters. This procedure is demonstrated by a survey of variable nucleotide sites in the Caiman crocodilus complex.
