A seroepidemiologic study of hepatitis A in Spanish children. Relationship of prevalence to age and socio-environmental factors.

Three child population groups from the Madrid area were studied for anti-HAV antibodies. Analysis was carried out with respect to age and socio-environmental factors. The population understudy was composed of 156 children, with ages ranging from 1 to 14 years; they were stratified in three socio-environmental groups (white-family unit, gypsy-family unit and orphanage), and also divided into subgroups according to age. As a whole, an age-related increase in prevalence was found. The overall seroprevalence by socio-environmental groups was: gypsy-family unit 63%, orphanage 46%, and white-family unit 23%. Significant differences between groups appeared from seven years on, being more marked among the eldest subgroups. Among the factors evaluated, hygienic-sanitary conditions and overcrowding influenced the high prevalence rate found in the gypsy-family unit subjects, whereas overcrowding appeared to be responsible for the higher prevalence in orphanage residents, as compared to white-family unit children.

Pulsed and continuous Doppler evaluation of renal dysfunction after kidney transplantation.

Seventy-eight recipients, average age 36 years, of cadaver kidneys were studied to evaluate the usefulness of Doppler ultrasonography for diagnosis of common complications in renal transplant patients. The patients were divided in five groups: Control (normal renal function), acute rejection (AR), acute tubular necrosis (ATN), obstructive uropathy (OU) and pathological vasculature (PV); renal artery stenosis (RAS) and renal artery thrombosis (RAT). Pulsed Doppler ultrasonography (PDUS) was an effective method to diagnose RAS and RAT, but did not sufficiently differentiate between AR and ATN. Despite this, PDUS may be useful for follow-up of renal transplant patients as specific changes in the PDUS curves or differences in successively recorded patterns indicate abnormality, which may initiate more specific diagnostic methods.

[Epidemiological characteristics of bladder tumors in the Cuenca area throughout a seventeen-year period].

OBJECTIVE: Bladder cancer is such an important health problem and accounts for most of the surgical duty in the urology departments. We achieve to draw the features of patients diagnosed with bladder tumors in the Cuenca area throughout a seventeen-year period. MATERIAL AND METHOD: From January 1985 to December 2002 we recorded in the Cuenca area the entire urologic tumor recently diagnosed. The following features were recorded: gender, age, pathology, TNM classification, risk factors (alcohol, cigarette smoking and coffee), social demography (rural/ urban) and the presence of metastasis. RESULTS: We achieve significant differences within pathology, age at diagnosis, incidence, social demography and some risk factors throughout the seventeen-year period. CONCLUSIONS: More bladder cancers are diagnosed along this time, more of them are infiltrating, in an aging population and from a rural precedence although a slightly decrease in the overall population in this are has been documented.

Sandfly Lutzomyia longipalpis in a Cutaneous leishmaniasis focus in central Colombia.

Lutzomyia longipalpis, 15 other species of the genus Lutzomyia, and one species of Brumptomyia were collected in an endemic focus of cutaneous leishmaniasis in a river canyon 450 m above sea-level, in Rio Claro, Antioquia, Colombia. The presence of Lu. longipalpis is associated with the destruction of the primary forest and the development of new farmland and rural settlement in this region. The composition of species identified a different habitat for Lu. longipalpis in Colombia. Lu. yuilli and Lu. longipalpis were predominant (68.26%) followed by Lu. trapidoi, Lu. hartmani, Lu. triramula, Lu. panamensis, Lu. gomezi.

Amaranthus leucocarpus lectin recognizes human naive T cell subpopulations.

Amaranthus leucocarpus lectin (ALL) is specific for GalNAc residue found in the inner core of Gal beta 1,3GalNAc alpha 1,O-Ser/Thr disaccharide (T-antigen) or GalNAc alpha 1,O-Ser/Thr (Tn-antigen). Flow cytometric analysis using fluorescein-labeled lectin and monoclonal antibodies against human cell surface markers indicated that 5.7% of mononuclear cells from human healthy donors are recognized by ALL. These cells have the phenotype CD2+CD4+CD19- and most of the lymphocytes recognized are also CD27+, CD45RA+ and CD43+. ALL possesses mitogenic activity on lymphocytes after neuraminidase treatment. Our results indicate that the receptors recognized by ALL could be considered surface markers for naive human T lymphocyte subsets.

The use of digital implants for the correction of hammer toe deformity and their potential complications and management.

This manuscript deals with use of silicone implants as part of the correction for hammer toe deformity. A brief description of the historical correction of hammer toe is presented, with emphasis placed on the development and use of silicone prostheses. The surgical procedure is discussed and an overview of the authors' long-term results are included. Analysis of an informal survey taken of several practitioners known to utilize digital implants is also presented. This manuscript primarily focuses on potential complications that may accompany the procedure. Management and prevention of these complications is also discussed.

[Aortic-esophageal fistula caused by inflammatory pseudotumor of the esophagus].

A 56-year-old, otherwise healthy man died in operative room from a massive esophageal hemorrhage, one week after a single episode of postprandial odynophagia. Surgical specimen of the esophagus and the aorta showed a fistulous tract extending from an esophageal tumoral mass to the aorta. Histological examination disclosed the microscopic finding of the so-called inflammatory pseudotumor of the esophagus (IPT). To our knowledge, this is the first reported case of such unusual complication of IPT, resulting in fatal gastrointestinal bleeding.
