Thrombolytic therapy for thromboembolism of vertebrobasilar artery.

Thrombolytic therapy was carried out on four patients with brain stem stroke due to thromboembolism of the vertebrobasilar system. Diagnosis was confirmed by angiography. Clinical and instrumental findings indicated a very poor prognosis. Two of these patients were treated with urokinase and the other two with streptokinase for periods ranging from 16 to 44 hours. The interval between onset of symptoms and start of therapy was less than 10 hours for three of these patients. All made a gradual and almost complete recovery after only a few hours of treatment, and only one presented minimal residual neurological disability. The condition of the fourth patient, who started therapy four days after the first episode, remained unchanged until he died 16 hours later. Our observations suggest that thrombolytic therapy may be useful in treating recent acute life-threatening brain stem stroke.

Ultraviolet spectra of membrane-rich human platelet particulates. Normal data and specular abnormalities in hypo- and hyperbetalipoproteinemia.

The analysis of UV spectra of human platelet particulates showed the existence of an evident protein component ordered in alpha-helical conformation in normal subjects. This ordered component was enhanced in type IIa hyperbetalipoproteinemia and, on the contrary, very low in a case of heterozygous familial hypobetalipoproteinemia. In the latter condition almost all proteins were indeed disordered as random coils. These data thus suggest that cholesterol significantly contributes to the control of molecular arrangement of platelet membrane proteins.

Persistent spontaneous heparinaemia in systemic mastocytosis.

A case of systemic mastocytosis with lymphatic, digestive, nervous and bone involvement and with persistent heparinaemia is described. The true heparin nature of the circulating anticoagulant was proved by the conventional titrimetric method with protamine sulphate and by the new specific amidolytic assay of its Xa-inhibiting properties. This human circulating heparin displayed a low specific activity as expressed by its activity/weight ratio.

Infusion of vincristine-loaded platelets in acute ITP refractory to steroids: an alternative to splenectomy.

Three infusions of vincristine-loaded platelets were carried out in a young man with acute idiopathic thrombocytopenic purpura (ITP) refractory to steroids. This procedure, used until now only for chronic refractory ITP, insured a remission without side effects. After 2 years of follow-up the patient has not had relapse. A potentially dangerous splenectomy was thus avoided in a phase of the disease in which a spontaneous remission was still very probable.

Acute thrombocytopenia following oral cholecystography with iopanoic acid.

We report a case of severe thrombocytopenia following oral cholecystography with iopanoic acid, a widely used contrast medium. A bone marrow aspirate showed increased numbers of megakaryocytes, suggesting a thrombocytopenia due to enhanced peripheral destruction of platelets; this examination was not reported in the previously described two cases. Although hematological reactions due to iopanoic acid are very rare, it would be wise to question patients regarding any preceding drug-induced purpura before proceeding with oral cholecystography.

Effect of pentoxifylline on platelet aggregation.

The effect of various concentrations of pentoxifylline, a methylxanthine, on platelet aggregation was evaluated in vitro by Born's method. The study was performed on platelet-rich plasma obtained from two series of clinically healthy subjects: in the first, pentoxifylline was added to platelet-rich plasma just before ADP or epinephrine; in the second, pentoxifylline was incubated with platelet-rich plasma for 150 minutes at 37 degrees C before testing. Statistical analysis of the results demonstrates that pentoxifylline exerts a dose-dependent inhibiting action on platelet aggregation, particularly when epinephrine is chosen as the aggregating agent, and that the pre-incubation of pentoxifylline with blood platelets greatly enhances its inhibitory effect.

Heparin assay: a comparison between titrimetric and amidolytic methods.

Titration assay of heparin with protamine sulfate permits the quantitative measurement of the concentration of the heparin molecule; on the contrary, the amidolytic method allows only a determination of the biological activity of heparin because it is subjected to the interferences of antiheparin factors. These may even be assayed with the amidolytic method by the evaluation of the residual activity after the addition to plasma of known amounts of heparin. The combined used of the two methods may prove useful in the assessment of concentration and specific activity of unknown heparin solutions in mediums free of antiheparin activities.

Mucus-secreting 'signet-ring' cells in CSF revealing the site of primary cancer.

A case is reported of leptomeningeal carcinomatosis in which identification of mucus-secreting 'signet-ring' carcinoma cells in the CSF allowed diagnosis of an otherwise asymptomatic gastric cancer. When lumbar puncture is performed, careful cytological examination of the CSF should be carried out in any undiagnosed patient with neurological symptoms and signs.
