[Reconstructive microsurgery in traumatic amputations of wrist and fingers (authors transl)].

A specialised department for vascular microsurgery was established in the Research Institute for Clinical and Experimental Surgery in 1973. 58 replantations of fingers in 42 patients were made. Total percentage of healing was 52 (30 fingers). Doppler's flowmetry is very useful for the control of microanastomoses of the arteries in fingers. Reconstruction of vessels in fingers was made with an operative microscope (magnification 16--24 times), special microtechnique and 10/0 sutures. Successful replantation is possible even 10--12 hours after severance, it the finger is kept in hypothermia at + 4 degrees C. Rapid admission of the patients to the hospital, i.e. improved information and emergency transport may shorten ischaemic time. The first stage of replantation is the marking of arteries and veins in the finger and in the stump. It is essential to suture not less than 2--3 veins for one artery and nerve. Thorough rehabilitation is of utmost importance to get the best functional results.

[Determination of local muscular blood flow in the hindlimbs of dogs following electrolysis of conducting vascular prostheses previously implanted in the abdominal portion of the aorta].

Peripheral blood flow in the posterior limbs in electrolysis of electroconductive vascular prosthesis preliminarily implanted into the abdominal aorta was experimentally determined on 25 mongrel dogs. Following reestablishment of the main blood flow a positive electrical potential (3--4 V) was fed on the prosthesis by means of a current conductor. Tissue blood flow was radiographically studied with xenon-133. Results of this investigation showed that during feeding the positive potential to the electroconductive prosthesis an increase of tissue blood flow occurred in the canine posterior limbs. After the cessation of the current feeding to the prosthesis the tissue blood flow occurred in the canine posterior limbs. After the cessation of the current feeding to the prosthesis the tissue blood flow decreased to the initial values. A conclusion was drawn that a positive potential from the source of direct current should be fed constantly to the electroconductive prosthesis or prolonged stable tissue blood flow increase.

[Indications for reconstructive surgery following occlusion of the popliteal artery and crural arteries].

Applications of optic systems and microsurgical technic enables one to perform reconstructive operations on arteries of small calibre - 1.5--1,8 mm (crural arteries). Clinically, 42 reconstructive operations (autovenous bypass shunting) were performed on crural arteries in 42 patients. After the surgical intervention the immediate positive effect with patency of the shunt and shunted artery was noted in 24 patients. In 18 patients thrombosis of the shunt developed in early postoperative terms.

[Thrombectomy in acute thrombosis after reconstructive operations on the arteries of the limbs].

While performing 152 reconstructive operations for obliterating lesions of large arteries of the lower extremities, the authors observed early acute thromboses of the graft in 12 patients (following operations on the femoropopliteal region and crural arteries). Of 12 patients 7 subjects were reoperated upon. In 6 cases indirect thrombectomy from the autovenous shunt or endarterectomized arterial segment by a baloon catheter proved to be quite feasible. The patients were discharged with adequate compensation of the extremity circulation. In one case there was obliteration of the shunt. In 5 patients treated conservatively for acute thrombosis of the graft a gangrene of the limb developed, and it had to be amputated.
