[Fundamental s of anesthesiology-reanimation. Part II. Principles of training].

The patient's reconvalescence, who fall ill suddenly or is injured or operated on, is impossible without the organism biological integrity (OBI) preservation or restoration and this constitutes the essence of the anesthesiology-reanimatology (AR) subject. The OBI is regarded as a process of necessary realization for the morphostructural balance preservation and of homeokinetic interaction of biological and energodynamical potential of organism. The OBI weak chains are determined, which are responsible for the course and the progression rate of bioenergetic insufficiency, acidotic alteration and the morphostructural balance disorder. The technology is adduced for the injuries elimination using the system of the local integrity restoration and the restoration OBI system. The AR application would raise the clinician's methodological armamentarium and promote to improve the result of treatment of patients in critical state.

Homeostasis-securing therapy and statusmetria.

Statusmetria allows to define the effective levels of oxygen status. The conception of Syndrome Intensive Care (SIC) has exhausted itself and invariable outcomes of SIC of Multiorgan System Failure (MOSF) confirms that. Therefore, an alternative to SIC should be advanced. Efficiency of treatment has been assessed in 257 patients with MOSF using values of metabolic rate and ability of the organism to cover it by oxygen and substrate supply. Oxygen Pulse (OP) and Index of Efficacy of Oxygen Transport (IETO2) were monitored. Intensive care was considered to be Homeostasis-Securing Therapy (HST) if the energostructure deficit was eliminated and the necessary recovery of regeneration rate was restored. OP in patients with MOSF was 0.8 mL.m-2 or less and IETO2 was 2.4 units in SIC. We managed to maintain an OP of 1.0-1.7 mL.m-2 and an IETO2 of 1.9-2.4 units in HST. 41 patients out of 135 with MOSF survived in SIC and 96 patients out of 122 survived in HST. The efficiency of HST appeared to be twice as much as the efficiency of SIC. Conception of homeostasis-securing therapy is advancing. The conception provides restoration of regeneration rate due to the effective elimination of energostructure deficit compared to SIC. The concept may be a basis of a new technology for the treatment of MOSF.

[Early signs of decompression of the body in acute suppurative peritonitis and ways of increasing the effectiveness of intensive therapy].

In studying the oxygen regimen, circulation and metabolism in 95 patients with acute purulent peritonitis at the toxic stage, it was established that the disorders in transcapillary exchange and oxygen regimen were the early functional criteria for decompensation of the general state of an organism. Differential, in accordance with a phase of the course of the disease, inclusion into the complex of intensive therapy, beginning from the preoperative preparation and anesthesiologic aid, of ornid and ornid with alupent permitted to improve considerably the results of surgical treatment of the patients.

[Anesthesia and intensive therapy in acute suppurative peritonitis].

The investigation of indices of hemodynamics and oxygen regimen in 58 patients with acute purulent peritonitis has shown the programmed intravenous narcosis in combination with Darenthin to promote increasing the cardiac and stroke index. Darenthin included in the complex intensive therapy of peritonitis was found to increase the portal blood flow and to cause early reestablishment of the intestine peristalsis.

[Oxygen transport in general anesthesia with ftorotan and sodium oxybutyrate].

The functional capacity of the oxygen transport system was studied in 51 patients subjected to the operative treatment for the complicated ulcerous disease. The general inhalation anesthesia by fluothane resulting in a decreased oxygen transport might limit the adequate metabolism. The general intravenous anesthesia by sodium hydroxybutyrate is followed by a true hypometabolic effect without any decrease in the efficiency of the cardio-vascular, system and disorders in the functional capacity of the oxygen transport system.
